BACKGROUND AND OBJECTIVES: Crohn's disease (CD) is an inflammatory bowel disease (IBD) characterized by transmural inflammation and intestinal fibrosis involving mostly the small intestine and colon. The pathogenic mechanisms of CD remain incompletely understood and cures are unavailable. Current medical therapies are aimed at inducing prolonged remission. Most of the medical therapies such as corticosteroids have substantial adverse effects. Consequently, many dietary therapies have been explored for the management of CD. Up to now, exclusive enteral nutrition (EEN) has been considered the on

BACKGROUND: This study compared the effectiveness of cognitive therapy (CT), nutritional therapy (NT), the combination of cognitive and nutritional therapy (CNT), against a control condition of support group (SG) in the treatment of bulimia nervosa.
METHODS: One hundred female out-patients who fulfilled DSM-III-R criteria for bulimia nervosa were randomized to the four treatment groups. NT and CT were designed to cover different areas with minimal overlap, and CNT provided all of the features of both of these treatments. The control condition was conducted in a group self-help format. Each of

Enhanced Recovery After Surgery (ERAS), as the plan A and standard for perioperative management, has shifted the focus from routine nutritional support toward the early identification and targeted management of high-risk patients. The new ICD-11 code for undernutrition offers an opportunity to better identify and characterize surgical patients at nutritional risk, while also potentially improving clinical recognition, acceptance of nutritional risk as a relevant perioperative condition, and reimbursement for targeted nutritional interventions. Early preoperative interventions should be conside

PURPOSE OF REVIEW: The primary aim of this review is to summarize the value of nutrition therapy in reversing or altering the core pathophysiology during severe acute pancreatitis (SAP) and how it may impact short-term and long-term clinical outcomes.
RECENT FINDINGS: Early, intense inflammation is ubiquitous during the acute phase of SAP, and specific nutrients like vitamins C, D, and omega-3 fatty acids may modulate inflammation. Omega-3 fatty acids supplementation shows promise in small studies, especially when given parenterally, though results from large randomized trials are pending. Mo

PURPOSE OF REVIEW: A challenging aspect of the care for patients with acute respiratory failure is their nutrition management. This manuscript consists of a literature review on nutrition therapy in non-intubated patients with acute respiratory failure receiving high-flow nasal cannula oxygenation or non-invasive positive pressure ventilation.
RECENT FINDINGS: Studies show that non-intubated patients with acute respiratory failure either on non-invasive ventilation or high-flow nasal cannula are largely underfed in the initial phase of their hospitalization. Although data is limited, the avai

INTRODUCTION: Selenium is an essential micronutrient that must be supplemented in infants and young children on exclusive parenteral nutrition (PN). We examined selenium status and clinical factors associated with a deficiency in infants on PN.
METHODS: This was a retrospective cohort study of pediatric patients receiving PN with routine monitoring of selenium status. Deficiency was diagnosed using age-based norms of plasma selenium status. Associations between selenium deficiency and the following clinical factors were examined: birthweight status: extremely low birthweight (ELBW) versus ver

Digestive and liver disease : official journal of the Italian Society of Gastroenterology and the Italian Association for the Study of the Liver2014eng
Cystic fibrosis can affect food digestion and nutrient absorption. The underlying mutation of the cystic fibrosis trans-membrane regulator gene depletes functional cystic fibrosis trans-membrane regulator on the surface of epithelial cells lining the digestive tract and associated organs, where Cl(-) secretion and subsequently secretion of water and other ions are impaired. This alters pH and dehydrates secretions that precipitate and obstruct the lumen, causing inflammation and the eventual degradation of the pancreas, liver, gallbladder and intestine. Associated conditions include exocrine p

INTRODUCTION: The delivery of adequate nutrition is an integral part of the care of the critically ill surgical patient, and the provision of nutrition may have a greater impact on outcome than many other therapies commonly employed in the treatment of certain disease states.
METHODS: A review of the existing literature was performed to summarize the evidence for utilizing disease-specific nutrition in critically ill surgical patients.
RESULTS: Enteral nutrition, unless specifically contraindicated, is always preferable to parenteral nutrition. Methodological heterogeneity and conflicting re

