Gonococcal scalp abscess in a neonate delivered by caesarean section.

Gonococcal infection in caesarean delivered babies is very rare and is usually limited to ophthalmia neonatorum. The mother had rupture of membranes 14 hours before the caesarean section. The infection was most likely introduced by the fetal scalp electrode probes applied 2 hours before delivery. This is the first reported of a neonatal gonococcal abscess in a caesarean delivered infant.

Neonatal Gonococcal Conjunctivitis Caused by Fluoroquinolone-Resistant Neisseria gonorrhoeae.

Prophylaxis for ophthalmia neonatorum remains in use despite decreased incidence of the condition. We report a breakthrough case of neonatal conjunctivitis in Japan caused by a levofloxacin-resistant Neisseria gonorrhoeae bacteria strain, co-infected with Chlamydia trachomatis bacteria. This case highlights failures in screening, prophylaxis, and treatment, underscoring the need to reassess prevention strategies.

Rapid, reliable diagnosis of chlamydial ophthalmia by means of monoclonal antibodies.

The use of fluorescein-conjugated monoclonal antibody (Syva, UK) provided a rapid reliable diagnostic test for Chlamydia trachomatis in conjunctival samples from 100 adults with acute follicular conjunctivitis and seven babies with suspected chlamydial ophthalmia neonatorum. Elementary bodies (EBs) were seen in smears from 11 of the adults, and culture confirmed C. trachomatis infection in nine of them. Both tests were positive with specimens from four of the neonates. No specimens from either group of patients produced a negative result in the smear test but a positive result by culture. Howe

Reevaluating Neonatal Erythromycin Prophylaxis Policy Amid Advances in Sexually Transmitted Infection Screening, Antenatal Treatment, and Antibiotic Stewardship.

Long-standing mandates in the United States require universal ophthalmic prophylaxis with erythromycin for all neonates, although recent evidence has led to reconsideration of this practice. This intervention was originally adopted to prevent blindness caused by Neisseria gonorrhoeae (gonococcal ophthalmia neonatorum, GON) and Chlamydia trachomatis (chlamydial ophthalmia neonatorum, CON). Today, however, prenatal screening and treatment have rendered such infections rare at birth. Current population data indicate that GON is exceedingly uncommon in the United States and that erythromycin is in

Simplified method for diagnosis of genital and ocular infections with Chlamydia.

The diagnosis of infections with chlamydia related to the trachoma agent is best confirmed by isolation of the organism in tissue-culture. A simplified technique using conventional monolayers of McCoy cells can be used as an alternative to current methods involving heavily irradiated cells and appears to be of reasonable sensitivity. Chlamydial infection was confirmed in 38/190 females attending venereal clinics, in 22/35 patients with hypertrophic cervical erosions, and in 7/18 infants with ophthalmia neonatorum.

Childhood blindness from corneal ulceration in Africa: causes, prevention, and treatment.

Corneal scarring is responsible for approximately 70% of blindness in children in African countries. Recent studies confirm that measles infection is a common predisposing factor and that vitamin A deficiency is responsible for the majority of bilateral corneal ulceration. Herpes simplex keratitis (caused by human (alpha) herpesvirus types 1 and 2), ophthalmia neonatorum, and the use of traditional eye medicines also constitute significant causes of corneal ulceration in children in Africa. Intervention strategies, the management of corneal ulceration in children, and areas for future investig

Common ocular problems in children: conjunctivitis and tear duct obstructions.

Conjunctivitis, both in the newborn period and later, and nasolacrimal duct obstructions are common ocular problems facing all pediatricians. Conjunctivitis in childhood is separated into ophthalmia neonatorum and conjunctivitis at a later time. Gonococcus and chlamydia are the organisms causing the most concern in the neonatal period. Later, the diversity of causes becomes greater, but the consequences are smaller except for herpes simplex keratoconjunctivitis. Nasolacrimal duct obstructions are common in the first few months of life with most patients clearing spontaneously. Management is ai

Single low-dose ceftriaxone for the treatment of gonococcal ophthalmia--implications for the national programme for the syndromic management of sexually transmitted diseases.

