Relationship between Nutritional Screening Tools and GLIM in Complicated IBD Requiring Surgery.

BACKGROUND: Accurate identification of malnutrition and preoperative nutritional care in Inflammatory Bowel Disease (IBD) surgery is mandatory. There is no validated nutritional screening tool for IBD patients. We developed a novel nutritional screening tool for IBD patients requiring surgery and compared it with other tools. METHODS: we included 62 consecutive patients scheduled for elective surgery. The IBD Nutritional Screening tool (NS-IBD) was developed to screen patients for further comprehensive assessment. NRS-2002, MUST, MST, MIRT, SaskIBD-NR are compared with the new test. All scree

Nutrition support for patients after cardiopulmonary bypass: required modifications of the TPN solution.

OBJECTIVE: This study was designed to identify the unique metabolic characteristics of patients undergoing cardiopulmonary bypass (CPB) surgery who require postoperative parenteral nutrition. SUMMARY BACKGROUND DATA: Patients undergoing CPB surgery occasionally develop postoperative complications that result in the need for nutrition support. Although enteral nutrition is generally the preferred feeding route, symptomatic hyperlipasemia has been described in critically ill CPB patients receiving enteral nutrition proximal to the ligament of Treitz. In such instances, enteral feeding must be t

Mg deficiency in chronic inflammatory bowel disease and requirements during intravenous nutrition.

Assessment of magnesium (Mg) status by serum and 24-hr urine estimations has been used to study a group of 17 patients with severe Crohn's disease, 10 of whom have required intravenous (IV) nutrition. Mg depletion was present in 15 (88%) and severe enough to cause symptoms in two. Urine levels were low in most patients and would appear to be a more sensitive indication of Mg depletion than serum levels alone. Serum Mg levels were significantly lower in the Crohn's group than in a group of hospital controls; 5-10 mmol of IV Mg were required daily to prevent Mg depletion during IV nutrition and

Pregnancy-induced alterations of 1-carbon metabolism and significance for maternal nutrition requirements.

OBJECTIVES: The pregnancy-induced alterations in 1-carbon (1C) metabolism, effects of advancing gestation on maternal plasma concentrations of methyl nutrients, and potential implications for maternal dietary intake and infant clinical outcomes are summarized in this narrative review. BACKGROUND: 1C metabolism encompasses a series of pathways where 1C units are transferred among nutrients such as B vitamins, choline, and amino acids (the methyl nutrients). Use of isotopic tracers and measuring methyl nutrients in maternal plasma and infant cord blood has advanced the understanding of 1C flux

The effectiveness of buccal Vitamin D replacement in patients requiring home parenteral nutrition.

BACKGROUND AND AIMS: Patients with intestinal failure requiring home parenteral nutrition are at risk of vitamin D and other micronutrient deficiencies. Conventional enteral replacement of Vitamin D may not be sufficient for this patient group. This study examines whether buccal Vitamin D provides an alternative, effective route for supplementing Vitamin D in patients with intestinal failure. METHODS: A retrospective review of patients who received buccal Vitamin D replacement between 1st January 2013 and 1st January 2020 at our hospital in Northern England was carried out. Demographics were

Critical path plan for food and nutrition research required for planetary exploration missions.

In preparation for future planetary exploration, NASA-Johnson Space Center has developed a critical path plan for food and nutrition research needs. The plan highlights the risk factors pertaining to food and nutrition associated with exposure to the space flight environment as well as the possible consequences if no corrective measures are implemented. Included in the plan are the initiating events such as microgravity, remote environment and mission duration, which obviously impact the risk factors. The plan includes points of intervention where mitigating factors can be implemented to avoid

The Vwa+ virulence factor of yersiniae: the molecular basis of the attendant nutritional requirement for Ca++.

The plague V antigen is a cytoplasmic 90,000-dalton peptide, and the W antigen is a primarily extracellular 145,000-dalton lipoprotein of unknown origin. Wild-type yersiniae undergo restriction of cell division when producing these plasmid-mediated virulence antigens (Vwa+) during cultivation at 37 C in Ca++-deficient media. In Yersinia pestis both restriction and synthesis of V and W antigens are potentiated in this environment by elevated Mg++ and prevented by Ca++ or exogenous nucleoside triphosphates. Restriction occurs by mechanisms that also are involved in nutritional stepdown (shutoff

UCI Sports Nutrition Project: Body Composition, Energy Requirements, and Energy Availability in Cycling.

