Licensed Nurse and Nursing Assistant Recognition of Delirium in Nursing Home Residents With Dementia.

Many nursing home residents experience delirium. Nursing home personnel, especially nursing assistants, have the opportunity to become familiar with residents' normal cognitive function and to recognize changes in a resident's cognitive function over time. The purpose of this study was to determine the accuracy of delirium recognition by licensed nurses and nursing assistants from eight nursing homes over a 12-month period. Participants were asked to complete five case vignette assessments at three different time points (in 6-month intervals) to test their ability to identify different subtype

Sexual Abuse of Older Residents in Nursing Homes: A Focus Group Interview of Nursing Home Staff.

The objective of this study was to increase knowledge of sexual abuse against older residents in nursing homes. A qualitative approach was used. Through a focus group interview with staff in nursing homes, the aim was to reveal employees' thoughts, experiences, and attitudes. Findings from the focus group interview show that sexual abuse of older residents is a taboo topic among health professionals. Acts of sexual abuse are difficult to imagine; it is hard to believe that it occurs. The fact that staff are not aware that it could happen, or have a hard time believing that it actually happens,

Barriers to Discharge for Nursing Home Residents With Serious Mental Illness.

IMPORTANCE: Nursing homes deliver rehabilitative and long-term care for people with serious medical illnesses, functional impairment, and/or cognitive impairment. The population of nursing home residents with serious mental illness (ie, bipolar disorder, schizophrenia, other psychotic disorders) has grown substantially. Other than prisons and jails, nursing homes are the largest institutional care setting for adults (aged ≥18 years) with serious mental illness. Nursing home residents with serious mental illness become long-term residents despite less functional impairment, which may be a funct

Collaborative Working to Address Inappropriate ED Attendances by Nursing Home Residents.

BACKGROUND: Increase in life expectancy in Ireland and social isolation has led to an increasing number of people living in residential care facilities (RCFs). Residents are frequently transferred to emergency departments (ED) for a variety of reasons. Studies found that up to 40% of these hospital admissions were deemed inappropriate. An inappropriate admission in previous studies has been defined as a situation in which care in lower cost settings would be as safe and less disruptive than care in higher cost hospital settings. METHODS: A review of a convenience sample of ED attendances to U

Type 2 diabetes mellitus treatment patterns in U.S. nursing home residents.

BACKGROUND: The prevalence of type 2 diabetes mellitus (diabetes) in nursing home residents (NHRs) is increasing, concurrently with obesity and other comorbid conditions. NHR would benefit greatly from antidiabetic medications that would improve glycemic control and give a lower risk of hypoglycemia but that do not contribute to weight gain in obese individuals. OBJECTIVE: To examine the prescription patterns to NHRs with diabetes, including the use of newer injectable therapies such as glucagon-like peptide-1 (GLP-1) receptor agonists. METHODS: Treatment patterns of diabetes in NHR were ana

Spirometric obstruction and tobacco exposure among male Turkish nursing home residents.

Spirometric obstruction is a prevalent problem in older adults and related to life-style risk factors. Symptoms related to chronic-obstructive-pulmonary-disease (COPD) are also prevalent symptoms with diverse etiologies - not limited to pulmonary obstruction. Older adults may have unrecognized airway obstruction due to functional limitations or symptoms mis-attributed to age/other co-morbidities. Therefore, spirometric obstruction may clinically be over/under diagnosed. Over last few decades, the burden of smoking-related diseases has increased in older adults. Additional evidence regarding ol

Nursing home residents attending the emergency department: clinical characteristics and outcomes.

Nursing home (NH) residents represent the frailest group of older people, and providing gerontologically attuned care that addresses these frailties is often a challenge within the emergency department (ED). This study sought to prospectively profile acutely unwell NH residents in order to clarify some of the challenges of providing emergency care to this group. Over an 18-week period, we prospectively reviewed all NH residents presenting to the ED of an urban university teaching hospital. Relevant data were retrieved by direct physician review (as part of a comprehensive geriatric assessment

Central nervous system medication changes and falls in nursing home residents.

We investigated the role of changes in 6 mutually exclusive medication categories on the risk of falling in nursing home residents. The 6 categories were: gastrointestinal, hypoglycemics, antibiotics, central nervous system (CNS) acting, cardiovascular disease agents, and analgesics. A change was defined as a new start, a dose change, an as-needed dose, or a discontinuation. Incident reports were used to determine the fall date and time. Medication records were abstracted to identify the date of changes before the date of each fall. The 158 residents who fell had 419 recorded falls during 2002
