Degenerative disc disease is strongly associated with low back pain, making it a leading cause of disability. With injury and age, cellular remodeling of the disc tissue leads to compositional changes, stiffening, and loss of stress relaxation, particularly in the central gelatinous nucleus pulposus (NP) region of the disc. As part of this extracellular matrix (ECM) remodeling, there is an increase in the deposition of fibronectin, a strongly adhesive integrin ligand that is known to regulate inflammatory signaling. However, it is unclear how these pathological changes in cellular adhesion reg

A 9-year-old Yorkshire terrier was brought to the emergency department for inability to maintain the correct station with acute onset. Neurological examination showed a non-ambulatory tetraparesis, spontaneous proprioceptive deficit in all limbs, and decreased flexor reflex in the forelimbs. The neurological symptoms suggested a cranial cervical spinal cord with suspicion of spinal shock. The clinical differential diagnoses included degenerative (intervertebral disc extrusion), vascular, inflammatory, or neoplastic disease. No pathological findings were evident in the hematobiochemical tests o

Intervertebral disc degeneration (IDD) is a common cause of low back pain. Understanding its molecular mechanisms is the basis for developing specific treatment. To demonstrate that miR-22-3p is critical in the regulation of IDD, miRNA microarray analyses are conducted in conjunction with in vivo and in vitro experiments. The miR-22-3p knockout (KO) mice show a marked decrease in the histological scores. Bioinformatic analysis reveals that miR-22-3p plays a mechanistic role in the development of IDD by targeting SIRT1, which in turn activates the JAK1/STAT3 signaling pathway. This is confirmed

BACKGROUND: Studies have shown that abnormal stress is a significant inducer of Intervertebral Disc Degeneration (IVDD). Although traction force is commonly used to delay IVDD, its effects on Nucleus Pulposus Cells (NPCs) and their secreted exosomes remain unclear. In addition, this study systematically revealed the relationship between miR-8485 and IVDD for the first time.
METHODS: Cellular experiments were performed using a Flexcell cell stretching platform to apply traction force to NPCs. After optimizing loading parameters, NPCderived exosomes (NPCs-exo) were isolated and subjected to miR

Notochordal cells (NCs), abundantly found in the developing nucleus pulposus (NP), show potential for intervertebral disc regeneration because of their unique instructive and healthy matrix-producing capacity. However, NCs are lost early in life, and attempts at in vitro expansion have failed because they lose their specific phenotype. Therefore, much effort is focused on the generation of cells resembling the properties of healthy matrix-producing NP-like cells from human induced pluripotent stem cells (hiPSCs). They are considered a promising alternative for employing native NCs. Given the o

BACKGROUND AND PURPOSE: Intervertebral disc degeneration (IVDD), a leading cause of low back pain, lacks effective disease-modifying therapies. Diabetes exacerbates IVDD risk, but the causal mechanisms and therapeutic potential of glucose-lowering agents remain underexplored. This study aimed to identify glucose-lowering drug targets strongly associated with IVDD using Mendelian randomisation (MR) and to validate the therapeutic efficacy and mechanisms of sitagliptin, an inhibitor of dipeptidyl peptidase 4 (DPP4), in alleviating IVDD progression.
EXPERIMENTAL APPROACH: Two-sample MR analysis

Intervertebral disc degeneration (IDD) represents a major global health challenge, primarily due to the inability of current therapies to reverse the progressive loss of nucleus pulposus (NP) tissue and function. Transplanting bioactive substitutes offers potential for overcoming this limitation. However, current strategies fail to generate large-sized, functional human NP substitutes, impeded by a fundamental trade-off: the incapacity to simultaneously achieve scalable expansion and maintain the essential cellular phenotype. Here, we found that human platelet lysate (hPL) acts not merely as a

Microplastics (MPs) have been detected in various tissues, yet evidence regarding the relatively isolated nucleus pulposus (NP) remains limited. This study investigated MPs in NP tissues from 51 patients undergoing lumbar disc herniation (LDH) surgery in Wuhan, China. Samples underwent chemical digestion and filtration, followed by candidate-particle screening using an operator-assisted micro-Raman spectroscopy mosaic imaging workflow and targeted Raman spectral assignment. Microplastic-sized polymer particles were Raman-assigned in five patients, including polyurethane foam, poly (propylene g

BACKGROUND: Low back pain remains the leading cause of disability worldwide, with intervertebral disc degeneration representing a major biological contributor. Although cell-based therapies have shown promise in preclinical models, clinical translation has yielded modest and inconsistent outcomes. Accumulating evidence suggests that therapeutic failure reflects not only limitations in cell source or differentiation potential, but also the hostile biochemical and biomechanical microenvironment of the degenerative disc. Hypoxia, nutrient deprivation, acidity, lactate accumulation, fibrosis, sene

