During August 27-September 1, 1998, 99 (12%) of 835 soldiers in one unit at a U.S. Army training center in El Paso, Texas, were hospitalized for acute gastroenteritis (AGE). Their symptoms included acute onset of vomiting, abdominal pain, diarrhea, and fever. Review of medical center admission records for AGE during the previous year indicated that fewer than five cases occurred each month. This report describes the outbreak investigation initiated on August 30 by a U.S. Army Epidemiologic Consultation Service (EPICON) team; the findings indicated the outbreak was caused by a Norwalk-like viru

Human Norovirus is currently the main viral cause of acute gastroenteritis (AGEs) in most countries worldwide. Nearly 50 years after the discovery of the "Norwalk virus" by Kapikian and colleagues, the scientific and medical community continue to generate new knowledge on the full biological and disease spectrum of Norovirus infection. Nevertheless, several areas remain incompletely understood due to the serious constraints to effectively replicate and propagate the virus. Here, we present a narrated historic perspective and summarize our current knowledge, including insights and reflections o

Between December 9, 1988 and January 28, 1989, there were four outbreaks of acute gastroenteritis in Saitama prefecture. Eighty-two of 123 persons (67%) attending four banquets in restaurants became ill: 44 cases attending three banquets were related to eating raw oysters, and 38 attending one banquet to eating sashimi. The most common symptoms were nausea, diarrhea, abdominal cramps, and vomiting. Average incubation periods were 29 to 32 hours long. Bacteriologic analysis of stool specimens did not reveal causative agents. Small round structured viruses were detected in fecal specimens from 1

OBJECTIVE: To investigate gastrointestinal infection episodes (GIEs) in relation to the appearance of islet autoantibodies in The Environmental Determinants of Diabetes in the Young (TEDDY) cohort.
RESEARCH DESIGN AND METHODS: GIEs on risk of autoantibodies against either insulin (IAA) or GAD (GADA) as the first-appearing autoantibody were assessed in a 10-year follow-up of 7,867 children. Stool virome was characterized in a nested case-control study.
RESULTS: GIE reports (odds ratio [OR] 2.17 [95% CI 1.39-3.39]) as well as Norwalk viruses found in stool (OR 5.69 [1.36-23.7]) at <1 year of a

It is now well known that several viruses are responsible for acute diarrhoea or gastroenteritis in both children and adults. These viruses are difficult to identify since most of them cannot be isolated by stool cultures on cells. The reality of proven reinfection by some of these organisms is not always clearly understood, even though the existence of several serotypes in the same group (notably rotavirus) can be blamed, and this explains why vaccines are difficult to develop.

More than 12 million international travellers contract diarrhea every year. Today more than 80% of the pathogenic agents can be found: virus (Rotavirus and virus of Norwalk), bacteria or parasites. Most of the aqueous diarrheas are due to E. coli secreting an enterotoxin which acts like the toxin of V. cholerae in stimulating adenylcyclase in the epithelial cells of the small intestine. This enterotoxin is bound to a plasmid and may be transferred to other enterobacteriaceae. In contrast, in the dysenteric form of diarrhea the bacteria penetrate into the mucosa and elicit inflammation and ulce

Noroviruses cause the majority of acute viral gastroenteritis cases that occur worldwide. The increased recognition of noroviruses as the cause of outbreaks and sporadic disease is due to the recent availability of improved norovirus-specific diagnostics. Transmission of these viruses is facilitated by their high prevalence in the community, shedding of infectious virus particles from asymptomatic individuals and the high stability of the virus in the environment. Currently, the spectrum of clinical disease and the understanding of host susceptibility factors are changing. Cases of chronic nor

Virus-like particles were produced with 4 genogroup I Norwalk-like viruses(NLVs) and 7 genogroup II NLVs by a baculovirus expression system, and used to detect the antibody to NLVs by enzyme-linked immunosorbent assays(ELISA). Very little cross reactivity was observed between the genogroup I NLVs and the genogroup II NLVs when the antibody ELISA was done with hyperimmune sera. Infections by several genotypes of the NLVs were thought to occur in oyster-associated acute gastroenteritis, while an infection by a single genotype of NLVs was found in the illness occurred in a hospital. A high preval

