Enhanced prevention of cell death by hypothermic storage with propyl gallate.

Current hypothermic preservation solutions are insufficient to prevent donor organ injury during hypothermic storage, which negatively impacts graft functional recovery and survival after transplantation. This study was to evaluate whether supplementation of the preservation solutions with antioxidant propyl gallates (PG) reduced cell injury during hypothermic storage and subsequently rewarming-reoxygenation or transplantation. Human cells and rat aorta were stored in University of Wisconsin (UW) or hyperbranched polyglycerol (HPG) solution supplemented with PG at 4 °C, followed by rewarming-r

Initial cardioplegic flush with crystalloid cardioplegia improves donor heart preservation and function via NR4A3 upregulation and metabolic reprogramming.

Static cold storage (SCS) remains the gold standard for preserving donor hearts for heart transplantation. The currently used University of Wisconsin (UW) solution has limitations in inducing rapid cardiac arrest and preservation of the heart, especially when a long cold ischemic time is required. Here, we propose a novel donor heart procurement and preservation method-crystalloid cardioplegia (CC)+UW involving an initial cardioplegic flush with crystalloid cardioplegia followed by preservation in UW solution. Our results demonstrated that compared with using UW solution alone, the CC+UW metho

Immune activation during liver machine perfusion is shaped by perfusate composition and is independent of the ischemic injury.

BACKGROUND: Ischemia-reperfusion injury remains a major challenge in liver transplantation and plays a major role in pathophysiology leading to graft failure. Machine perfusion (MP) is a promising approach to restitute marginal donor livers. However, the impact of MP on innate immunity remains insufficiently defined. We hypothesized that MP activates the innate inflammatory response. Thus, we investigated whether the ischemic injury and perfusate composition [plasma-rich perfusate (PRP) versus plasma-poor perfusate (PPP)] modulate the inflammatory response, identified by complement activation

Comparative evaluation of PEGylated bovine hemoglobin as a perfusate in rat ex vivo lung perfusion.

BACKGROUND: Ex vivo lung perfusion (EVLP) is pivotal for assessing and preserving marginal donor lungs. While red-blood-cell (RBC)-based perfusates enhance oxygen delivery, they increase pulmonary vascular resistance, thereby potentially exacerbating perfusion-induced injury. This study compared a novel acellular oxygen carrier, PEGylated bovine hemoglobin (PEG-bHb), against RBC-based and acellular perfusates in a rat EVLP-transplantation model. METHODS: A total of 49 rats were included in this study. Lungs underwent 4 h EVLP with acellular, RBC or PEG-bHb solutions, followed by left-lung tra

Association of cardiac preservation solution with short-term outcomes after heart transplantation.

AIMS: There is wide variability in the practice of cardiac preservation for heart transplantation. Prior reports suggest that the type of solution may be linked with a reduced incidence of posttransplantation complications. METHODS: Adult (≥18 years old) heart recipients who underwent transplantation between 2015 and 2021 in the United States were examined. Recipients were stratified by solution utilized for their grafts at the time of recovery: University of Wisconsin, histidine-tryptophan-ketoglutarate (HTK), or Celsior solution. The primary endpoint was a composite of 30-day mortality, pri

Applicability of the histidine-tryptophan-ketoglutarate solution as a machine perfusion solution for marginal liver grafts.

BACKGROUND AND AIM: There are very few reports comparing the use of the University of Wisconsin solution and histidine-tryptophan-ketoglutarate solution as machine perfusion solutions for marginal liver grafts. We aimed to clarify whether the use of the histidine-tryptophan-ketoglutarate solution in hypothermic machine perfusion improves the split-liver graft function in a large animal model. METHODS: Porcine split-liver grafts were created by 75% liver resection. Hypothermic machine perfusion experimental groups were divided as follows: Group 1, perfusate, University of Wisconsin gluconate s

Preservation strategies to reduce ischemic injury in kidney transplantation: pharmacological and genetic approaches.

PURPOSE OF REVIEW: In the current graft shortage, it is paramount to improve the quality of transplanted organs. Organ preservation represents an underused therapeutic window with great potential to reduce ischaemia-reperfusion injury (IRI) and improve graft quality. Herein, we review strategies using this window as well as other promising work targeting IRI pathways using pharmacological treatments and gene therapy. RECENT FINDINGS: We highlight studies using molecules administered during kidney preservation to target key components of IRI such as inflammation, oxidative stress, mitochondria

Minimal amounts of hyaluronidase in HTK or UW solution substantially improve the recovery of preserved hearts.

