Advancements of non-invasive prenatal testing: the role of obstetricians.

Since its debut in 2011, Non-Invasive Prenatal Testing (NIPT) has continually demonstrated its effectiveness in detecting an expanding number of diseases. NIPT offers a less invasive approach to prenatal chromosomal disease screening, providing prospective parents with vital information to better prepare for their potential pregnancy outcomes. NIPT was primarily designed for screening trisomy 13, 18, and 21. However, its scope has since broadened to encompass microdeletions and autosomal dominant monogenic diseases. Conversely, the normalization of NIPT can have unintended consequences. Some p

What U.S. Obstetricians Need to Know About Respiratory Syncytial Virus.

Respiratory syncytial virus (RSV) is the leading cause of acute lower respiratory tract infections in neonates, infants, and children worldwide. The virus is estimated to infect 97% of this population in the United States by the age of 2 years, leading to hospitalization for severe lower respiratory tract disease in 2-3% of infants younger than age 6 months. Two preventive options, prenatal administration of a maternal vaccine and administration of a long-acting monoclonal antibody to the infant, are now available for the prevention of RSV-associated lower respiratory tract infection in infant

Burnout in Obstetricians-Gynecologists: Its Prevalence, Identification, Prevention, and Reversal.

Studies indicate that burnout rates among obstetricians-gynecologists range from 40% to more than 75%, which is in the middle to upper one-third of medical specialties. Symptoms range from feelings of underappreciation and unresolved fatigue, to cynicism, depression, physical symptoms, and illness. Burnout is associated with poor job satisfaction, questioning career choices, and dropping out of practice, impacting workforce concerns and patient access. Awareness of the symptoms and some simple stress and fatigue reduction techniques can decrease the risk of being trapped in the downward spiral

How can obstetricians improve outcomes for infants born extremely preterm?

Prematurity remains a leading cause of perinatal morbidity and mortality, and also has significant implications for long-term health. Obstetricians have a key role to play in improving outcomes for infants born at extremely preterm gestations. This review explores the evidence for interventions available to obstetricians caring for women at risk of birthing at extremely preterm gestations, including antenatal corticosteroids, magnesium sulfate, tocolysis and antibiotics. It also addresses the importance of strategies to facilitate safe in-utero transfer, to maximise the chance of extremely pre

Advanced abdominal pregnancy: an increasingly challenging clinical concern for obstetricians.

Advanced abdominal pregnancy is rare. The low incidence, high misdiagnosis rate, and lack of specific clinical signs and symptoms explain the fact that there are no standard diagnostic and treatment options available for advanced abdominal pregnancy. We managed a case of abdominal pregnancy in a woman who was pregnant for the first time. This case was further complicated by a concurrent singleton intrauterine pregnancy; the twin pregnancy was not detected until 20 weeks of pregnancy. The case was confirmed at 26 weeks gestational age using MRI to be an abdominal combined with intrauterine preg

Proactive management of extreme prematurity: disagreement between obstetricians and neonatologists.

OBJECTIVE: To verify, in extremely preterm infants, if disagreement between obstetricians and neonatologists regarding proactive management is associated with early death. STUDY DESIGN: Prospective cohort of 484 infants with 23(0/7) to 26(6/7) weeks, without malformations, born from January 2006 to December 2009 in eight Brazilian hospitals. Pro-active management was defined as indication of ≥1 dose of antenatal steroid or cesarean section (obstetrician) and resuscitation at birth according to the international guidelines (neonatologist). Main outcome was neonatal death in the first 24  h of

Obstetricians, gynecologists and the anti-smoking campaign: a national survey.

OBJECTIVE: To asses the role Jordanian obstetricians and gynecologists play as tobacco cessation counsellors through examining their smoking status, opinions on health risks, factors that influence tobacco use and their perceived barriers to providing effective counselling. STUDY DESIGN: The setting is a tertiary referral university hospital. A pre-tested postal questionnaire survey was mailed to all 462 licensed obstetricians and gynecologists in Jordan. Descriptive statistics were generated and statistical significance was determined by the chi2-test. RESULTS: Of 392 respondents, 37.9% wer

Defining the relationship between obstetricians and maternal-fetal medicine specialists.

OBJECTIVE: The purpose of this study was to determine how frequently general obstetricians refer pregnant patients to maternal-fetal medicine specialists in the presence of the clinical indications specified as appropriate for referral or consultation by the 1996 statement of the Society of Perinatal Obstetricians. STUDY DESIGN: A questionnaire was mailed to 400 randomly selected general obstetricians across the United States. The obstetricians were asked how often they refer their high-risk pregnant patients to maternal-fetal medicine specialists in the presence of (1) a need for diagnostic

Obstetricians and nurse-midwives: The team approach in private practice.

