The term naso-orbital fracture refers to the backward displacement of the nasal bones into the interorbital space. The fracture is most commonly the result of traumatic contact with the dashboard in automobile accidents. The trauma results in soft tissue eyelid deformities, which may also involve nasolacrimal disruption and life-threatening intracranial injury. Early management consists of closed reduction of the nasal fracture when feasible. Late management, which is more common, consists of transnasal wiring, restoration of the nasolacrimal apparatus, and contouring of the nasal bones.

Naso-orbital fractures designate a backward displacement of the bones of the nasoskeletal framework into the interorbital space. This anatomical disruption results in deformities, lacrimal dysfunction, medial canthal deformities, ptosis, blow-out fractures, ocular complication, and most severely, penetration of bony fragments into the anterior cranial fossa. Early and late treatment methods are considered. Restoration of the bony contour of the nose and medial canthoplasty procedures are discussed.

This study retrospectively examines 551 patients with 597 orbital floor fractures for sequelae of enophthalmos and diplopia. Ocular injuries are associated with 6.9% of the fractures. Nondisplaced orbital floor fractures have been treated with observation alone, resulting in an incidence of sequelae of zero in 263 patients. Displaced fractures that had not undergone surgery have resulted in an incidence of sequelae of 18 (34%) in 53 patients. Displaced fractures that had undergone orbital floor exploration have resulted in an incidence of sequelae of 29 (10.3%) in 281 patients. Only one mild l

BACKGROUND: Complex orbital fractures are impure orbital fractures because they involve the orbital walls and mid-facial bones. The author reported an orbital wall restoration technique in which the primary orbital wall fragments were restored to their prior position in complex orbital fractures in 2020. As a follow-up to a previous preliminary study, this study retrospectively reviewed the surgical results of complex orbital wall fractures over a 4-year period and compared the surgical outcomes by dividing them into groups with and without balloon restoration.
METHODS: Data of 939 patients w…

Although trauma is one of the main causes of orbital apex syndrome (OAS), reports of OAS associated with orbital fractures are relatively rare. We recently treated two patients who sustained severe visual impairment with damage to multiple cranial nerves (third to sixth) associated with inferomedial orbital wall fractures. In these patients, posterior movement of the globe caused neuropathy of the cranial and optic nerves by posterior globe edema and hemorrhage, or direct impact between the globe and wall, which might then have induced OAS in the cases described in this report. Steroid therapy…

PURPOSE: Facial trauma and orbital fractures are common reasons for ophthalmology consultation in the emergency department (ED). The purpose of this study is to assess intervention rates and evaluate the acuity of ophthalmology consultation for orbital fractures in the ED.
BASIC PROCEDURES: A retrospective chart review of orbital fractures was conducted over a 23-month period. 379 cases of orbital fractures were identified in a single-center study. All patients that received an ophthalmology consultation in the ED were included. Demographics, mechanism and location of orbital fracture, ophtha…

Objective Orbital roof fractures are more likely to occur in younger children, specifically younger than 7 years. Cranium to face ratio decreases with age; however, there is no definition for measurement of the neurocranium or face. We propose using the length of the orbital roof as a measurement of the neurocranium and length of the orbital floor as a tool to estimate midface size. The purpose of this study is to test this measurement as a correlation rate of orbital roof fractures within the pediatric population. Design This is a retrospective study. Setting This study was done at the LeB

PURPOSE: To report a case of a respiratory epithelial-lined, air-filled orbital cyst as a late complication of orbital fracture repair.
METHOD: Case report.
RESULTS: Recurrent episodes of diplopia and hyperophthalmia developed secondary to pneumatic inflation of a respiratory epithelial-lined orbital cyst 6 months after orbital fracture repair. This cyst remained in communication with an ethmoidal air cell and became inflated during pressurization of the ethmoid sinus. Removal of the cyst and orbital implant was curative.
CONCLUSION: Air-filled, respiratory epithelial-lined orbital cyst is

INTRODUCTION: Aeromedical evacuation of patients with post-traumatic orbital emphysema following zygoma and orbital floor fractures has often been viewed with considerable concern. The expansion of intra-orbital air with increasing altitude can theoretically result in orbital tension, which can lead to blindness. This concern is mainly based on extremely rare case reports and theoretical models rather than significant clinical experience. The purpose of this work is to evaluate hazards and complications associated with aeromedical evacuation of combat casualties with post-traumatic orbital emp

BACKGROUND: To predict which patients will develop more than 2 mm of enophthalmos and require surgery after orbital fracture is challenging. Although high herniated orbital volume (HV) might be a predictor, its measurement can be complex and time-consuming. This study aimed to identify a simple, reliable, and clinically applicable method for measuring herniated orbital volume.
METHODS: This single-center retrospective study examined HV and approximate volume in 42 patients with an orbital floor fracture (OF group) and 56 patients with a medial wall fracture (MW group). Approximate herniated v

An unusual late complication to an orbital floor fracture is presented. Five years after trauma a now 22-year old man demonstrated an entamoeba infected cyst in the orbit.

Orbital emphysema refers to the presence of air within the orbital tissues and most commonly develops following trauma involving the paranasal sinuses or orbital walls. Although typically benign and self-limiting, it may rarely cause vision-threatening complications due to orbital compartment syndrome. The authors report a case of traumatic orbital emphysema presenting with clinical features suggestive of an orbital floor fracture. A 35-year-old man presented with left periorbital swelling and limitation of upgaze after blunt trauma. Orbital computed tomography demonstrated intraorbital air co

A 24-year-old man with a deviation of the right eye noted since childhood presented with significant enophthalmos, limitation of elevation, and globe retraction on attempted elevation. A disproportionately small right hypotropia was suggestive of a restrictive pathology, and forced-duction testing confirmed tightness of the inferior rectus muscle. Clinical features resembled those of orbital floor fracture with inferior rectus entrapment. Computed tomography disclosed no fracture but did reveal an abnormal structure arising from the orbit adjacent to the inferior rectus origin and attaching to

Orbital cellulitis and abscess have been described in the literature as complication that usually occur secondary to infection in the maxillary, ethmoidal, and frontal sinuses. If left untreated, it can lead to blindness, cavernous sinus thrombosis, meningitis, or cerebral abscess. Orbital fractures are a common sequela of blunt orbital trauma, but are only rarely associated with orbital cellulitis. So, the authors present rare orbital cellulitis after orbital blow-out fracture. A 55-year-old Asian complains of severe orbital swelling and pain on the left side. These symptoms had started 2 day…
