INTRODUCTION: Opioid use disorder (OUD) and chronic pain are major global health challenges. Although opioid therapies provide effective analgesia, long-term use is limited by safety concerns, dependence, and variable efficacy. Modulation of the endocannabinoid system (ECS) has emerged as a potential therapeutic strategy for pain management and opioid-related disorders.
AREAS COVERED: This narrative review summarizes evidence on ECS-targeted interventions for OUD, chronic non-cancer pain, and cancer-related pain. Relevant literature was identified through PubMed using terms related to the ECS

Evidence-based opioid use disorder (OUD) relapse prevention interventions designed to the unique needs of the postpartum period are lacking. We aimed to identify novel intervention targets using ecological momentary assessment (EMA) with Bayesian mixed-effects models to assess infant caregiving (e.g., skin-to-skin contact), relational factors (e.g., loneliness), and individual factors (e.g., pain): (1) by OUD status, and (2) in relation to craving and coping. Participants with and without OUD (OUD+ and OUD-, respectively) were followed from late pregnancy to postpartum week 12 with daily EMA a

BACKGROUND: The relation between impulsivity and sleep indices is not well determined in patients receiving methadone maintenance treatment (MMT).
AIMS: to evaluate high impulsivity prevalence, its risk factors and relation with sleep indices.
METHODS: a random MMT sample (n = 61) plus MMT current cocaine users (n = 20) were assessed for impulsivity (Barratt impulsivity scale [BIS-11] and Balloon Analogue Risk task [BART]), sleep quality (Pittsburg Sleep Quality Index [PSQI]), sleepiness (The Epworth sleepiness scale [ESS]), and substance in urine.
RESULTS: 81 patients, aged 56.6 ± 10, 54.3

BACKGROUND: Recovery from opioid use disorder (OUD) includes improvements in health-related quality of life (HRQOL) and is supported by recovery capital (RC). Little is known about RC and HRQOL among recovery residents taking medication for OUD. We described HRQOL and RC and identified predictors of HRQOL.
METHODS: Project HOMES is an ongoing longitudinal study implemented in 14 recovery homes in Texas. This is a cross-sectional analysis of data from 358 participants' on HRQOL (five EQ-5D-5L dimensions-mobility, self-care, usual activities, pain/discomfort, and anxiety/depression) and RC (Ass

IMPORTANCE: Stimulant medication use is common among individuals receiving buprenorphine for opioid use disorder (OUD). Associations between prescription stimulant use and treatment outcomes in this population have been understudied.
OBJECTIVES: To investigate whether use of prescription stimulants was associated with (1) drug-related poisoning and (2) buprenorphine treatment retention.
DESIGN, SETTING, AND PARTICIPANTS: This retrospective, recurrent-event cohort study with a case-crossover design used a secondary analysis of administrative claims data from IBM MarketScan Commercial and Mult

Background: The evidence is still sparse on the effects of various doses and routes of buprenorphine on emotional and clinical outcomes for individuals with OUD. Methods: A systematic review and meta-analysis of RCTs was performed following PRISMA and Cochrane guidelines. The electronic databases and other sources were searched for studies that were published from February 2013 to September 2025. Negative emotional symptomology, illicit opioid use, treatment retention, and adverse events were eligible outcomes. Standardized mean differences and odds ratios were used to summarize continuous and

OBJECTIVES: The COVID-19 pandemic led to increased substance-related morbidity and mortality and transformed care for opioid use disorder (OUD). We assessed the perceived impacts of the pandemic on substance use and related consequences among patients in office-based addiction treatment (OBAT).
METHODS: We recruited patients with OUD on buprenorphine from July 2021 to July 2022, with data collection at baseline and 6 months. Exposures of interest were the following 6 domains potentially impacted by COVID-19: personal or family infection, difficulty accessing healthcare/medication, economic st

OBJECTIVES: Opioid use disorder (OUD) and cancer gained attention as co-occurring diseases in the last 2 decades due to the possible relationship between opioid prescriptions for cancer pain and the risk of developing substance use disorder in cancer patients. However, little is known about patients previously diagnosed with OUD who develop cancer and how to manage both OUD symptoms and control pain.
METHODS: The present case series deals with this subpopulation and proposes a dose escalation of methadone to control both the cancer-related pain and drug addiction symptoms.
RESULTS: This appr

INTRODUCTION: Hospitalizations due to severe injection-related infections (SIRIs) and patient-directed discharge (PDD) in people who inject drugs (PWID) are increasing, but research on readmission outcomes at PDD is limited. In this retrospective, matched cohort study we evaluated predictors of 30-day readmission by discharge status among PWID.
METHODS: Among patients diagnosed with SIRIs at a tertiary hospital, Fisher's exact tests assessed differences in readmission rates by discharge status. Medications for opioid use disorder (MOUD) at discharge was defined as either having a buprenorphin