Intervertebral disc degeneration is a major contributor to discogenic low back pain and is characterized by a pro-inflammatory and catabolic microenvironment within the nucleus pulposus. We previously showed that the phosphatase PH domain leucine-rich repeat protein phosphatase 1 (PHLPP1) is positively correlated with intervertebral disc degeneration and that its deficiency promoted nucleus pulposus cell survival and matrix homeostasis; however, its role in inflammatory signaling during intervertebral disc degeneration remained unknown. Here, we show that PHLPP1 functions as an upstream regula

Intervertebral disc degeneration (IDD) is characterized by an inflammatory environment and dysregulation of the extracellular matrix metabolism (ECM). Improving nucleus pulposus cells (NPCs) condition first requires a supportive surrounding environment. However, how to provide an excellent microenvironment before regulating the function of the NPCs is not yet a complete strategy. In this study, a sequential drug delivery system is comprised of chitosan hydrogel (G-CS) loaded with Angiotensin (1-7) and mesoporous silica nanospheres (MSN) coating with α-mangostin to achieve synergistic effects o

The nucleus pulposus (NP) of the intervertebral disc is an immune-privileged tissue. During intervertebral disc degeneration (IDD), this immune privilege is compromised, resulting in the exposure of NP components to the peripheral immune system, which in turn activates monocytes and elicits an immune response. In this study, we demonstrate that monocytes respond to NP immunogenicity by activating damage-associated molecular patterns (DAMPs), thereby initiating a sustained NF-κB-mediated inflammatory response in NP tissue and ultimately driving a vicious cycle of inflammation and oxidative stre

Intervertebral disc degeneration (IVDD) is a common imaging change, and it is characterized by increased production of inflammatory cytokines such as IL-1β and elevated degradation of extracellular matrix (ECM). IVDD has been indicated as the most important reason of low back pain and the leading cause of disability. IVDD is a common concomitant disease in patients with obesity; and obesity is a metabolic syndrome pathological condition that results in the concentrations of circulating fatty acid increasing and metabolic stress. Increasing circulating fatty acid could evoke a widespread inflam

It is yet unknown what causes intervertebral disc degeneration (IVDD), a chronic inflammatory systemic illness. Lower back pain may result from the disease's primary pathogenic characteristic, which is the degeneration of nucleus pulposus cells (NPCs). While IVDD can currently be improved by medications, there are still serious toxic side effects that require immediate attention. Consequently, the identification of novel therapeutic targets depends on a thorough examination of the mechanisms underlying the onset and progression of IVDD. Particularly in NPCs, annulus fibrosus, and cartilage end

Intervertebral disc degeneration is a major cause of neck and back pain, representing a significant global socioeconomic burden. The polysaccharide hyaluronan is key to maintaining disc physiology and mediating disc disease through its structural and biological roles in the nucleus pulposus, a component of the intervertebral disc highly susceptible to degeneration. In this study, we introduce a novel composite bioink designed for extrusion bioprinting of structures resembling the nucleus pulposus. Our bioink combines levels of hyaluronic acid and collagen that approach physiological concentrat

BACKGROUND: Intervertebral disc degeneration (IVDD) stands as a primary pathophysiological driver of low back pain, yet no therapeutic intervention effectively arrests its progression. Evidence shows that certain Sirt1 agonists may confer protective effects on intervertebral discs, but the underlying mechanisms remain unclear. This study aims to delineate the interaction between Sirt1 and the inflammatory microenvironment, offering potential novel avenues for IVDD prevention and treatment.

The decreased regeneration potential of aging nucleus pulposus resident progenitor cells (NPPCs) fails to resist intervertebral disc degeneration (IVDD), and strategies to remodel the regeneration capacity of senescent NPPC are urgently needed. A decrease in Klotho gene expression in NPPCs of both old mice and humans exacerbates the impaired regenerative functionality of NPPC. Here, an NPPC-targeted lipid thymine nanoparticle (NT-LNP) is reported for the in situ manipulation of the regenerative repair potential of NPPCs, restoration of degenerated nucleus pulposus tissue, and mitigation of IVD

Osteoarthritis (OA) and chronic low back pain due to degenerative (intervertebral) disc disease (DDD) are two of the major causes of disabilities worldwide, affecting hundreds of millions of people and leading to a high socioeconomic burden. Although OA occurs in synovial joints and DDD occurs in cartilaginous joints, the similarities are striking, with both joints showing commonalities in the nature of the tissues and in the degenerative processes during disease. Consequently, repair strategies for articular cartilage (AC) and nucleus pulposus (NP), the core of the intervertebral disc, in the

Bone-morphogenetic protein-7 (BMP-7) is a growth factor that plays a major role in mediating anabolism and anti-catabolism of the intervertebral disc matrix and cell homeostasis. In osteoblasts, Forkhead box protein C2 (FoxC2) is a downstream target of BMPs and promotes cell proliferation and differentiation. However, the role FoxC2 may play in degenerative human intervertebral disc tissue and the relationship between FoxC2 and BMP-7 in nucleus pulposus (NP) cells remain to be elucidated. This study aims to investigate the presence and signaling mechanisms of FoxC2 in degenerative human interv
