Architecture of the optic chiasm and the mechanisms that sculpt its development.

At the optic chiasm the two optic nerves fuse, and fibers from each eye cross the midline or turn back and remain uncrossed. Having adopted their pathways the fibers separate to form the two optic tracts. Research into the architecture and development of the chiasm has become an area of increasing interest. Many of its mature features are complex and vary between different animal types. It is probable that numerous factors sculpt its development. The separate ganglion cell classes cross the midline at different locations along the length of the chiasm, reflecting their distinct periods of prod

Ganglioglioma of the optic chiasm: case report and review of the literature.

We report a case of a hypothalamic chiasmatic ganglioglioma in a 21-year-old woman who presented with hyperprolactinemia and developed visual field defects. This circumscribed cystic lesion with an enhancing mural nodule was radiologically indistinguishable from a pilocytic astrocytoma. Although rare, gangliogliomas should be included in the differential diagnosis of lesions occurring in this area of the brain.

The early development of the optic nerve and chiasm in embryonic rat.

To establish the time course and major features of the development of the optic nerve and chiasm in the embryonic rat, the growth of axons from the retina to the brain has been studied by light and electron microscopy. On embryonic day 14 (E14), the first axons are generated by retinal ganglion cells. Fascicles of axons can be detected in the optic stalk at E14.5 and in the diencephalon by E15.0. In the vitreal retina and optic fissure, large extracellular spaces resemble the oriented channels previously described in the mouse. They form approximately 12 hours before the invasion of optic axon

Characterization of a Novel MRI Sign of the Optic Chiasm in Patients With Optic Nerve Hypoplasia.

PURPOSE: To characterize a novel magnetic resonance imaging (MRI) finding affecting the optic chiasm in patients with optic nerve hypoplasia (ONH). METHODS: This was a retrospective case-control study including 78 patients diagnosed as having ONH at a single institution, with an average age at MRI of 2.66 years. The control group included 70 patients of similar demographics without a diagnosis of ONH or gross abnormalities on MRI. MRI images obtained on a Siemens 3T Prisma or Siemens 1.5T Aera (Siemens Healthineers) were examined for all patients by two independent graders for presence of an

Optic Nerve Glioblastoma with Optic Chiasm Involvement: A Case Report and a Brief Literature Review.

Background: optic nerve glioblastoma is an uncommon pathology. The optic chiasm, optic tract, or optic nerves are possible places from which the tumor can originate. Most of the neuroimaging findings are nonspecific. To confirm the diagnosis, a biopsy is required. A delay to the treatment plan for optic nerve glioblastoma results in poor patient survival rates. Case report: a 68-year-old woman with an uncomplicated medical history presented with exophthalmos, deteriorating eyesight, and partial loss of vision. Using radiological data together with postoperative histopathological and histochemi

Relationship between optic nerve length and interoptic angle in the prediction of optic chiasm location.

INTRODUCTION AND OBJECTIVES: The sellar region is an area in the base of the skull that is among the most common sites for tumors of the central nervous system. Surgical interventions are currently performed via different routes. While the optic chiasm occupies its expected position in 70% of the population, it can deviate from this position. In such cases, surgery involving this region becomes more difficult as the known surgical routes are narrowed. Advance awareness of these variations can help surgeons to identify the optimal route for safe surgical intervention in the sellar region. By pe

Magnetic resonance imaging evidence of optic nerve glioma progression into and beyond the optic chiasm.

Optic nerve glioma has been a topic of great interest and controversy, particularly with respect to its tendency to invade the optic chiasm. This case report demonstrates histologically verified optic nerve glioma in an otherwise healthy 5-year-old girl. MRI revealed sequential prechiasmal, chiasmal, and postchiasmal disease during the untreated course of 18 months. The authors also discuss the results obtained with a specific immunohistochemical technique, the mindbomb homolog 1 (MIB-1) labeling index, and speculate that this information, if known early in the course of disease, might influen

Longitudinal investigation of optic chiasm in patients with traumatic brain injury.

BACKGROUND: Traumatic brain injury (TBI) often precipitates a cascade of neurophysiological alterations, impacting structures such as the optic nerve and ocular motor system. However, the literature lacks expansive investigations into the longitudinal changes in the optic chiasm and its relationship with the clinical recovery of visual processing. This study aimed to scrutinize longitudinal changes in optic chiasm volume (OCV) and establish the relationship of OCV with process speed index at 12 months post-injury. Process speed index is derived from Wechsler Adult Intelligence Scale IV. METHO

How I do it: endoscopic endonasal chiasmopexy for optic chiasm prolapse.