Rat hearts were preserved by simple storage for 18 h at 0-1 degree C and reperfused parabiotically with whole blood from a host rat. The preservation solutions used for flush perfusion and storage were the commercial solutions EuroCollins, HTK, or UW with or without adding 40 mg/l hyaluronidase or EuroFlush-Glutathione (EFG) solution, especially designed for prolonged heart storage. All solutions were filtered (0.45 micron) before use. The functional recovery was measured using a latex balloon in the left ventricle for LVP, dp/dt, and isotonic stroke volume. The metabolic recovery as well as t

Prolonged Cold Ischemia Time in Mouse Heart Transplantation Using Supercooling Preservation.

BACKGROUND: Supercooling preservation techniques store a donor organ below 0°C without freezing. This has great advantages in inhibiting metabolism and preserving the organ in comparison to conventional preservation at 4°C. We developed a novel supercooling technique using a liquid cooling apparatus and novel preservation and perfusion solutions. The purpose of this study was to evaluate the preservation effect of our supercooling preservation technique in a mouse heart transplantation model. METHODS: Syngeneic heterotopic heart transplantation was performed in 3 groups of mice: (1) the nonpr

Small animal ex vivo machine perfusion of the liver: A comprehensive literature review.

BACKGROUND: Liver transplantation is the only treatment for acute and chronic liver failure, but the global organ shortage has increased reliance on extended criteria donor livers, which are more susceptible to ischemia-reperfusion injury. While static cold storage is standard, these grafts often require improved preservation strategies. AIM: To summarize the current state of small animal liver machine perfusion (MP), highlight variability in protocols, and emphasize the need for standardization to guide future research. METHODS: A comprehensive literature search of PubMed was conducted to i

Oxygenated versus non-oxygenated flush out and storage of donor livers: An experimental study.

BACKGROUND: During donor organ procurement and subsequent static cold storage (SCS), hepatic adenosine triphosphate (ATP) levels are progressively depleted, which contributes to ischemia-reperfusion injury (IRI). We sought to investigate a simple approach to prevent ATP depletion and IRI using a porcine donation after circulatory death (DCD) liver reperfusion model. METHODS: After 30 min warm ischemia, porcine livers were flushed via the portal vein with cold (4°C) non-oxygenated University of Wisconsin (UW) preservation solution (n = 6, control group) or with oxygenated UW (n = 6, OxyFlush g

Severe Sepsis Mimicking Primary Nonfunction Following Liver Transplantation: Normothermic Machine Perfusion Is a Potential Environment for Bacterial Overgrowth and Transmission From Donor to Recipient. A Case Report.

Primary nonfunction (PNF) in the early postoperative period following liver transplantation is fatal if not managed appropriately with early retransplantation. Severe early allograft dysfunction can mimic PNF. The identification of treatable causative factors such as sepsis, hepatic artery, or portal vein thrombosis is essential to distinguish it from PNF, and their early management may avoid the need for retransplantation. In this article, we describe a case of sepsis-induced severe liver dysfunction from a contaminated graft perfused with normothermic machine perfusion (NMP), which presented

Efficacy of Amphotericin B Against Fusarium and Aspergillus in Corneal Storage Medium.

PURPOSE: The incidence of postkeratoplasty fungal infection is increasing in the United States, and our most commonly used corneal storage medium, Optisol-GS, contains antibiotics but no antifungal agents. We previously demonstrated the efficacy of amphotericin B additives in eliminating Candida albicans contaminants in Optisol-GS. The purpose of this study was to determine whether amphotericin B would also be efficacious against Fusarium solani and Aspergillus fumigatus. METHODS: Vials of Optisol-GS were supplemented with 0.255 μg/mL of amphotericin B. Half of the vials were inoculated with

Protective effects of hypothermic ex vivo perfusion on ischemia/reperfusion injury and transplant outcomes.

