HIV and Hickam's Dictum: Multiple Opportunistic Infections.

Opportunistic infections are common in patients with acquired immunodeficiency syndrome (AIDS) or advanced human immunodeficiency virus infection when adherence to anti-retroviral is poor. We present a case report of multiple opportunistic infections simultaneously present in a patient with AIDS who presented with fevers, abdominal pain, and diarrhea. He was ultimately diagnosed with Streptococcus pneumoniae pneumonia, oral thrush, esophageal histoplasmosis, cytomegalovirus esophagitis and herpes simplex esophagitis. This case is a reminder of the severity of illness in AIDS and the need for t

Opportunistic CNS Viral Infections in Immunocompromised Patients.

Immunocompromised patients face increased morbidity and mortality from central nervous system (CNS) opportunistic viral infections, with risk level driven by specific immune defects. While our diagnostic capacity expands, immunocompromised patients may present atypically or with mild symptoms, delaying or complicating diagnosis. Treatment options are often limited, although novel agents show promise. This review discusses why immunocompromised patients carry higher immunologic risk and how certain viral classes display neurotropism to invade the CNS. We outline the utility and limitations of c

Renal transplantation and predisposition to opportunistic infections.

PURPOSE OF REVIEW: Infections represent a significant source of morbidity and mortality after kidney transplantation in children. We review recent advances in epidemiology, assessment, prevention and treatment for several different infections. RECENT FINDINGS: Infections, such as bacterial urinary tract infection or opportunistic viral infection remain common, may be increasing and represent a large proportion of hospitalization. Extended antiviral agent use reduces the incidence of cytomegalovirus disease but its efficacy to reduce Epstein-Barr virus disease remains controversial. Human herp

Opportunistic infections in women with HIV AIDS.

Women account for half of the global human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS) infections. Heterosexual contact is the leading risk factor in women. Over 50% of patients are significantly immunosuppressed at the time of diagnosis. Women with advanced HIV infection are at a risk for opportunistic infections (OIs). OIs lead to significant morbidity and cost. Some of OIs may impact women more significantly than men, that is, human papillomavirus infection and cervical cancer. OIs during pregnancy can increase the risk of maternal-to-child transmission of HIV and

Opportunistic parasitic infections among immunocompromised Egyptian patients.

The commonest opportunistic parasites causing morbidity and/or mortality in the immuno-compromised subjects are mainly the gastro-intestinal ones. This study clarified the prevalence of the opportunistic parasites among a group of immunocompromised patients selected from Al Azhar University Hospitals. One hundred immunocompromised patients (GI) were divided into GIa: 40 malignancy patients. GIb: 30 with diabetes mellitus. GIc: 30 with chronic renal failure. GII: included 20 cross-matched healthy subjects as controls. Sheets were filled out on each subject including all personal and medical his

[Non tuberculous anti-TNF associated opportunistic infections].

Anti-TNFalpha agents have revolutionized the treatment of patients with rheumatoid arthritis, spondylarthropathies and Crohn's disease. However, their use is associated with an increased risk of infections. Pyogenic infections (involving the lungs, skin and urinary tract) and tuberculosis are the more commonly observed infectious complications in patients receiving anti-TNFalpha agents. However, opportunistic infections have been increasingly reported in anti-TNFalpha-treated patients, and include non tuberculous mycobacteria, fungi (Pneumocystis jiroveci, Candida sp, Aspergillus, Cryptococcus

Preventing and treating biologic-associated opportunistic infections.

A variety of opportunistic pathogens have been reported to infect patients receiving tumor necrosis factor (TNF) antagonists for the treatment of autoimmune diseases. These pathogens are numerous, and include coccidioides, histoplasma, nontuberculous mycobacteria, Mycobacteria tuberculosis, and others of public health concern. Accordingly, TNF antagonists should be used with caution in patients at risk for tuberculosis, and screening for latent tuberculosis infection should be undertaken before anti-TNF therapy is initiated. Although screening and prevention efforts have decreased the risk of

Klebsiella oxytoca: opportunistic infections in laboratory rodents.

