Intracellular invasion of Orientia tsutsugamushi activates inflammasome in asc-dependent manner.

Orientia tsutsugamushi, a causative agent of scrub typhus, is an obligate intracellular bacterium, which escapes from the endo/phagosome and replicates in the host cytoplasm. O. tsutsugamushi infection induces production of pro-inflammatory mediators including interleukin-1β (IL-1β), which is secreted mainly from macrophages upon cytosolic stimuli by activating cysteine protease caspase-1 within a complex called the inflammasome, and is a key player in initiating and maintaining the inflammatory response. However, the mechanism for IL-1β maturation upon O. tsutsugamushi infection has not been

New genotypes of Orientia tsutsugamushi isolated from humans in Eastern Taiwan.

Scrub typhus, an acute febrile illness, is caused by the obligate intracellular bacterium Orientia tsutsugamushi. In our study, O. tsutsugamushi was rapidly detected and typed by polymerase chain reaction (PCR) and restriction fragment length polymorphism (RFLP) analysis of the 56-kDa type-specific antigen (TSA) gene. To investigate the genotypes of clinical variants of O. tsutsugamushi, we collected 3223 blood samples from eastern Taiwanese patients with suspected scrub typhus from 2002 to 2008. In total, 505 samples were found to be positive for scrub typhus infection by PCR, and bacteria we

[Molecular typing of Taiwanese Orientia tsutsugamushi isolates by restriction fragment profile].

Scrub typhus, which is caused by Orientia tsutsugamushi, is one of the reportable diseases in Taiwan, ROC. Positive cases seemed to be increasing in recent years and our laboratory had confirmed a total of 247 cases in 1997. In this study, 71 strains of O. tsutsugamushi were isolated from heparinized blood of the patients by cell culture, and their DNA was isolated. Two-step polymerase chain reaction was then used to amplify a specific sequence from the DNA, which was digested with restriction enzymes, Hha I and Sfa NI, for typing. With this technique, 34 strains gave rise to the same profile

Analysis of outer membrane protein from Orientia tsutsugamushi for epitope mining.

Scrub typhus, caused by Orientia tsutsugamushi, is an emerging health threat that can lead to severe complications if untreated. Therefore, it is of interest to analyse 130 sequences of the 56 kDa outer membrane protein, a key immunodominant antigen. SNP profiling identified 300 conserved and 8 highly variable residues. Epitope prediction revealed 73 conserved and 6 variable B cell epitopes, along with 4 conserved and 1 variable CD8+ T cell epitopes and 3 conserved and 1 variable CD4+ T cell epitopes. Notably, five immunogenic regions were predicted to activate B cells and both CD8+ and CD4+ T

Transmission Cycle of Shimokoshi-Type Orientia tsutsugamushi in Shimane Prefecture, Japan.

To demonstrate the transmission cycle of Shimokoshi-type Orientia tsutsugamushi in Shimane Prefecture, field rodents were captured from areas where four human infections caused by the pathogen have been reported. The rodents were investigated for the transmission cycle of the pathogen based on the pathogen's genome, antibodies against the pathogen, and the vector of the pathogen (Leptotrombidium palpale). In addition, the vector was captured from the soil in the study area. A total of 44 rodents were captured. No O. tsutsugamushi DNA was detected in the blood or spleen samples by real-time pol

Scrub Typhus and Molecular Characterization of Orientia tsutsugamushi from Central Nepal.

Scrub typhus is a vector-borne, acute febrile illness caused by Orientia tsutsugamushi. Scrub typhus continues to be an important but neglected tropical disease in Nepal. Information on this pathogen in Nepal is limited to serological surveys with little information available on molecular methods to detect O. tsutsugamushi. Limited information exists on the genetic diversity of this pathogen. A total of 282 blood samples were obtained from patients with suspected scrub typhus from central Nepal and 84 (30%) were positive for O. tsutsugamushi by 16S rRNA qPCR. Positive samples were further subj

Bioinformatic Exploration of Metal-Binding Proteome of Zoonotic Pathogen Orientia tsutsugamushi.

Metal ions are involved in many essential biological processes and are crucial for the survival of all organisms. Identification of metal-binding proteins (MBPs) of human affecting pathogens may provide the blueprint for understanding biological metal usage and their putative roles in pathogenesis. This study is focused on the analysis of MBPs from Orientia tsutsugamushi (Ott), a causal agent of scrub typhus in humans. A total of 321 proteins were predicted as putative MBPs, based on sequence search and three-dimensional structure analysis. Majority of proteins could bind with magnesium, and t

Central Nervous System Infection Associated with Orientia tsutsugamushi in South Korea.

