Behavioral Challenges and Adaptive Strategies in Providing Mealtime Care to Residents With Dementia: A Qualitative Study of Nursing Home Clinical and Administrative Staff.

This study examined how nursing home staff adapt to challenging mealtime behaviors in residents with dementia, such as turning away or refusing food. Ten focus groups were conducted with 52 clinical and administrative staff from three southeastern U.S. nursing homes. Transcripts were analyzed using four key codes: feeding behaviors, interventions, training, and communication. Staff described a range of mealtime challenges, including cognitive and functional decline, dysphagia, and elopement. Feeding-related behaviors like refusing to eat reflect the complexity of care required. Staff reported

Evaluating nurse preferences for a novel on-body delivery system vs. manual syringes for large-volume subcutaneous drug administration: a survey study.

While nurses report challenges with the manual administration of large-volume subcutaneous drugs, these challenges and potential solutions are not captured in the literature. In this cross-sectional study, 45 nurses with experience administering large-volume subcutaneous biologics completed an 18-item survey about preferences for syringes vs. on-body delivery systems. 100% responded that an on-body delivery system seemed easy to learn and use and preferable to syringes. In a drug delivery scenario including comprehensive administration details and assuming equivalent safety and efficacy, 97.78

Impact of a nurse practitioner-led dedicated outpatient parenteral antibiotic therapy clinic on patient outcomes and administrative workload: a retrospective cohort study.

BACKGROUND: Outpatient parenteral antibiotic therapy (OPAT) enhances patient safety, improves outcomes, and reduces healthcare costs by decreasing 30-day readmissions and adverse events. However, the optimal structure and follow-up protocols for OPAT programs remain undefined. Identifying high-risk patients for readmission and managing adverse drug events (ADEs) are critical components of OPAT care. OBJECTIVES: This study aimed to evaluate the impact of a dedicated OPAT clinic on hospital readmissions, and quantified the administrative workload required to manage patients on OPAT post-dischar

Electroencephalography-based evaluation of the impact of night-time nursing care on sleep during intensive care unit administration after cardiac surgery.

After highly invasive cardiac surgery, critically ill patients require night-time nursing care for life support. Even in patients administered dexmedetomidine for sedation, nursing care potentially disrupts sleep and affects circadian rhythms. However, the impact of night-time nursing care on sleep in intensive care unit (ICU) patients have not been objectively evaluated, and the effects of sedatives have not been objectively confirmed. The current study investigated how night-time nursing care affected sleep in patients who had undergone cardiac surgery, by analyzing dynamic changes in electr

Prolonged use of intravenous administration sets on central line associated bloodstream infection, nursing workload and material use: A before-after study.

OBJECTIVES: One of the interventions to reduce risk of central line associated bloodstream infection (CLABSI) is routine replacement of the intravenous administration sets. Guidelines advises a time interval that ranges between four and seven days. However many hospitals replace intravenous administration sets every four days to prevent CLABSI. RESEARCH METHODOLOGY: In this single centre retrospective study we analysed whether the extension of the time interval from four to seven days for routine replacement of intravenous administration sets had impact on the incidence of CLABSI and coloniza

Effectiveness of two distinct web-based education tools for bedside nurses on medication administration practice for venous thromboembolism prevention: A randomized clinical trial.

BACKGROUND: Venous thromboembolism (VTE) is a common cause of preventable harm in hospitalized patients. While numerous successful interventions have been implemented to improve prescription of VTE prophylaxis, a substantial proportion of doses of prescribed preventive medications are not administered to hospitalized patients. The purpose of this trial was to evaluate the effectiveness of nurse education on medication administration practice. METHODS: This was a double-blinded, cluster randomized trial in 21 medical or surgical floors of 933 nurses at The Johns Hopkins Hospital, an academic m

Intraoperative administration of tramadol for postoperative nurse-controlled analgesia resulted in earlier awakening and less sedation than morphine in children after cardiac surgery.

In adults, intraoperative administration of tramadol could result in earlier recovery and less sedation than morphine. In this controlled, randomized, double-blind study, we investigated whether an intraoperative initial dose of tramadol could cause more rapid awakening from general anesthesia, less sedation, and earlier tracheal extubation than morphine in children during the immediate postoperative period. Forty children aged 1-6 yr, scheduled for atrial or ventricular septal defect repair and tracheal extubation in the pediatric intensive care unit, were randomly allocated to receive morphi

A quasi-experimental effectiveness evaluation of a nurse-led Hospital Violence Intervention Programme in the United Kingdom using linked routine health and administrative data.

