The Use of Physiology-Based Pharmacokinetic and Pharmacodynamic Modeling in the Discovery of the Dual Orexin Receptor Antagonist ACT-541468.

The identification of new sleep drugs poses particular challenges in drug discovery owing to disease-specific requirements such as rapid onset of action, sleep maintenance throughout major parts of the night, and absence of residual next-day effects. Robust tools to estimate drug levels in human brain are therefore key for a successful discovery program. Animal models constitute an appropriate choice for drugs without species differences in receptor pharmacology or pharmacokinetics. Translation to man becomes more challenging when interspecies differences are prominent. This report describes t

Orexin / hypocretin 1 receptor antagonist reduces heroin self-administration and cue-induced heroin seeking.

The orexin/hypocretin system is involved in several addiction-related behaviors. In the present experiments, we examined the involvement of orexin in heroin reinforcement and relapse by administering the orexin 1 receptor antagonist SB-334867 prior to heroin self-administration or prior to cue-induced or heroin-induced reinstatement of extinguished heroin seeking in male Sprague Dawley rats. SB-334867 (30 mg/kg, intraperitoneal) reduced heroin intake during self-administration under fixed ratio-1 and progressive ratio schedules. SB-334867 also attenuated reinstatement of heroin seeking elicite

[The preclinical profiles and the clinical efficacy and safety of vornorexant (Vorzzz® tablets), a new orexin receptor antagonist].

Traditionally, benzodiazepines and non-benzodiazepines have been primary pharmacological treatments for insomnia. The introduction of orexin receptor antagonists has provided safer alternatives. Orexin is a neuropeptide involved in maintaining wakefulness, and blockade of its receptors OX1 and OX2 is considered to promote sleep with minimal disruption to the natural sleep. Vornorexant (Vorzzz® tablets 2.5 mg, 5 mg, 10 mg) is a dual orexin receptor antagonist developed in Japan and approved in August 2025 for insomnia. Designed with a short elimination half-life, vornorexant aims to minimize ne…

Central administration of an orexin receptor 1 antagonist prevents the stimulatory effect of Olanzapine on endogenous glucose production.

Atypical antipsychotic drugs such as Olanzapine (Olan) induce weight gain and metabolic changes associated with the development of type 2 diabetes. The mechanisms underlying these undesired side-effects are currently unknown. It has been shown that peripheral injections of Olan activate neurons in the lateral hypothalamus/perifornical area and that a large part of these neurons are orexin (Ox) A-positive. We investigated further the possible involvement of the central Ox system in the metabolic side-effects of Olan by comparing the hyperglycaemic effects of an intragastric (IG) Olan infusion b

Orexin receptor type-1 antagonist SB-334867 inhibits the development of morphine analgesic tolerance in rats.

Herein the effect of orexin receptor type-1 antagonist SB-334867 on the development of tolerance to analgesic effects of morphine was studied in rats. To incite tolerance, morphine sulfate was injected intraperitoneally (i.p., 10mg/kg) once a day for 7 days. The tail flick test was used to evaluate antinociceptive effects of the morphine. A selective OxR1 receptor antagonist, SB-334867, was microinjected (i.c.v.) into the right cerebral ventricle (10 μg/10 μl) immediately before each morphine injection. Repeated morphine application resulted in tolerance to morphine analgesic effects as a decr

A retrospective clinical practice study comparing the usefulness of dual-orexin receptor antagonists and a melatonin receptor agonist in patients switching from long-term benzodiazepine receptor agonists.

STUDY OBJECTIVES: Although novel hypnotics have recently emerged, there are currently no data comparing the clinical potency of benzodiazepine receptor agonists (BZRAs) and novel hypnotics, or the effectiveness of different methods of switching between them. This study examined how novel hypnotics might help reduce BZRA use in real-world practice. METHODS: 289 patients with psychiatric disorders who took BZRAs for over 1 year before switching to either of 2 dual-orexin receptor antagonists (DORAs; suvorexant [SUV] or lemborexant [LEM]) or a melatonin receptor agonist (ramelteon [RMT]) were en…

Effects of Orexin Receptor Antagonists on Nocturia and Overactive Bladder Symptoms in Patients with Insomnia: A Single-Center Retrospective Study.

PURPOSE: Although orexin receptor antagonists (ORAs) are widely prescribed for insomnia, experimental studies have suggested that these agents may prolong intervals between bladder contractions. This study investigated the hypothesis-generating associations between ORA treatment and improvements in nocturia and overactive bladder symptoms in patients with insomnia. METHODS: Enrolled were 45 patients who visited our hospital between September 2022 and December 2025 with nocturia as their main complaint and experienced two or more nighttime urinations. All had overactive bladder symptoms define…

Dual orexin receptor antagonist-assisted dose reduction of benzodiazepines and benzodiazepine receptor agonists: a mirror-image analysis of insurance claims data.