Hypothermic machine preservation (HMP) has been used in renal transplantation since the late 1960s with recent robust prospective, multicenter data showing lower rates of delayed graft function and improved graft survival. Although now clearly beneficial for renal transplantation, extrarenal machine perfusion has remained predominantly in preclinical investigations. Pancreatic HMP has drawn little clinical interest because HMP has been suggested to cause graft edema and congestion, which is associated with early venous thrombosis and graft failure. Early investigation showed no benefit of HMP

Comparative results of prolonged hypothermic storage of canine kidneys preserved with hyperosmolar colloid (TP-II) or crystalloid (Euro-Collins) solution.

The function of kidneys stored for 48 to 72 hours in hypertonic crystalloid, intracellular solution, Euro-Collins (360 mOsm./l.), was compared with those stored in a new hyperosmolar (520 mOsm./l.) colloid solution designated as TP-II. The TP-II solution contained less K2HPO4 (1.05 gm./l.) and KH2PO4 (317 gm./l.) and more glucose (40 gm./l.) than the Euro-Collins, and also had an albumin concentration of 4.3 gm./dl. Kidneys obtained from beagle dogs were flushed with either Euro-Collins or TP-II solution (4C). Hypothermic storage followed for either 48 or 72 hours, prior to autotransplantation

Efficiency of different storage media for avulsed teeth in animal models: a systematic review.

BACKGROUND/AIMS: Tooth avulsion consists of the complete displacement of a tooth from the alveolar socket. When immediate replantation is not possible, the avulsed tooth should be kept in a storage medium capable of maintaining the viability of periodontal ligament (PDL) cells on the root surface. However, there is no consensus on the best storage medium able to prevent sequels such as ankylosis and tooth resorption. The aim of this study was to perform a systematic review to evaluate the in vivo effectiveness of different storage media for avulsed teeth. METHODS: Two reviewers performed a da

Deceased Donor Flush Volume Similar for Histidine-Tryptophan-Ketoglutarate and University of Wisconsin at a Single US Organ Procurement Organization: Adult and Pediatric Data.

BACKGROUND: Histidine-tryptophan-ketoglutarate (HTK) and University of Wisconsin (UW) solutions are the two primary solid-organ preservation solutions used in the United States (>95%), but flush volumes vary markedly by region and center. This study analyzes data from a single organ procurement organization (OPO) to determine the actual clinical flush volumes used for HTK and UW for liver and pancreas grafts. METHODS: All procurements at Indiana Donor Network were analyzed (2016-2020), and data were extracted from the on-site records. Variables included procuring center, solution, volumes, an

The effect of storage solutions, gene therapy, and antiproliferative agents on endothelial function and saphenous vein graft patency.

Vein graft failure remains a major concern after coronary artery bypass graft operations, and is initiated by loss of endothelial cell integrity. Preservation of saphenous vein grafts in the optimal solution after meticulous harvesting can limit the endothelial damage. Despite both experimental and clinical results in favor of buffered solutions, normal saline is still the most widely used solution. This review examines the literature to identify the most optimal storage solutions currently available for vein graft preservation.

Does University of Wisconsin solution harm the transplanted heart?

BACKGROUND: University of Wisconsin solution (UW) has been shown to be an effective preservative for the cardiac allograft. Recently, the high potassium content of UW has been implicated in causing coronary endothelial damage, allegedly contributing to development of cardiac allograft vasculopathy (CAV) and eventually to poorer survival. METHODS: We examined our experience using UW for preservation of cardiac allografts between 1990 and 1994 (n = 94), and compared these to hearts preserved with the lower potassium-containing Stanford solution used at our center between 1986 and 1990 (n = 65).

Successful 20-hour rat liver preservation with chlorpromazine in sodium lactobionate sucrose solution.

We investigated the effect of the addition of chlorpromazine to a new, simplified organ preservation solution, sodium lactobionate sucrose (SLS), for 20-hour hypothermic rat liver preservation. Survival beyond 7 days after orthotopic transplantation of the stored liver was eight of eight rats in control groups (immediate transplantation, less than 1-hour preservation), one of 14 rats with the University of Wisconsin (UW) solution, four of 14 rats with SLS, seven of eight rats with SLS + chlorpromazine, 1 mg/L, and seven of eight rats with SLS + chlorpromazine, 10 mg/L. The differences is survi
