High-resolution endoscopic imaging of the GI tract using optical coherence tomography.

BACKGROUND: Optical coherence tomography (OCT) has demonstrated the microscopic structure of the gastrointestinal (GI) tract mucosa and submucosa in vitro. We evaluated a prototype OCT system and assessed the feasibility of OCT in the human GI tract. METHODS: The 2.4 mm diameter prototype OCT probe, inserted through an endoscope, provides a 360-degree radial scan. Images (6.7 frames/sec) are displayed on a television monitor. Tissue contact is not required. In patients undergoing elective endoscopy, OCT images were obtained of normal mucosa (confirmed by biopsy). RESULTS: Seventy-two sites w

Peripheral macrophages and T-cells accumulate in the degenerating optic tract after repetitive head impact.

Chronic white matter inflammation is a feature of traumatic brain injury (TBI), consistently observed in rodent models of repetitive mild TBI and persistnat in human TBI patients for years post-injury. We use a high-frequency head impact model (HFHI, 5 impacts per day, over 6 consecutive days) to investigate neuroimmune responses in the optic tract, a white matter region particularly vulnerable to injury-induced degeneration in mouse repeat mild TBI models. We integrated immunohistochemistry, digital spatial proteomics, transcriptomic profiling, and blood-brain barrier (BBB) assessments with n

Functional optic tract rewiring via subtype- and target-specific axonal regeneration and presynaptic activity enhancement.

Mechanisms underlying functional axonal rewiring after adult mammalian central nervous system (CNS) injuries remain unclear partially due to limited models. Here we develop a mouse intracranial pre-olivary pretectal nucleus (OPN) optic tract injury model and demonstrate that Pten/Socs3 knockout and CNTF expression in retinal ganglion cells (RGCs) promotes optic tract regeneration and OPN reinnervation. Revealed by transmission electron microscopy, trans-synaptic labeling, and electrophysiology, functional synapses are formed in OPN mainly by intrinsically photosensitive RGCs, thereby partially

Development of myelin in human optic nerve and tract. A light and electron microscopic study.

The early development of myelin in human optic nerve and tract was studied in plastic-embedded material from post-mortem examinations of 18 infants and children. Specimens were examined by light and electron microscopy, and a good correlation was found between the data obtained by the two techniques. The characteristic lamellae of myelin were first seen around a few fibers of tract and intracranial optic nerve at 32 weeks of gestation. By term, these myelin sheaths had become thicker, and a majority of the nerve fibers had become myelinated. In the optic nerve near the globe, myelin was first

[A follow-up of patients with anterior optic tract glioma concurrent with type 1 neurofibromatosis].

Fifty-nine patients with anterior optic tract (AOT) lesion concurrent with type neurofibromatosis (NF-1) were followed up for 1 to 36 years (median 5 years). Based on the study, the authors identified several grades of the disease: - stabilization without intervention; - stabilization after treatment; - insignificant negative changes as a slight tumor growth and a further stabilization of the process; - occasionally negative changes irrespective of treatment. The behavior of a tumor is unpredictable. The onset of progression in infancy is a poor predictor. Ophthalmological symptoms are not the

[MR imaging study of edema along the optic tract in patient with Rathke's cleft cyst].

We report an 80-year-old woman who was admitted to our hospital with symptoms due to diabetes insipidus. Magnetic resonance (MR) imaging demonstrated a sellar/suprasellar cystic lesion with marginal enhancement and the thick pituitary stalk. The MR imaging depicted edema spreading along the optic tract on fluid-attenuated inversion recovery (FLAIR) images. Upon neurological examination at the time of admission, there were no abnormal findings affecting the field of vision or visual acuity. In endocrinological examination, the basal plasma values of pituitary hormones were within normal range e

Optic chiasm, optic tract and deep white demyelination: an unusual distribution of myelin oligodendrocyte glycoprotein-associated demyelination (MOGAD), case report and review of literature.

A preschool girl presented with sudden-onset bilateral painless loss of vision from 2 days prior. Child's examination showed light perception vision, sluggishly reacting pupils, otherwise normal anterior segment, healthy optic disc and retina in both eyes. MRI of brain and orbit with contrast revealed thickened left part of the optic chiasm with contrast enhancement extending proximally to bilateral optic tract and hyperintensities in the left thalamus and periventricular white mater. Considering the topographical distribution of lesions in the brain, neuromyelitis optica spectrum disorder was

Comparison of pituitary stalk angle, inter-neural angle and optic tract angle in relation to optic chiasm location on 3-dimensional magnetic resonance imaging.

