Indwelling urinary catheter use at the end of life: a retrospective audit.

This audit aimed to measure: the prevalence of indwelling urinary catheters in patients at the end of life; the use of nursing documentation relating to catheter insertion and care; and prevalence of continence screening on admission. The audit involved a retrospective examination of case notes of patients who died on two oncology wards and a hospice at a large teaching hospital in the south of England. The audit showed that 63% of patients had an indwelling catheter during their admission. Documentation relating to urinary catheter insertion and care was present in 75% of cases, and 75% of pa

Assessment and nonpharmacological management for patients with cancer anorexia-cachexia syndrome: a best practice implementation project.

INTRODUCTION AND AIMS: Cancer anorexia-cachexia syndrome (CACS) is a common multifactorial syndrome, which affects up to 80% patients with advanced cancer. At present, evidence to support the benefit of pharmacological intervention in the management of CACS is limited. Patients would benefit from standard procedures for early assessment and identification of cancer anorexia-cachexia, and using nonpharmacological strategies to manage patients with CACS. This best practice implementation project aimed to implement an evidence-based practice in assessing and managing patients with CACS, thereby i

Implementation and clinical audit of alcohol detoxification guidelines.

This article describes the implementation and clinical audit of alcohol detoxification guidelines at the Royal Cornwall Hospital. The progress in developing a hospital alcohol strategy as recommended by the Royal College of Physicians (2001) is also reviewed. The Clinical Institute Withdrawal Assessment for Alcohol (revised) (Sullivan et al, 1989) is used to assess the alcohol-dependent patient's baseline requirement for their detoxification regimen in the first 24 hours of their care. The prevention and treatment of Wernicke's encephalopathy by parenteral and oral vitamin supplementation is a

Malnutrition and pressure ulcers: nutritional assessment tools.

Good nutrition is essential throughout life and often patients have a poor understanding of what a balanced diet comprises. Nurses need to have a good comprehension of malnutrition. Where malnutrition is evident it can be resolved by working with the patient to encourage a healthy diet. One way to improve patient nutrition is by a nutrition audit of patient food. Numerous nutritional assessment tools exist which help to identify a patient's nutritional status. No nationally standardized nutritional assessment tool exists and there is a danger of assessment methods becoming as proliferate as pr

The development and implementation of a standardized policy for the management of dysphagia in motor neurone disease.

Following an audit to determine the standards of the assessment and management of dysphagia within the Wisdom Hospice, a standardized, multidisciplinary dysphagia policy was established, incorporating a dysphagia trained nurse (DTN) scheme. The standards of care of dysphagic patients were audited after its introduction and the results indicate that the resourcing and quality of care difficulties identified have been improved by the implementation of the policy. The service development implications for the management of dysphagia in palliative care settings are discussed.

Pain assessment by patients and nurses, and nurses' notes on it, in early acute myocardial infarction. Part I.

Pain is a multi-dimensional phenomenon unique to each individual. Most of the research to date on acute pain has focused on oncology, medical and surgical patients. This pilot study was designed to explore the issues of pain assessment by patients and nurses in acute myocardial infarction, and the nurses' subsequent documentation of the pain assessment. Three research instruments were chosen to elicit the information required: A demographic questionnaire was constructed to elicit relevant data on patients and nurses. The Short Form McGill Pain Questionnaire (SFMPQ) was used to measure the qual

Indwelling urinary catheter surveillance using a Task-oriented nurse acuity system.

The task-oriented nurse acuity system (TONAS) has long been used to calculate nursing care needs on hospital patient units, and include nursing documentation on indwelling urinary catheter use. We performed a 2500-patient validation study of our organization's TONAS, which demonstrated high interrater reliability with manual audits (κ >0.92). For institutions that continue to rely on manual surveillance of urinary catheter use, a TONAS may represent a reliable method of automated surveillance.