PURPOSE: Discontinuing benzodiazepines and benzodiazepine receptor agonists (BZ/BZRAs) remains challenging for patients with chronic insomnia despite safety concerns. This study evaluated whether continuous treatment with dual orexin receptor antagonists (DORAs) facilitates a reduction in BZ/BZRA doses among long-term users. METHODS: This self-controlled (mirror-image) study, in which each patient served as their own control, used Japanese health insurance claims data (Dokenpo database, 2019-2024). Eligible patients had continuous BZ/BZRA use for ≥ 6 months and initiated suvorexant or lembore…

Dual orexin receptor antagonist ameliorates sleep deprivation-induced learning and memory impairment in APP/PS1 mice.

Sleep is considered closely related to cognitive function, and cognitive impairment is the main clinical manifestation of Alzheimer's disease (AD). Sleep disturbance in AD patients is more severe than that in healthy elderly individuals. Additionally, sleep deprivation reportedly increases the activity of the hypothalamic orexin system and the risk of AD. To investigate whether intervention with the orexin system can improve sleep disturbance in AD and its impact on AD pathology. In this study, six-month-old amyloid precursor protein/presenilin 1 mice were subjected to six weeks of chronic sle

Discovery of Nivasorexant (ACT-539313): The First Selective Orexin-1 Receptor Antagonist (SO1RA) Investigated in Clinical Trials.

The orexin system consists of two neuropeptides (orexins A and B) and two receptors (OX1 and OX2). Selective OX1 receptor antagonists (SO1RA) are gaining interest for their potential use in the treatment of CNS disorders, including substance abuse, eating, obsessive compulsive, or anxiety disorders. While blocking OX2 reduces wakefulness, the expected advantage of selectively antagonizing OX1 is the ability to achieve clinical efficacy without the promotion of sleep. Herein we report our discovery efforts starting from a dual orexin receptor antagonist and describe a serendipitous finding that

Distinct effects of orexin receptor antagonist and GABAA agonist on sleep and physical/cognitive functions after forced awakening.

The majority of patients with insomnia are treated with hypnotic agents. In the present study, we evaluated the side-effect profile of an orexin receptor antagonist and γ-aminobutyric acid A (GABAA) receptor agonist on physical/cognitive functions upon forced awakening. This double-blind, randomized, placebo-controlled, cross-over study was conducted on 30 healthy male subjects. Fifteen minutes before bedtime, the subjects took a pill of suvorexant (20 mg), brotizolam (0.25 mg), or placebo and were forced awake 90 min thereafter. Physical- and cognitive-function tests were performed before tak

A selective orexin-1 receptor antagonist, SB334867, blocks 2-DG-induced gastric acid secretion in rats.

We have previously demonstrated that intracisternal orexin-A potently stimulated gastric acid secretion through the vagus nerve. Considering its stimulatory action on feeding, we hypothesized that orexin-A is a candidate mediator of cephalic phase gastric secretion. It has also been suggested that the stimulation of acid by central orexin-A may be mediated by orexin 1 receptor (OX1R) in the brain. In the present study, we tried to clarify whether endogenously released orexin-A in the brain indeed plays a physiological role in gastric secretion. To address the question, the effects of OX1R anta

Effects of Vornorexant, a Dual Orexin Receptor Antagonist, on Bladder Function in Rats and Nocturnal Voiding Frequency in Patients With Insomnia.

OBJECTIVES: Nocturia has been reported to exacerbate sleep disturbances, such as insomnia and poor sleep quality, resulting in increased risks of falls, fractures, and mortality. These consequences significantly impact the physical health and overall prognosis of patients. Dual orexin receptor antagonists, prescribed for the treatment of insomnia, have been shown to reduce nocturnal voiding frequency. However, the mechanisms underlying this effect are not yet fully elucidated. METHODS: Intercontraction intervals and micturition pressure were measured using cystometry following intraduodenal a…

Acute administration of a dual orexin receptor antagonist attenuates sleep fragmentation in a mouse model of traumatic brain injury.

Traumatic brain injuries (TBI) induce persistent sleep dysregulation. We aimed to determine if a dual orexin receptor antagonist (DORA) could improve sleep dysregulation acutely in mice after TBI across multiple timepoints after an injury to mimic as needed clinical interventions. Male and female C57BL/6J mice (N = 5-8 per treatment group) received a TBI by controlled cortical impact or sham surgery (SHAM). Sleep architecture was assessed at baseline and after vehicle (VEH) or DORA (Lemborexant) given by gavage 24 hours (H), 2 weeks (W), 1 month (M), or 2 M post-TBI. TBI produced increases in