Optic chiasm location is critical in sellar surgeries and may be obscured by tumor invasion. This study evaluates inter-neural angle, pituitary stalk angle, optic tract angle in prefixed, normal and post fixed optic chiasms using 3-dimensional constructive interface in steady state (CISS) sequences of magnetic resonance imaging (MRI) in essentially normal studies. MRI Brain studies of 154 patients were retrospectively evaluated. Location and height of optic chiasm was noted. Interneural angle between intracranial segments of bilateral optic nerves, pituitary stalk angle between pituitary stalk

Microphthalmia, aphakia and agenesis of the optic chiasm and tract in a wild fox squirrel (Sciurusniger).

We describe gross and histopathological features of multiple ocular and neuro-ophthalmic abnormalities in a fox squirrel (Sciurus niger). Ophthalmic findings included severe bilateral microphthalmos, with the right eye more affected than the left. Histopathology confirmed severe microphthalmia, aphakia, disorganized retinal tissue and small optic nerves, as well as agenesis of the optic chiasm and optic tract. This combination of neuro-ophthalmic abnormalities has not been previously described in wild animals.

Direct Invasion of the Optic Nerves, Chiasm, and Tracts by Cryptococcus neoformans in an Immunocompetent Host.

Cryptococcus spp is a common fungal infection and frequent cause of meningitis in immunocompromised patients; however, immunocompetent patients are also at risk of infection. Visual loss often occurs via elevated intracranial hypertension but can rarely occur through direct optic nerve, chiasm, or tract invasion. We report a case of a 38-year-old woman who presented with decreased acuity in both eyes. She had generalized visual field constriction in the right eye and temporal hemianopsia in the left eye. Magnetic resonance imaging of the brain and orbits showed multiple areas of ill-defined en

Aggressive granular cell tumor of the neurohypophysis with optic tract edema and invasion into third ventricle.

BACKGROUND: Granular cell tumors (GCTs) of the neurohypophysis are parasellar tumors arising from pituicytes in the neurohypophysis and are generally considered benign slow-growing tumors. We present a case of sellar GCT with aggressive features. CASE DESCRIPTION: A 70-year-old female presented with progressive vision impairment found to have bitemporal visual field defects. Subsequent magnetic resonance imaging (MRI) revealed a 2.9 cm × 2.5 cm × 2.5 cm parasellar mass with extension into the third ventricle and causing optic tract edema (OTE). Right frontotemporal orbital craniotomy was perf

Intraoperative Subcortical Electrical Mapping of the Optic Tract in Awake Surgery Using a Virtual Reality Headset.

BACKGROUND: Brain mapping during awake craniotomy is a well-known technique to preserve neurological functions, especially the language. It is still challenging to map the optic radiations due to the difficulty to test the visual field intraoperatively. To assess the visual field during awake craniotomy, we developed the Functions' Explorer based on a virtual reality headset (FEX-VRH). METHODS: The impaired visual field of 10 patients was tested with automated perimetry (the gold standard examination) and the FEX-VRH. The proof-of-concept test was done during the surgery performed on a patien

Nonthermal ablation with microbubble-enhanced focused ultrasound close to the optic tract without affecting nerve function.

OBJECT: Tumors at the skull base are challenging for both resection and radiosurgery given the presence of critical adjacent structures, such as cranial nerves, blood vessels, and brainstem. Magnetic resonance imaging-guided thermal ablation via laser or other methods has been evaluated as a minimally invasive alternative to these techniques in the brain. Focused ultrasound (FUS) offers a noninvasive method of thermal ablation; however, skull heating limits currently available technology to ablation at regions distant from the skull bone. Here, the authors evaluated a method that circumvents t

Primary non-Hodgkin lymphoma of the chiasm and optic tract in a nonimmunocompromised patient: illustrative case.

BACKGROUND: Chiasmatic and optic track lymphoma as the primary lesion of the central nervous system (CNS) is extremely rare. OBSERVATIONS: The authors report a case of a previously healthy 62-year-old woman who presented with quick and progressive visual impairment leading to bilateral blindness. Brain imaging studies suggested glioma or lymphoma of the chiasm and the posterior visual pathway. Postoperative examination revealed low-grade malignant B-cell lymphoma. No evidence of extracranial lymphoma was found, so a final diagnosis of primary CNS lymphoma (PCNSL) was made. LESSONS: To the au

Grading upper tract urothelial carcinoma with the attenuation coefficient of in-vivo optical coherence tomography.

INTRODUCTION: With catheter based optical coherence tomography (OCT), high resolution images of the upper urinary tract can be obtained, thereby facilitating the detection of upper tract urothelial carcinomas (UTUC). We hypothesized that the attenuation coefficient of the OCT signal (μOCT ) is related to the histopathologic grade of the tumor. OBJECTIVES: In this study, we aimed to define the μOCT cut-off for discriminating high grade and low grade papillary UTUC. METHODS: For this post-hoc analysis, data from OCT imaging of papillary UTUC was obtained from patients during ureterorenoscopy.