Opportunistic pathogens have become increasingly relevant as the causative agents of clinical disease and pathological lesions in laboratory animals. This study was conducted to evaluate the role of Klebsiella oxytoca as an opportunistic pathogen in laboratory rodents. Therefore, K. oxytoca-induced lesions were studied from 2004 to early 2006 in naturally infected rodent colonies maintained at The Jackson Laboratory (TJL), Bar Harbor, USA, the Animal Research Centre (Tierforschungszentrum, TFZ) of the University of Ulm, Germany and the Central Animal Facility (ZTM) of the Hannover Medical Scho

[Opportunistic infections after treatment with monoclonal antibodies].

During recent years the therapeutic indications of monoclonal antibodies are on the increase. Most monoclonal antibodies are immunosuppressants. Thus, therapeutic successes are accompanied by an increase of serious infections. Closest cooperation between the haematologist/oncologist and the infectious diseases specialist is a prerequisite for the successful outcome of treatment of the individual patient. Rituximab and alemtuzumab especially pave the way for a variety of opportunistic infections. Prophylactic drugs directed against bacterial infections and invasive fungal infections have not be

Management of opportunistic infections in pediatric HIV.

Human immunodeficiency virus (HIV) causes a chronic infection that leads to profound immunosuppression. A hallmark of this process is the depletion of CD4+ lymphocytes, which predisposes the patient to develop a variety of opportunistic infections and certain neoplasms. The CD4+ lymphocyte count is the best validated predictor of the likelihood of developing opportunistic infections (OI). Susceptibility to OI increases as HIV induced immunosuppression becomes more severe. The management of HIV infection hence involves not only inhibiting viral replication using antiretroviral drugs, but also t

Cushing's syndrome complicated by multiple opportunistic infections.

The case history of a 56-year-old man is described who suffered from severe adrenocorticotrophic hormone (ACTH)-dependent Cushing's syndrome. The clinical course was complicated by simultaneous infections with Pneumocystis carinii, Staphylococcus aureus, Candida albicans, Aspergillus fumigatus and Herpes simplex, which proved to be fatal. A study of the literature shows that opportunistic infections in endogenous Cushing's syndrome are associated with severe cortisol excess and carry a high mortality. Opportunistic infections are most prevalent in the ectopic ACTH syndrome explained by the ver

Opportunistic infections and psychosocial stress in HIV.

Persons infected with the human immunodeficiency virus (HIV) are susceptible to both opportunistic infections and psychosocial crises at varying stages of their disease. Many interventions, including lifestyle changes and chemoprophylaxis, may help prevent, delay or lessen the extent of the morbidities associated with immunodeficiency. In appropriate patients, prophylaxis for infections with Pneumocystis carinii, Mycobacterium avium complex and Toxoplasma helps to reduce morbidity.

Drug treatment of HIV-related opportunistic infections.

The AIDS epidemic has led to the emergence of several disease entities which in the pre-AIDS era were rare or seemingly innocuous. Experience of treating these diseases varies. In some instances, such as Pneumocystis carinii pneumonia, there is an abundance of published literature to direct our course of action. However, for many of these newly recognised diseases our treatment experience is limited. Furthermore, in many instances, well controlled trials evaluating treatment modalities in the AIDS population are lacking. We have identified 13 disease entities (P. carinii pneumonia, toxoplasmos

[Mycoses as opportunistic infections in AIDS patients].

Since January 1985 more than 100 patients with deep fungal infections have been treated with itraconazole (200 to 400 mg/day) in Northern Italy. Evaluation of the drug efficacy and tolerance was possible in one patient with sporotrichosis, in 34 with aspergillosis, and in 36 with cryptococcosis (mainly patients positive for human immunodeficiency virus). Response to itraconazole alone was obtained in the case of sporotrichosis and in 24 of 34 patients with different forms of aspergillosis (of the 18 patients with invasive pulmonary aspergillosis, 15 were cured). Patients with cryptococcosis re