BACKGROUND: Numerous interventions aimed at addressing the Canadian 'opioid crisis' have been implemented. However, no empirical estimaes of the number of people with problematic prescription opioid (PO) use exist to inform and guide intervention system needs.
METHODS: The annual numbers of Canadian adults (≥ 15 years) with PO misuse and/or use disorders ('addiction') were estimated by combining data on the prevalence and associated 95% Confidence Intervals (CIs) of PO use in the Canadian population, obtained from national surveys, with PO misuse and use disorders transition probabilities, ob

Panic disorder (PD) occurs at greater rates among those with HIV compared to those without HIV. Rates of PD may be elevated among those with opioid dependence (persons who inject drugs, PWID). Persons with HIV experience common bodily symptoms as a result of the disease and these symptoms overlap with those of PD which may contribute to a "fear of fear" cycle present in PD. HIV-positive, PWID represent an at-risk population in terms of poor medication adherence. HIV symptoms and HIV medication side-effects commonly overlap with panic symptoms and may affect HIV medication adherence. The aim of

BACKGROUND: Opioid overdoses have continued to increase since the start of the COVID-19 pandemic. The pathways through which the COVID-19 pandemic has affected trajectories of opioid use and opioid-related problems are largely unknown. Using the Epidemic-Pandemic Impacts Inventory (EPII), a novel instrument that assess pandemic-related impacts across multiple life domains, we tested the hypothesis that COVID-related impacts (on e.g., interpersonal conflict, employment, infection exposure, and emotional health) experienced in the early months of the pandemic would predict changes in opioid use

BACKGROUND: Opioid-related disorders (ORD) among patients with chronic pain represents a significant clinical challenge, yet surveillance data within Chinese healthcare settings remain limited. This study explored the feasibility of utilizing hospital information system (HIS) data to quantify indicators of ORD and analyze associated factors in a tertiary care setting.
METHODS: A retrospective study was conducted at Nanjing Drum Tower Hospital. Patients with chronic pain prescribed opioids for ≥ 90 days between 2022 and 2023 were included. Indicators of ORD were defined by the presence of an I

OBJECTIVE: To evaluate the effect of baseline post-traumatic stress disorder (PTSD) and each symptoms cluster on the risk of developing opioid use disorder (OUD) in those exposed to opioid painkillers and to assess the effect of comorbid PTSD and OUD on functioning, OUD severity, and treatment seeking compared with individuals with OUD only.
METHODS: We obtained data from 4025 individuals exposed to opioid painkillers from the National Epidemiologic Survey on Alcohol and Related Conditions III. We matched individuals with baseline PTSD with individuals without PTSD on demographics, developmen

INTRODUCTION: Pregnant individuals with opioid use disorder (OUD) deliver >20,000 infants annually in the United States, with serious health consequences for perinatal individuals and their infants. This study compares delivery-related outcomes and healthcare utilization among Michigan Medicaid-enrolled perinatal individuals with and without an OUD diagnosis.
METHODS: We conducted a retrospective cohort study among Michigan Medicaid enrollees aged 15-44 who delivered a live infant between 2012 and 2021.
RESULTS: The cohort included 218,890 deliveries among 170,002 individuals enrolled contin

Background: Numerous studies have highlighted the pivotal role of alterations in the monetary reward system in the development and maintenance of substance use disorder (SUD). Although these alterations have been well documented in various forms of SUD, the electrophysiological mechanisms specific to opioid use disorder (OUD) remain underexplored. Understanding these mechanisms is critical for developing targeted interventions and advancing theories of addiction specific to opioid use. Objectives: To explore abnormalities in monetary reward outcome processing in males with OUD. We hypothesized

The opioid overdose crisis has continued to worsen, with a concomitant increase in serious injection-related infections, such as endocarditis and osteomyelitis. Usual care of these infections involves long-term intravenous antibiotics, typically administered via a peripherally inserted central venous catheter (PICC) at home. In patients with a history of opioid use disorder who inject drugs, a PICC has long been viewed as a high-risk intervention that may contribute to illicit substance use due to ease of venous access; thus, providers are often uncomfortable discharging these patients home to

INTRODUCTION: While the relationship between various obstetric procedures and the onset of opioid use disorder (OUD) remains ambiguous, this study aims to elucidate the immediate and prolonged risks of OUD in women who have undergone procedures such as vaginal and cesarean deliveries, induced abortions, and treatments related to miscarriages and ectopic pregnancies.
METHODS: Retrospective data (n = 632,872) from the Cerner Real-World Data™ for pregnant females (age 15-44) between January 2010 and March 2020 were used. Adjusted odds ratios (ORs) and 95% confidence intervals (CIs) were used to

BACKGROUND: As CRH-binding protein (CRHBP) SNP rs1500 was associated with reduced cocaine abuse after 1 year in methadone maintenance treatment (MMT) for heroin addiction, we evaluated the association of additional 28 selected SNPs, in 17 stress-related genes, with MMT outcome.
METHODS: The distribution of genotypes of each SNP by cocaine abuse after 1 year in MMT was assessed under the dominant and recessive models using χ2. Cumulative retention (up to 26.5 years) was studied using Kaplan-Meier analyses. Logistic regression and Cox model were used for multivariate analyses.
RESULTS: Of a no