Much has been written about the need for help for the overworked obstetrician. Two obstetricians delivering over 500 babies annuallay had insufficient time for their gynecology practice. Two nurse-midwives were employed and in the first year they delivered 61 per cent of those patients who delivered vaginally. All normal patients had the privilege of choosing their accoucheur; patients, nurse-midwives, and obstetricians were very well pleased with the team approach.

New perspectives on fetal and neonatal alloimmune thrombocytopenia for obstetricians.

Fetal and neonatal alloimmune thrombocytopenia (FNAIT) is a serious disorder that arises when a mother produces alloantibodies against specific human platelet antigens (HPAs) expressed on fetal platelets. These maternal alloantibodies cross the placenta and destroy fetal platelets, significantly increasing the risk of fetal or neonatal intracranial hemorrhage (ICH). Indeed, FNAIT is the leading cause of isolated severe thrombocytopenia in otherwise healthy neonates. In this review, we summarize recent advances to provide obstetricians and maternal-fetal medicine specialists with an updated ove

Assessing maternal cardiac function by obstetricians: technique and reference ranges.

BACKGROUND: A strong body of evidence has now coalesced indicating that some obstetrical syndromes may result from maladaptive responses of the maternal cardiovascular system. Longitudinal studies have shown that these changes are complex and present before the clinical recognition of preeclampsia and fetal growth restriction, suggesting that hemodynamic maladaptation may play an etiologic role in obstetrical complications. Chronic hypertension is one of the most frequent complications of pregnancy, and recent evidence suggests that control of mild hypertension in early pregnancy improves outc

Obstetricians' Opinions of the Optimal Caesarean Rate: A Global Survey.

BACKGROUND: The debate surrounding the optimal caesarean rate has been ongoing for several decades, with the WHO recommending an "acceptable" rate of 5-15% since 1997, despite a weak evidence base. Global expert opinion from obstetric care providers on the optimal caesarean rate has not been documented. The objective of this study was to examine providers' opinions of the optimal caesarean rate worldwide, among all deliveries and within specific sub-groups of deliveries. METHODS: A global online survey of medical doctors who had performed at least one caesarean in the last five years was cond

Hereditary breast/ovarian cancer syndrome: a primer for obstetricians/gynecologists.

An understanding of the diagnosis and clinical management of hereditary breast and ovarian cancer syndrome (HBOC) is essential for obstetricians/gynecologists. This article provides practical information regarding collecting a family history, cancer risk assessment and genetic testing, BRCA-associated cancer prognosis and treatment, screening recommendations, and prevention strategies. Through appropriate cancer risk assessment, women with BRCA1/2 mutations can be identified, and screening and prevention strategies can be used before a diagnosis of cancer occurs. Obstetricians' attitudes to caesarean delivery on maternal request in Nigeria.

The study was done to determine obstetricians' attitude to and factors predicting obstetricians' acceptance of caesarean delivery on maternal request in Nigeria. Nigerian obstetricians were asked to respond to case scenarios depicting caesarean delivery on maternal request without medical indications and the reasons for their responses. The case scenarios were designed from previously published reasons for caesarean delivery on maternal request from Nigeria. Multiple logistic regressions analysis was done to determine factors predicting obstetricians' acceptance of caesarean delivery on matern

'Obstetricians' perceptions of midwifery-led care in Bangladesh - A qualitative study.

The midwife as a separate and autonomous profession is a relatively new concept in Bangladesh; as a result, its integration into the maternal healthcare system is yet to be determined. Obstetricians are key stakeholders in maternal health, and understanding how they perceive midwifery-led care can provide valuable insight into the acceptance of the model, and the profession itself. Data was collected from 13 semi-structured interviews conducted with obstetricians working in the Dhaka division, Bangladesh, in July 2023. Thematic analysis was then carried out on the transcripts. Four main themes

Obstetric venous thromboembolism prophylaxis: What obstetricians should know about anesthesia guidelines.

In 2018 two documents were released from major anesthesia societies, the American Society for Regional Anesthesia (ASRA) and the Society for Obstetric Anesthesia and Perinatology (SOAP), to aid anesthesiologists in decision making regarding neuraxial procedures for obstetric patients receiving anticoagulation. For obstetrical providers seeking to provide appropriate inpatient thromboprophylaxis while also maximizing access to neuraxial anesthesia, awareness of these recommendations may be critically important. In comparison to anesthesiologists in other medical and surgical scenarios, obstetri
