Double tension band wiring for treatment of olecranon fractures.

PURPOSE: Although tension band wiring (TBW) is generally accepted as standard treatment for olecranon fractures, it has several shortcomings such as loss of reduction, skin irritation, and migration of the K-wires. To overcome these problems and increase fixation stability, we used a rigid fixation technique with double tension band wiring (DTBW). Here, we describe the technique and outcomes of the treatment. METHODS: We retrospectively reviewed 21patients with olecranon fractures who were treated by DTBW. We evaluated clinical and radiological outcomes by checking the range of motion, loss o

Biomechanical comparison of interfragmentary compression in transverse fractures of the olecranon.

Compression and absolute stability are important in the management of intra-articular fractures. We compared tension band wiring with plate fixation for the treatment of fractures of the olecranon by measuring compression within the fracture. Identical transverse fractures were created in models of the ulna. Tension band wires were applied to ten fractures and ten were fixed with Acumed plates. Compression was measured using a Tekscan force transducer within the fracture gap. Dynamic testing was carried out by reproducing cyclical contraction of the triceps of 20 N and of the brachialis of 10

Displaced olecranon apophyseal fractures in children with osteogenesis imperfecta.

Three boys with mild osteogenesis imperfecta (OI) who sustained eight apophyseal avulsion fractures of the olecranon were compared with four normal boys with a unilateral apophyseal fracture. The children with OI were younger (11 years 7 months) than the normal children (14 years 3 months). All fractures were treated with tension band wiring (TBW). The contralateral elbow fractured 1 to 12 months later in the children with OI. Refractures occurred in two elbows, 6 to 16 months after the initial fracture and after TBW removal. One refracture presented late as a nonunion and was treated with bon

Hegemann's Disease Detected with a Traumatic Olecranon Fracture: A Case Report.

CASE: We describe a case of Hegemann's disease in a 10-year-old boy practicing karate. The disease was discovered by chance when evaluating a traumatic olecranon fracture. Radiography showed not only olecranon fracture but also a shortening of the epiphysis of the humeral trochlea and a segmental lesion with sclerosis. The trochlea lesion was considered asymptomatic Hegemann's disease. After the olecranon healed conservatively, the patient resumed karate activities and underwent follow-up radiography. The trochlea lesion gradually normalized after 2 years without symptoms. CONCLUSIONS: Regard

Lateral Para-Olecranon Approach for the Treatment of Distal Humeral Fracture.

PURPOSE: To assess the outcomes of the lateral para-olecranon triceps-splitting approach for the treatment of distal humeral fracture. METHODS: Ten patients (3 males, 7 females) with a mean age of 59 years were retrospectively reviewed. There were 2 A2, 3 C1, and 5 C2 fractures according to the AO/ASIF classification. Types B3 and C3 fractures were excluded from this study because the olecranon osteotomy approach was indicated to visualize the anterior fragment. The triceps was split at the midline, and the anconeus muscle was incised from the proximal ulna. The lateral half of the triceps al

Critical analysis of olecranon fracture management by pre-contoured locking plates.

BACKGROUND: Fractures of the proximal ulna are rare and usually managed surgically. Strong fixation of the harware is essential to obtain good outcomes. We report our experience with pre-contoured locking plate fixation of complex olecranon fractures and present a critical appraisal of the outcomes. HYPOTHESIS: Pre-contoured locking plates provide good outcomes, but their clinical tolerance may be limited in some instances. MATERIALS AND METHODS: From September 2009 to December 2011, 28 patients were managed using a pre-contoured locking compression plate (LCP(®)). Among them, 6 were exclude

Cancellous screw with tension band wiring for fractures of the olecranon.

BACKGROUND: Olecranon fractures are one of the most commonly seen orthopaedic injuries in the emergency room. The K-wire which is used in the AO Tension Band Wiring (TBW) technique resists the shear better than the figure of eight wire alone, but, it does not add compression to the fixation strength. But, the cancellous screw plus the tension band wire, in combination, provides the strength of fixation i.e., by converting the tensile force to a compressive force at the fracture site, with additional resistance to the displacement due to the lag screw compression. OBJECTIVE: To clinically eval

Olecranon osteotomy for exposure of fractures and nonunions of the distal humerus.

Although olecranon osteotomy provides excellent exposure of the distal humerus, enthusiasm for this approach has been limited by reports suggesting numerous complications. It has been suggested that specific techniques for creating and repairing an olecranon osteotomy may help limit complications. This paper describes a technique for olecranon osteotomy using an apex, distal, chevron-shaped osteotomy, Kirschner wires directed out the anterior ulnar cortex distal to the coronoid process and bent 180degrees and impacted into the olecranon proximally, and two 22- gauge, figure-of-eight, stainless

Management of comminuted fractures of the olecranon by tension band wiring.

BACKGROUND: Open reduction and rigid internal fixation has become the generally accepted method of treatment for displaced fractures of the olecranon in order to allow early mobilisation and to prevent contracture of the elbow. Comminuted fractures of olecranon are unstable, therefore, bone graft and tension band wiring are supposed to give good stability. Here we give an account of our experience with this procedure. METHODS: We treated ten patients with comminuted fractures of the olecranon by multiple tension-band wiring and a graft from the iliac crest between 1999 and 2002 at Ayub Teachi

Olecranon apophysis fractures in children with osteogenesis imperfecta revisited.

The authors reviewed 17 fractures of the olecranon apophysis in 10 children with mild osteogenesis imperfecta (OI). Seven of the 10 patients sustained the same injury to the opposite extremity 1 to 70 months (mean 15.1) after their initial fracture. Four fractures were initially treated by cast immobilization alone. Two of these eventually required operative treatment because of refracture or late displacement. In all, 15 fractures were treated operatively. All had healed at the time the cast was removed; however, two refractured. At latest follow-up (mean 53 months), no patient reported pain

Elbow Arthroscopy for Posteromedial Impingement and Fixation of Olecranon Stress Fracture.

Posteromedial elbow impingement due to valgus extension overload often develops as a result of excessive valgus and extension force during repetitive overhead throwing activities. Impingement classically occurs in throwing athletes such as baseball, tennis, softball, or lacrosse players. If isolated, arthroscopic removal of the posteromedial olecranon osteophytes shows excellent postoperative satisfaction, return to sport rates, and return to previous level of activity. This Technical Note describes treatment of posteromedial elbow impingement syndrome and associated olecranon stress fracture
