Weight management in obesity - past and present.

AIMS: To describe the treatment of obesity from ancient times to present day. METHODS: Articles reporting the development of anti-obesity therapies were identified through a search for 'anti-obesity' AND 'pharmacotherapy' AND 'development' within the title or abstract on PubMed and 'obesity' in ClinicalTrials.gov. Relevant articles and related literature were selected for inclusion. RESULTS: Stone-age miniature obese female statuettes indicate the existence and cultural significance of obesity as long as 30,000 years ago. Records from Ancient Egyptian and Biblical eras through Greco-Roman to

European Guidelines for Obesity Management in Adults.

Obesity is a chronic metabolic disease characterised by an increase of body fat stores. It is a gateway to ill health, and it has become one of the leading causes of disability and death, affecting not only adults but also children and adolescents worldwide. In clinical practice, the body fatness is estimated by BMI, and the accumulation of intra-abdominal fat (marker for higher metabolic and cardiovascular disease risk) can be assessed by waist circumference. Complex interactions between biological, behavioural, social and environmental factors are involved in regulation of energy balance and

Obesity and type 1 diabetes mellitus management.

Patients with type 1 diabetes mellitus (T1DM) traditionally had a low body mass index and microangiopathic complications were common. The Diabetes Control and Complications Trial, published in 1993, demonstrated that therapy aimed at maintaining HbA1c levels as close to normal as feasible reduced the incidence of microangiopathy. Since then, the use of intensive insulin therapy to optimise metabolic control became generalised, with two main side effects: a higher rate of severe hypoglycaemia and increased weight gain. Approximately 50% of patients with T1DM are currently obese or overweight, w

Obesity: assessment and management in primary care.

Obesity is a complex, multifactorial condition in which excess body fat may put a person at health risk. National data indicate that the prevalence of obesity in the United States is increasing in children and adults. Reversing these trends requires changes in individual behavior and the elimination of societal barriers to healthy lifestyle choices. Basic treatment of overweight and obese patients requires a comprehensive approach involving diet and nutrition, regular physical activity, and behavioral change, with an emphasis on long-term weight management rather than short-term extreme weight

Management of Obesity: Review of Adjunctive Therapies and Clinical Management Pearls.

Obesity remains one of the most prevalent and costly diseases in the United States and across the world. New therapies are available to treat obesity, and more are in the pipeline. Although lifestyle therapy by itself achieves only modest weight loss, the new generation of anti-obesity medications, endoscopic bariatric and metabolic therapies, and bariatric surgeries are highly effective for the treatment of obesity. Despite these therapies, the rate of obesity in the United States has remained stable. Multiple factors likely contribute to inadequate obesity treatment in the United States, inc

Advances in pharmacologic management of obesity: oral semaglutide for weight management.

Obesity is a chronic, multifactorial disease characterized by excessive adiposity and associated with an increased risk of cardiovascular disease, type 2 diabetes, and reduced quality of life. Despite the widespread adoption of lifestyle interventions, long-term weight maintenance remains challenging, highlighting the need for effective pharmacologic therapies. Recent advances, including the US Food and Drug Administration approval of oral semaglutide, offer new opportunities for patient-centered obesity management. Evidence from the OASIS 1 and OASIS 4 trials demonstrates that oral semaglutid

Management of the kidney transplant candidate with obesity.

PURPOSE OF REVIEW: Nearly half of all patients listed for kidney transplant now have obesity, which is associated with increased rates of perioperative complications and graft loss. Here, we provide an update on the management of obesity in patients with end-stage kidney disease (ESKD). RECENT FINDINGS: Lifestyle interventions are the backbone of obesity therapy but may be challenging to implement in transplant candidates due to dietary and activity limitations associated with ESKD and hemodialysis. Antiobesity medications (AOMs) acting on the glucagon-like peptide-1 receptor can result in we

Ventilation in the obese: physiological insights and management.

Obesity can have profound adverse effects on the respiratory system, including an impact on pulmonary function, respiratory mechanics, respiratory muscle strength and endurance, gas exchange, control of breathing, and exercise capacity. Lung mechanics are modified by increased pleural pressure resulting from increased abdominal mass and subsequent peripheral airway occlusion and worsening of lung compliance due to reduced functional residual capacity without impairment of chest wall compliance. Arterial blood gases are frequently altered in these subjects and these abnormalities are directly p

Role of Behavioral Interventions in the Management of Obesity.

Overweight and obesity are the leading lifestyle-related causes of clinical and public health concerns. Health behavior change is central in obesity management. This review provides the various behavioral interventions in the management of obesity. Behavior modification is a structured method for improving lifestyle habits such as exercise, diet, and other practices that might influence behavior. This review article was based on current and older literature on behavioral approaches to target overweight/obesity. Articles about the various interventions to reduce obesity, particularly behavior i

Ventral Hernia Management in Obese Patients.

Ventral and incisional hernias in obese patients are particularly challenging. Suboptimal outcomes are reported for elective repair in this population. Preoperative weight loss is ideal but is not achievable in all patients for a variety of reasons, including access to bariatric surgery, poor quality of life, and risk of incarceration. Surgeons must carefully weigh the risk of complications from ventral hernia repair with patient symptoms, the ability to achieve adequate weight loss, and the risks of emergency hernia repair in obese patients.

A review of unmet needs in obesity management.

The prevalence of obesity continues to escalate in the USA; however, there is no consensus regarding the optimal therapy for obesity. For the vast majority of severely obese patients, conventional medical therapies (i.e., diet, exercise, behavioral counseling) often fail over the long term. Existing pharmacotherapy adjunctive to behavioral therapy has limited effectiveness and an imperfect safety record. In contrast, bariatric surgery has a high degree of weight loss efficacy, yet only a small fraction of the qualifying obese population undergoes these procedures because of the associated peri

Postoperative pain management of the obese patient.

In the obese patient, the goal of postoperative pain management is provision of comfort, early mobilisation and improved respiratory function without causing inadequate sedation and respiratory compromise. The pathophysiology of obesity, typical co-morbidities and the high prevalence of obstructive sleep apnoea (OSA) amongst obese patients make safe analgesic management difficult. In particular, pain control after bariatric surgery is a major challenge. Although several reviews covering anaesthesia and analgesia for obese patients are published, there is mainly expert opinion and a paucity of

Contemporary surgical management of the obese adolescent.

PURPOSE OF REVIEW: Adolescent obesity is rapidly becoming a significant public health issue. To date, there are no medical therapies that provide significant and durable weight loss. The surgical treatment of adolescent obesity is a rapidly evolving field. RECENT FINDINGS: Recent publications have focused on the increase in the morbidity and mortality of adolescent obesity into adulthood. There have been significant findings that overweight children and adolescents progress towards severe obesity as adults. Firm guidelines for bariatric surgery in adults have been established for over 15 year