BACKGROUND: Prolapse of the optic system into an empty sella is an unusual complication of the management of pituitary tumors. METHOD: We described the technical nuances for an endoscopic endonasal chiasmopexy, the indications for the procedure, and the caveats on the diagnosis of optic chiasm prolapse. CONCLUSION: The endoscopic endonasal chiasmopexy provides a direct route to lift the optic system. In most cases, the procedure may be achieved extradurally, protecting the chiasm from manipulation and postoperative fibrosis.

Diagnostic accuracy of neuroimaging in pediatric optic chiasm/sellar/suprasellar tumors.

Preoperative diagnosis for tumors arising in the optic chiasm/sellar/suprasellar region in children is helpful to determine surgical necessity and approach, given the high operative risk in this area. We evaluated the ability to differentiate tumor type by preoperative neuroimaging. Thirty-eight of 53 tumors were correctly diagnosed by neuroimaging based on final pathologic diagnosis (prediction accuracy 72%). Prediction accuracies were 87% (20/23) for craniopharyngioma, 79% (11/14) for optic pathway glioma, 64% (7/11) for germ cell tumor, and 0% (0/5) for Langerhans cell histiocytosis. Diagno

Case Report: Late Sequela of a Muslinoma Involving the Optic Chiasm.

An 84-year-old woman presented with 3 months of vertical binocular diplopia and difficulty reading at near. She had a history of bilateral ophthalmic artery aneurysm repair involving use of muslin in the 1990s. The patient then developed bitemporal hemianopsia secondary to muslin-induced inflammation ("muslinoma") extending to the optic chiasm, which required surgical decompression. She had a persistent bitemporal hemianopsia but was stable for two decades after surgery. In 2017, the patient re-presented with double vision. Exam showed a non-paretic diplopia due to a small angle comitant right

Brain and Optic Chiasm Herniation into Sella after Pituitary Tumor Apoplexy.

Brain and optic chiasm herniation has been rarely reported following dopamine agonist treatment for large prolactinomas. We report a case of brain and optical chiasm herniation, secondary to an empty sella due to apoplexy of a prolactinoma, and we focus on the specific presentation of this case. A 32-year-old female presented to a neurologist complaining of headaches. Her past medical history was significant for acute vision loss in both eyes accompanied by right third nerve palsy when she was 16 years old. She does not recall any endocrine or imaging evaluation at that time and she had sponta

Isolated primary central nervous system lymphoma arising from the optic chiasm.

A 58-year-old previously healthy woman rapidly developed progressive bilateral visual loss. Magnetic resonance imaging revealed a bulging appearance of the optic chiasm, with homogeneous enhancement after gadolinium administration, which suggested an optic glioma or inflammatory disease. In the absence of (para)clinical clues for a specific diagnosis despite extensive investigation, a biopsy of one optic nerve was performed, resulting in a diagnosis of non-Hodgkin B-cell lymphoma. There was no evidence of any other ocular or systemic involvement, therefore the conclusion was that this immunoco

[Functional assessment before and after interventions on the optic chiasm system].

OBJECTIVES: In the course of a tumor in the area of the optic chiasm the optochiasmal system is often exposed to compressive and vascular irritations. This study deals with the functional results (visual acuity and visual field) before and after surgery based on the primary situation and morphology of the tumor in order to give a prognosis on this matter. METHODS: 62 patients (41 female, 21 male) suffering from tumors in the area of the optic chiasm were retrospectively reviewed. The duration of the follow-up was an average of 25.82 months, +/- 20.04. Among the tumors investigated there were

Vascular architecture of the human optic chiasm and bitemporal field defects.

The system of blood supply of the optic chiasma was studied on 85 human fresh bodies by several methods. The optic chiasma is supplied by the Willis' circle, arterioles from which, after forming anastomosis at two levels, enter the chiasma and break into capillaries. The authors found that the median portion of chiasma is a weak point of microcirculation, as demonstrated by rarefied and transverse capillaries. When the blood supply of chiasma is compromised by lesions near the sella turcica, this weak point of microcirculation is apt to be disturbed first and becomes ischemic, causing disorder