BACKGROUND: Emergency Department (ED) care of people injured in violence is increasingly seen as an opportunity to address patients' psychosocial vulnerabilities as well as their treatment. These vulnerabilities include alcohol, drug use, mental health and social vulnerabilities that are also associated with high levels of unplanned ED attendances for reasons other than violent injury. Since evidence of the effectiveness of interventions designed to reduce these risks is weak, we evaluated the impact of nurse-led Hospital Violence Intervention Programmes (HVIPs) across two sites in Wales, Unit

Salary compression of nursing managers.

An American Hospital Association survey has shown that rapid escalation of staff nurse salaries and benefits, in the wake of local and national shortages, has pushed clinical wage structures beyond those that determine the salaries of nurse managers and administrators. This leads to difficulty both in recruitment and retention of these key personnel. In this hospital, attrition of nurse managers into clinical positions has ceased. Their approach makes nurse managers eligible for differentials in addition to a modest economic incentive for assumption of managerial responsibilities.

Why board of nursing disciplinary actions do not always yield the expected results.

One of the ways boards of nursing serve to protect the public health, safety, and welfare is by removing from practice those nurses who fail to meet recognized standards of care or otherwise pose a public threat. Self-reporting and discovery through criminal background checks and reports from the court system or other regulatory bodies represent only small numbers of those disciplined. Most complaints investigated by a board are reported by nursing administrators, either chief nursing officers or nurse managers. Frequently, the reporting nurses provide the board with information from their own

Antipsychotic initiation and new diagnoses excluded from quality-measure reporting among Veterans in community nursing homes contracted by the Veterans Health Administration in the United States.

OBJECTIVES: To assess whether prevailing antipsychotic use rates in community nursing homes (CNH) influence new initiation of antipsychotics and diagnosis with antipsychotic indications among Veterans. METHODS: We used linked 2013-2016 Veterans Administration (VA) data, Medicare claims, Nursing Home Compare, and Minimum Data Set (MDS) assessments. The exposure was the proportion (in quintiles) of all CNH residents prescribed antipsychotics in the quarter preceding a Veteran's admission date. Using adjusted logistic regression, we analyzed two outcomes measured using MDS: antipsychotic initiat

Mother/offspring co-administration of the traditional herbal remedy yokukansan during the nursing period influences grooming and cerebellar serotonin levels in a rat model of neurodevelopmental disorders.

Neurodevelopmental impairment in the serotonergic system may be involved in autism spectrum disorder. Yokukansan is a traditional herbal remedy for restlessness and agitation in children, and mother-infant co-administration (MICA) to both the child and the nursing mother is one of the recommended treatment approaches. Recent studies have revealed the neuropharmacological properties of Yokukansan (YKS), including its 5-HT1A (serotonin) receptor agonistic effects. We investigated the influence of YKS treatment on behavior in a novel environment and on brain monoamine metabolism during the nursin

Expanding the Abortion Provider Workforce: A Qualitative Study of Organizations Implementing a New California Policy.

CONTEXT: Access to abortion care in the United States varies according to multiple factors, including location, state regulation and provider availability. In 2013, California enacted a law that authorized nurse practitioners (NPs), certified nurse-midwives (CNMs) and physician assistants (PAs) to provide first-trimester aspiration abortions; little is known about organizations' experiences in implementing this policy change. METHODS: Beginning 10 and 24 months after implementation of the new law, semistructured interviews were conducted with 20 administrators whose five organizations trained

Eye movement desensitization and reprocessing for occupational memories associated with prolonged depressive and trauma-related symptoms after COVID-19 administrative redeployment: a public health nurse case report.

Reports remain limited on individual eye movement desensitization and reprocessing (EMDR) for prolonged trauma-related symptoms linked to occupational memories after COVID-19 administrative redeployment among public health nurses. A woman in her 30s working as a public health nurse developed prolonged depressive symptoms, intrusive recollections, nightmares, and hyperarousal after reassignment to unfamiliar COVID-19 administrative duties. During inpatient treatment in 2021, EMDR was applied to early family-related memories in routine care, whereas occupational memories were deferred and later
