Training in Occupational Medicine: Jurisprudential Malfunctions in the Italian System and European Perspectives.

BACKGROUND: To practice occupational health in Europe, a medical doctor must qualify in occupational medicine. This requires a period of postgraduate specialist medical training lasting a minimum of four years, in conformity with European regulations, to obtain a certificate of completion of training which is then mutually recognized within the entire European Union. DISCUSSION: In 2002 an Italian law allowed doctors specialized in public health medicine and legal/forensic medicine to also practice as consultants in occupational medicine in the country. However a subsequent law in 2008 determ

[Roles of physical, occupational and speech therapists in in-home medical care].

To clarify the current status and issues encountered by physical, occupational, and speech therapists in in-home medical care, a mail survey was conducted in 1996 of 1,292 physicians in the Tokyo metropolitan area who were presumed to have in-home patients. Of the 656 respondents, 243 physicians (37.0%) stated that therapists were involved in treatment of their patients, with 223 of the therapists being physical therapists, 42 being occupational therapists, and 25 being speech therapists. Of the 243 responses, 75 answered that they themselves provided instructions to the therapists, 58 stated

The 10-year experience of an academically affiliated occupational and environmental medicine clinic.

Occupational and environmental diseases are underrecognized. Among the barriers to the successful diagnosis, treatment, and prevention of these conditions are inadequate consultative and information resources. We describe the 10-year clinical and training experiences of an academically affiliated referral center that has as its primary goal the identification of work-related and other environmental diseases. The University of Washington Occupational and Environmental Medicine Program has evaluated 6,048 patients in its diagnostic and screening clinics. Among the 2,841 seen in the diagnostic cl

Fitness for work: the SIMLII Health Surveillance Guidelines.

Italian legislation on prevention in occupational hygiene and safety, which is based largely on European Union Directives, requires health surveillance when risk assessment has identified a risk for workers' health. Health surveillance must be carried out by the occupational physician and concludes with the issue of a fitness for work certificate for the specific job. The guidelines produced by the "Italian Society of Occupational Medicine and Industrial Hygiene" (SIMLII), through the "Consortium for Accreditation and Updating in Occupational Medicine" founded by the "S. Trends in Uterine Cervical Cancer Screening at Physical Health Checkups for Company Employees in Japan.

The consultation rate for uterine cervical cancer screening in Japan is markedly low in comparison with other developed countries. The purpose of this study is to investigate the trends in uterine cervical cancer screening during regular company checkups and to identify potential problems. Questionnaires were sent to occupational health physicians through Sansuiken (Alumni Association of the University of Occupational and Environmental Health, Japan). Overall, 127 valid responses showed that Papanicolaou (Pap) tests are conducted in 100 companies (79%). The detailed information from 50 of the

Diagnosing occupational disease: a new standard of care?

Is a physician civilly liable for failure to diagnose an occupational disease in his patient? Under principles of common-law tort, physicians are held to certain standards of care in treatment. When that standard is breached and a patient is injured, a negligence action may lie. Might a court find that a physician has breached that standard of care if he fails to include in his examination of thorough work history and evaluation of job exposures to toxic chemicals? A number of medical malpractice cases involving standards of care are analyzed and parallels are drawn to the occupational disease

[Psychosocial and socio-medical problems in the medical profession].

Under conditions of contemporary highly developed medicine and allied disciplines of the new scientific concept of health care it is essential that doctors and other health workers should be harmoniously developed personalities. One of the main characteristics of such a personality is the overall health potential. Doctors should not only propagate a healthy lifestyle but should in this respect serve as an example. Unfortunately in many instances paradoxical behaviour in relation to their own health is found. The problem of health protection of doctors must be conceived comprehensively not only

[Health care professionals under programmes for addictions. Personality characteristics of the patients, barriers to treatment and challenges].

Physicians have privileged information regarding the importance of leading a healthy and drug-free lifestyle, but in everyday reality, they are subject to extreme stressors (work stress, compassion fatigue, burnout syndrome) that have a profound emotional impact. As a result, it is not uncommon for physicians to present substance use disorders and dependencies. Recently, greater attention has been paid to specific factors that may lead to substance use disorder. Despite these efforts, there is a lack of prevention policies and substance use disorders are excluded in the list of occupational di

[Reasons for consultation in occupational medicine].

The inability to characterize more precisely the extent of occupational diseases limits the implementation of an effective preventive policy. Furthermore, not all work-related conditions are reported by the Swiss workers' compensation system. A seven-year (1986 to 1992) retrospective study of medical visits in an Institute of Occupational Health Sciences is presented. The objective of this study is to expand data on occupational diseases for clinical and public health intervention. 298 patients have been examined for a possible work-related condition. In 140 cases (47%), an occupational diseas

Prevalence and Associated Factors of Latent Tuberculosis Infection Among Healthcare Workers in a Mexican Tertiary Care Hospital.

BACKGROUND/OBJECTIVES: Healthcare workers (HCWs) are globally recognized as a high-risk group for tuberculosis (TB) infection. However, limited data exist on the prevalence of latent TB infection (LTBI) and associated occupational risk factors in the Mexican context. Identifying the burden of LTBI is essential for effective prevention. This study aimed to estimate the prevalence of LTBI among HCWs in a tertiary care hospital in Mexico and to explore associated risk factors. METHODS: An analytical cross-sectional study was conducted among 300 HCWs (including physicians, nurses, and stretcher-b

U.K. standards of care for occupational contact dermatitis and occupational contact urticaria.

The diagnosis of occupational contact dermatitis (OCD) and occupational contact urticaria (OCU) is a process that involves fastidious clinical and occupational history taking, clinical examination, patch testing and skin-prick testing. A temporal relationship of work and/or the presence of a rash on the hands only raises suspicion of an occupational cause, and does not necessarily confirm an occupational causation. The identification of allergy by patch or prick tests is a major objective, as exclusion of an offending allergen from the environment can contribute to clinical recovery in the ind

Surveillance of work-related carpal tunnel syndrome in Massachusetts, 1992-1997: a report from the Massachusetts Sentinel Event Notification System for Occupational Risks (SENSOR).

BACKGROUND: This surveillance study was undertaken to target efforts to prevent work-related carpal tunnel syndrome (WR-CTS) and to assess physician case-reports as a source of surveillance data. METHODS: Physician case-reports and workers' compensation disability claims were used to document patterns of WR-CTS in Massachusetts from March 1992 to June 1997 by age, gender, industry, occupation, and calendar year. Characteristics of cases identified through the two data sources were compared. RESULTS: 4,836 cases of WR-CTS were ascertained; 6% were identified by both data sources. Whereas the

Just what the nurse practitioner ordered: Independent prescriptive authority and population mental health.

We examine whether relaxing occupational licensing to allow nurse practitioners (NPs)-registered nurses with advanced degrees-to prescribe medication without physician oversight improves population mental health. Exploiting time-series variation in independent prescriptive authority for NPs from 1990 to 2014, we find that broadening prescriptive authority leads to improvements in self-reported mental health and decreases in mental health-related mortality. These improvements are concentrated in areas that are underserved by physicians and among populations that have difficulty accessing physic