Maintaining adequate energy availability is a key concern for many cyclists, many of whom must balance the need to support their very high training and competition energy expenditure while simultaneously adopting nutritional strategies that will allow them to maintain the body mass deemed most conducive to optimal performance. Available evidence indicates that cyclists tend to be a weight-conscious group that frequently reports engaging in restrictive eating behaviors. Together, the dual demands of supporting the energetic cost of their training alongside restricting energy intake to maintain

Enteral Nutrition in Critically Ill Patient With Septic Shock Requiring Vasopressor: Case Report.

Nutritional support in critically ill patients is an essential aspect of treatment. In particular, the benefits of enteral nutrition (EN) are well recognized, and various guidelines recommend early EN within 48 hours in critically ill patients. However, there is still controversy regarding EN in critically ill patients with septic shock requiring vasopressors. Therefore, this case report aims to provide basic data for the safe and effective nutritional support in septic shock patients who require vasopressors. A 62-year-old male patient was admitted to the intensive care unit with a deep neck

Gastrostomy Tube Placement by Radiological Methods for Older Patients Requiring Enteral Nutrition: Not to be Forgotten.

Background: The use of gastrostomy tubes for long-term nutritional support in older patients is frequent. Percutaneous gastrostomy tube placement may be performed using various techniques, including endoscopic, surgical, and radiologically-guided methods. While percutaneous endoscopic gastrostomy (PEG) placement is the most widely used and accepted approach, experience with the use of percutaneous radiological gastrostomy (PRG) is more limited. Objective: To evaluate the safety and short-term outcomes of PRG in older patients requiring long-term enteral feeding. Method: We performed a prospect

Predicted versus measured resting energy expenditure in patients requiring home parenteral nutrition.

OBJECTIVES: Guidelines from the European Society for Clinical Nutrition and Metabolism (ESPEN) recommend between 20 and 35 kcal/kg daily for patients requiring home parenteral nutrition (PN). Other guidelines use predictive equations. However, these equations have not been validated. Indirect calorimetry is recommended as the gold standard for determining resting energy expenditure (REE). The aim of this study was to compare the frequently used equations with measured REE. METHODS: Seventy-six hospitalized patients suffering from intestinal failure (ages 21-85 y) were enrolled between January

Conception, Pregnancy, and Lactation Despite Chronic Intestinal Failure Requiring Home Parenteral Nutrition.

BACKGROUND: Short-term parenteral nutrition is commonly accepted to be safe in pregnancy, but knowledge about the management of pregnancy during long-term home parenteral nutrition (HPN) is sparse. METHODS AND RESULTS: A systematic literature review revealed that the published experience is limited to 15 pregnancies with parenteral nutrition from preconception to delivery and beyond. Maternal morbidity was surprisingly low, and fetal outcome was good; however, micronutrient deficiencies may have contributed to fetal anomalies. Herein, we additionally report the case of a 26-year-old Caucasian

Dietary copper in excess of nutritional requirement reduces plasma and breast muscle cholesterol of chickens.

Male commercial broiler strain chickens were fed from hatching to 42 d of age either a control diet (based on corn and soybean meal) or the control diet supplemented with 250 mg copper/kg diet from cupric sulfate pentahydrate (for 35 or 42 d). Hypocholesterolemia (11.8% reduction) and decreased breast muscle cholesterol (20.4% reduction) were observed in copper-supplemented birds. There was a slight increase (P > .05) in breast muscle copper (14.5%), and all levels were very low (< .5 mg/kg). Feeding copper for 42 vs 35 d resulted in lower levels of cholesterol in the plasma (12.9 vs 10.8% red

Special considerations required for the formulation and administration of total parenteral nutrition therapy in the elderly patient.

Providing total parenteral nutrition (TPN) to hospitalised patients is not a benign procedure and can be associated with appreciable risks including the development of septic, mechanical and/or metabolic complications. In the older patient, the risks are heightened due to the effects of aging on vital organ function, as well as on the body's ability to respond to injury and infection. In addition, the existence of co-morbid disease will increase the rate of complications. The design of nutritional regimens must account for the changes in body composition and function with age in order to reduc

Nutrition for critically ill children and neonates requiring dialysis: Application of clinical practice recommendations.

Approximately 30% of all children and neonates admitted to the intensive care unit (ICU) experience acute kidney injury (AKI). Children with AKI are largely poorly fed and experience high rates of malnutrition. Nutrition prescription and provision are exceptionally challenging for critically ill neonates, infants, and children with AKI given the dynamic nature of AKI and its respective treatment modalities. Managing the nutrition prescription of critically ill neonates, infants, and children with AKI requires nutrition support clinicians to have a high-level understanding of the various treatm