An acute gastrointestinal tract illness affected 213 (52%) of 407 campers and 64 (53%) of 121 staff members attending a boys' camp in the Catoctin Mountains of Maryland during the summer of 1981. Nausea was the predominant symptom for ill campers and staff members (73%), but more staff members experienced diarrhea (49%) than did campers (9%). Twenty-three individuals had more than one episode of illness compatible with the case definition. Eight of nine paired blood specimens from ill staff members showed a fourfold increase in antibody titer to Norwalk virus by radioimmunoassay. Environmental

Norovirus (NoV) is the leading cause of acute gastroenteritis (AGE) in many countries that have introduced universal rotavirus mass vaccination. This is the first study to report data on NoV strains in Estonia. We recruited 2249 children aged 0-18 years hospitalized for AGE in Estonian hospitals from February 1, 2015 to August 31, 2016. Norovirus gastroenteritis (NoVGE) was diagnosed in 14.5% (n = 325) cases. Stool sample for RNA extraction and genotyping was available in 86% (n = 280) of NoVGE cases (2015, n = 91; 2016, n = 189). Dominant capsid types detected in 75% (n = 210) samples were, G

The Norwalk, Snow Mountain (SMA), and Hawaii agents are etiologically associated with separate outbreaks of acute viral gastroenteritis. Previous cross-challenge of volunteers, immune electron microscopy, and/or enzyme-immunoassay analysis suggested that these agents are antigenically distinct. We examined paired sera from human volunteers challenged with these agents for the presence of homologous and heterologous serum antibody titer rises to the agents. Two-way cross-reactions occurred between Hawaii agent and SMA. A one-way cross-reaction occurred between Norwalk agent and SMA, as voluntee

Previous studies have shown bismuth subsalicylate (BSS) has antimicrobial properties, but few studies have addressed the mechanism of action. Furthermore, following BSS ingestion other bismuth salts form throughout the gastrointestinal tract including bismuth oxychloride (BiOCl) that also act upon enteric pathogens. To further understand the antimicrobial activity of bismuth in infectious diarrhea, the antimicrobial effect of BSS and BiOCl on Clostridium difficile, Salmonella, Shigella, Shiga toxin-producing Escherichia coli strains and norovirus (NoV) were measured. Bacterial enteric pathogen

From 1986 to 1988, 24 states and Puerto Rico reported 50 outbreaks of illness due to water that people intended to drink, affecting 25,846 persons. The protozoal parasite Giardia lamblia was the agent most commonly implicated in outbreaks, as it has been for the last 10 years; many of these outbreaks were associated with ingestion of chlorinated but unfiltered surface water. Shigella sonnei was the most commonly implicated bacterial pathogen; in outbreaks caused by this pathogen, water supplies were found to be contaminated with human waste. In the 1970s investigations generated a remarkable amount of information on the viruses responsible for acute gastroenteritis. The two viruses responsible for most cases occur in epidemiologically distinct clinical forms. Although occasionally epidemic, rotavirus infections are usually sporadic and primarily affect infants and young children. This illness is characterized by severe diarrhea commonly lasting five to eight days and frequently associated with upper respiratory tract symptoms, fever, and vomiting. The Norwalk agent and Norwalk-like viruses are characteristically epidemic and respo

Three viral gastroenteritis (VGE) outbreaks that occurred in 1998-1999, in Hungary were investigated for the presence of human caliciviruses (HuCVs). HuCVs in stool specimens were detected by reverse transcription-polymerase chain reaction (RT-PCR) using primer pair 289/290, which was designed based on the RNA-dependent RNA polymerase (RdRp) sequence. RT-PCR results were confirmed by sequencing showing that all three outbreak strains belonged to genogroup II of "Norwalk-like viruses" (NLVs). Two strains had high sequence identity with strains in known genetic clusters (Hawaii and Lordsdale clu

Children are at a greater risk of infections from serious enteric viral illness than adults for a number of reasons. Most important is the immune system, which is needed to control the infection processes. This difference can lead to more serious infections than in adults, who have fully developed immune systems. There are a number of significant physiological and behavioral differences between adults and children that place children at a greater risk of exposure and a greater risk of serious infection from enteric viruses. Although most enteric viruses cause mild or asymptomatic infections, t
