PEEK patient-specific implants for orbital reconstruction: accuracy and clinical outcomes.

Orbital fractures represent a common and complex challenge in maxillofacial surgery, with functional and aesthetic sequelae. Advances in computer-aided design and manufacturing, virtual planning, and three-dimensional printing have enabledpatient-specific implants (PSIs). Polyetheretherketone (PEEK), a bioinert polymer with properties similar to cortical bone, has emerged as a suitable material for orbital reconstruction. Surgical navigation has also been advocated to enhance accuracy in challenging regions. This retrospective service evaluation analysed ten patients with unilateral orbital fr

Outcomes of planned versus emergent enucleation procedures with primary orbital implants.

OBJECTIVE: The primary purpose of this study was to explore the outcomes of primary implant placement in patients who have undergone enucleation on either a planned or emergent basis. METHODS: A retrospective chart review was performed of 128 enucleations with at least a 1-year postoperative follow-up between November 2008 and May 2019 by a single oculoplastic surgeon at Albany Medical Center. Emergent cases were categorized as those with an active, unclosed globe perforation, secondary to either acute trauma, dehiscence or failed closure of a previously opposed wound, or exposure of a surgic

Rapid prototyped patient specific implants for reconstruction of orbital wall defects.

Defects of orbital walls can be reconstructed using implants. The authors report a safe and accurate method to reconstruct bone defects in the orbital area using patient specific implants. A detailed process description of computer aided design (CAD) reconstructive surgery (CRS) is introduced in this prospective study. The 3D volumetric virtual implant was design using MSCT data and PTCProEngineer™ 3D software. The intact orbital cavity of twelve patients was mirrored to the injured side. Specific ledges steered the implant into correct place. Postoperatively the position was assessed using im

Comparison of fibrovascular ingrowth into hydroxyapatite and porous polyethylene orbital implants.

Two porous orbital implants available for clinical use in the anophthalmic socket are hydroxyapatite (HA) and porous polyethylene (PP). We examined the rate and the extent of fibrovascular ingrowth into these implants using histopathologic criteria in a rabbit model. Thirty-two New Zealand white rabbits underwent a unilateral enucleation with placement of a 14-mm spherical orbital implant. Twelve rabbits received HA, 12 small-pore PP, and 8 large-pore PP. The implants inserted were wrapped either in autologous sclera with and without anterior fenestrations or as unwrapped spheres. The implants

Deviated hydroxyapatite orbital implant syndrome.

PURPOSE: To describe a patient with a deviated orbital implant after enucleation. METHOD: Case report. RESULT: We examined a postenucleation patient with a medially deviated ("esotropic") implant after peg drilling. Modification of the posterior surface of the prosthesis improved cosmesis in the primary position. CONCLUSIONS: During enucleation surgery, surgeons may use a spherical implant rather than modify the anterior face in patients with preexisting strabismus, with severe trauma to the extraocular muscles, or with risk of implant deviation. Additionally, secondary procedures may alter

Complications of dermis-fat orbital implantation.

Primary dermis-fat implantation can be effectively performed in cases without pre-existing systemic vascular disease or orbital burns. Meticulous handling of the graft (using a Goeller trephine and Tenon's capsule traction sutures), filleting Tenon's capsule, and avoiding cautery of the graft bed may minimize graft necrosis and atrophy. Pyogenic granulomas of the conjunctival-graft interface and graft hirsutism are easily managed. Keratinization of the socket, graft wound dehiscence, donor site hematomas, and wound dehiscence are avoided with careful surgical technique. Secondary dermis-fat or

Orbital Floor and Rim Reconstruction With a Titanium Orbital Implant and Acellular Dermis.

Reconstruction of the orbital rim and floor following tumor excision is traditionally performed with bone grafts, vascularized grafts, or free flaps. The authors describe a case of an osteosarcoma of the maxillary sinus and a second case of an ossifying fibroma of the orbital floor, both of which required reconstruction of the orbital floor and rim. In both of these cases, reconstruction of the orbital floor and rim was performed with a titanium implant whose anterior portion was bent inferiorly to recreate the orbital rim. Acellular dermis was sutured over the anterior portion of the orbital