OBJECTIVE: To evaluate the protein-calorie adequacy of enteral nutrition therapy (ENT) in surgical patients.
METHODS: A prospective study was performed in surgical patients who received ENT from March to October 2011. Patients were evaluated anthropometrically and by subjective global assessment (SGA). The amount of calories and protein prescribed and administered were recorded daily, as well as the causes of discontinuation of the diet. A 90% value was used as the adequacy reference. The difference between the prescribed and administered amount was verified by Student's t-test.
RESULTS: A s

B.H. did not consistently meet the expected outcomes related to maintenance of fluid balance and normal blood glucose levels. Interdialytic episodes of hypotension continued intermittently despite efforts of the multidisciplinary team to frequently assess the patient, alter the treatment plan, and avoid complications. Although postdialysis rebound hypoglycemia occurred, it was infrequent. B.H. did, however, meet the expected patient outcome related to nutrition. His nutritional parameters at the end of IDPN therapy correlated with dramatic improvement in his clinical status. Three months after

A retrospective and prospective analysis was carried out of 48 patients with 54 enterocutaneous small bowel fistulae. 31 patients received total parenteral nutrition (TPN) by 17 patients only the daily losses of fluids and electrolytes were covered. In 19 fistulae (39,5%) closed spontaneously (48,5% with TPN and 14,3% without TPN). 10 patients underwent surgery; 5 fistulae closed operatively. The efficacy of treatment of enterocutaneous fistulae was influenced by the location of fistula, by the daily output, by the septic complications and by the use of TPN. Total parenteral nutrition can be t

The provision of medical nutrition therapy (MNT) is a medical intervention that requires an indication, a therapeutic goal, and the consent of the patient. The decision-making process surrounding MNT often involves complex ethical considerations. The aim of this guideline is to provide practical recommendations on an ethical approach to the decision-making process in clinical nutrition. This practical guideline was developed by an international multidisciplinary working group. The practical, solely consensus-based guideline is based on and updates the 2016 ESPEN Guideline on Ethical Aspects of

PURPOSE OF REVIEW: Malnutrition, whether pre-existing or acquired in the ICU, is common in critically ill children. This narrative review synthesizes current evidence on nutritional assessment and therapy, highlighting how illness phase, underlying disease, and medical interventions influence energy expenditure and nutrient requirements.
RECENT FINDINGS: International guidelines recommend comprehensive nutritional assessment and early initiation of enteral nutrition (EN) for all patients admitted to the Pediatric ICU. However, EN is frequently underutilized or insufficient to meet metabolic d

Adults undergoing hematopoietic cell transplantation often develop serious complications that cause rapid nutritional decline. We developed and evaluated an AI approach to standardize intravenous nutrition (total parenteral nutrition, TPN) during this vulnerable period. Using real-world records from Stanford Health Care (6402 transplants, 2008-2025), we analyzed 1473 adults who received TPN, totaling 27,447 patient-days, linking each day's clinical state to the next day's prescription. We created a library of 30 standardized TPN regimens and trained a model to recommend next-day dose adjustmen

A significant proportion of patients (10%-20%) with acute pancreatitis develop severe acute pancreatitis characterized by pancreatic necrosis, systemic inflammation, and organ failure, commonly requiring intensive care unit (ICU) admission. In this specific population, nutrition therapy is more challenging than that in the general ICU population, primarily because of inevitable gastrointestinal involvement by pancreatic inflammation. In this review, we discussed several key aspects of nutrition therapy in this population, including key pathophysiology that may impede nutrition therapy, the tim

Patients with alcohol-associated liver disease (ALD) consume large amounts of empty calories and are at risk for malnutrition. Malnutrition can present with micro- or macro-nutrient deficiencies. The standard-of-care drug treatment for severe alcohol-associated hepatitis (AH) is corticosteroids. While still in the standard treatment there are limitations in efficacy and certain patients do not respond to treatment (Lille score ≥.45). This article will focus on important concepts related to nutrition and ALD and on recent findings on predicting corticosteroid response and prognosis for AH patie

Critically ill (intensive care unit [ICU]) patients are characterized by organ failure, intense inflammatory response, insulin resistance, and altered metabolic response. The sicker the patient, the higher the threat to nutrition and micronutrient status. In addition, many patients start the ICU stay with an altered nutrition status, which requires assessment upon admission. Nutrition needs vary among patients as well as during hospitalization, as the metabolic response changes over time. Shock and acute organ failure result in a metabolic shift toward intense catabolism: endogenous glucose pr