We prospectively analysed a total of 21 baby-mother pairs with culture-proven Neisseria gonorrhoeae treated with a single low dose of ceftriaxone, namely 62.5 mg for babies and 125 mg for mothers respectively. N. gonorrhoeae was eradicated from all babies' eyes with no residual damage, as well as from the mothers' cervixes. A single low dose of 62.5 mg ceftriaxone has emerged as the treatment of choice for gonococcal ophthalmia neonatorum because of its excellent activity against N. gonorrhocae, including penicillinase-producing strains.

Benjamin Gibson 1774-1812: Manchester's first ophthalmologist.

Benjamin Gibson, a Newcastle-born surgeon, trained in Lancaster, Chester, London and Edinburgh before being appointed as assistant to Charles White, Manchester surgeon and man-midwife. He developed expertise in eye diseases, particularly of children. In 1804 he was appointed Honorary Surgeon to the Manchester Infirmary. He died young in 1812, but had published significantly on the cause of ophthalmia neonatorum, on cataract surgery in infants (the first to do so) and on surgery to reform damaged pupils. He was the first specialist oculist in Manchester and the North of England, and the first i

What have we learned from sexually transmitted infection research in sub-Saharan Africa?

Many of the most important research findings on STIs in the past 30 years have come from sub-Saharan Africa. African researchers and their international collaborators have led the way in the development and validation of syndromic STI management, in furthering our understanding of the interactions between HIV and other STIs, in the development of evidence-based strategies for the control of HIV and other STIs in high risk groups and in the general population, and in clinical, microbiological and epidemiological studies on syphilis, chancroid and ophthalmia neonatorum. This review summarises th

Neonatal Visual Impairment: Etiologies, Screening, and Management.

Severe visual impairment in the newborn period can be attributed to prenatal and perinatal etiologies. Prenatal causes include various congenital anomalies such as anophthalmos or microphthalmos, congenital cataracts, congenital glaucoma, retinal dystrophies, and retinoblastoma. Acquired perinatal causes include ophthalmia neonatorum, cerebral visual impairment related to neurologic insult, and retinopathy of prematurity. Severe visual impairment can negatively affect a child's growth, development, social skills, and educational abilities, but prompt recognition and treatment of visual deficit

Corneal ulceration, measles, and childhood blindness in Tanzania.

One hundred and thirty Tanzanian children with corneal ulceration were clinically examined to determine the cause of the ulceration. 37% of the ulcers were associated with recent measles infection and 38% of the children had bilateral ulceration. Herpes simplex virus infection was the commonest cause of ulceration in the series, but vitamin A deficiency was the major cause of bilateral ulceration, subsequent blindness, and mortality in this series. Other significant causes of childhood corneal ulceration were the use of traditional eye medicines, confluent measles keratitis, and ophthalmia neo

Eikenella corrodens as a causative agent for neonatal conjunctivitis.

Ophthalmia neonatorum is the most common eye infection in the first month of life. The etiologies have been attributed to chemical, bacterial, and viral agents. Neonatal bacterial conjunctivitis is caused by many species of bacteria, the most important being Neisseria gonorrhoea and Chlamydia trachomatis. Eikenella corrodens is a Gram-negative bacillus. It is a fastidious, slow growing, facultative anaerobic bacterium that may be found as the normal flora of the human mouth, nasopharynx, gut, and genitourinary tract. Its role in causing infections in humans has been recognized in the last 2 de

[Sexually transmissible disease in tropical Africa].

Sexually transmitted diseases in tropical Africa are a severe problem. The facilities for diagnosis and treatment in general are pour. Reliable informations on epidemiology are rare. Gonorrhea and non-gonorrheal urethritis are probably the commonest in tropical Africa, frequently giving rise to epididymitis, urethral strictures and salpingitis as well as pelvic inflammatory diseases, important factors of infertility. Evidence available suggests that the incidence of infectious syphilis is high. Some authors however are of the opinion that there is a steady decline of the condition. Among young

BACKGROUND: The Canadian Paediatric Society no longer recommends the use of universal ocular prophylaxis with erythromycin ointment to prevent ophthalmia neonatorum. Screening for Chlamydia trachomatis and Neisseria gonorrhoeae in all pregnant women is considered the most effective way of preventing vertical transmission and ophthalmia neonatorum. OBJECTIVE: The aims of this study were to assess prenatal screening rates of C. trachomatis and N. gonorrhoeae and to compare sociodemographic factors between those screened and those not screened. METHODS: The list of all women who delivered at a