Orientia tsutsugamushi is a major cause of vector-borne infection in Asia. Prompt recognition and appropriate treatment are crucial because of its potentially fatal complications and lack of response to beta-lactam antibiotics. The present study retrospectively evaluated the clinical characteristics and laboratory findings of 16 patients with scrub typhus-related central nervous system (CNS) infections. Single titers ≥ 1:40 of total serum antibodies against O. tsutsugamushi detected by an indirect immunofluorescent assay were considered as positive results. The median age was 35.5 (range, 14-7

Orientia tsutsugamushi Modulates Endoplasmic Reticulum-Associated Degradation To Benefit Its Growth.

Orientia tsutsugamushi, an obligate intracellular bacterium that is auxotrophic for the aromatic amino acids and histidine, causes scrub typhus, a potentially deadly infection that threatens 1 billion people. O. tsutsugamushi growth is minimal during the first 24 to 48 h of infection but its growth becomes logarithmic thereafter. How the pathogen modulates cellular functions to support its growth is poorly understood. The unfolded protein response (UPR) is a cytoprotective pathway that relieves endoplasmic reticulum (ER) stress by promoting ER-associated degradation (ERAD) of misfolded protein

A Systematic Review of Mortality from Untreated Scrub Typhus (Orientia tsutsugamushi).

BACKGROUND: Scrub typhus, a bacterial infection caused by Orientia tsutsugamushi, is increasingly recognized as an important cause of fever in Asia, with an estimated one million infections occurring each year. Limited access to health care and the disease's non-specific symptoms mean that many patients are undiagnosed and untreated, but the mortality from untreated scrub typhus is unknown. This review systematically summarizes the literature on the untreated mortality from scrub typhus and disease outcomes. METHODOLOGY/PRINCIPAL FINDINGS: A literature search was performed to identify patient

Orientia tsutsugamushi bacteremia and cytokine levels in Vietnamese scrub typhus patients.

Scrub typhus, caused by the intracellular bacterium Orientia tsutsugamushi, is a major cause of febrile illness in the Asia/Pacific region. Here, we implemented a novel real-time PCR and determined the relation of DNA target gene concentration with serum cytokine levels. The limit of detection of the novel real-time PCR was 1,062 DNA copies per ml of EDTA whole blood. Specificity was excellent as determined on a panel of blood- and skin-borne bacteria, including Rickettsia spp. as well as healthy Vietnamese blood donors. Bacterial DNA concentrations after 9 to 12 days from symptoms onset were

Tissue distribution of Orientia tsutsugamushi in rodents: a molecular and immune diagnostic approach.

BACKGROUND: Scrub typhus is caused by a bacterium Orientia tsutsugamushi, which is transmitted to humans by infected chigger mites found in scrub vegetation. This study examined the tissue distribution of O. tsutsugamushi in rodents collected from different districts in Kerala, India. Tissue tropism helps to understand disease spread to cells and tissues, leading to inflammation and multi-organ failure. METHODS: O. tsutsugamushi infection was screened using molecular and immunological diagnostic tools in 137 rodents. Of these, 78 were positive for O. tsutsugamushi in blood samples by both mol

Orientia tsutsugamushi meningitis in a patient with tuberculous meningitis complications-a Case Report.

Scrub typhus (ST) is caused by infection with Orientia tsutsugamushi, transmitted through chigger mite bites, resulting in a febrile disease. Clinical manifestations are complex and may be combined with other microbial infections. This report describes a rare case of scrub typhus meningitis and tuberculous meningitis. The patient presented with fever and chills, and also exhibited a dull reaction upon admission. Timely diagnosis and appropriate antibiotic therapy are critical for preventing complications and reducing mortality. Next-generation sequencing (NGS) provided diagnostic clues, and th

Targeted capture and sequencing of Orientia tsutsugamushi genomes from chiggers and humans.

Scrub typhus is a febrile disease caused by Orientia tsutsugamushi, transmitted by larval stage Trombiculid mites (chiggers), whose primary hosts are small mammals. The phylogenomics of O. tsutsugamushi in chiggers, small mammals and humans remains poorly understood. To combat the limitations imposed by the low relative quantities of pathogen DNA in typical O. tsutsugamushi clinical and ecological samples, along with the technical, safety and cost limitations of cell culture, a novel probe-based target enrichment sequencing protocol was developed. The method was designed to capture variation a

Distribution of Orientia tsutsugamushi in rodents and mites collected from Central India.

Orientia tsutsugamushi, the causative agent of scrub typhus, is an obligate intracytosolic bacterium transmitted among humans and small mammals by some species of larval trombiculid mites (chiggers). It has been recognized as a pathogen of major public health concern in the Asia-Pacific region. As disease is considered as a neglected, there exists a gap in our knowledge of the disease with regard to the sporadic epidemiologic data in endemic areas. The purpose of the study was to find out the vector as well as pathogen distribution in rodents present in the scrub typhus-reported areas in centr

Comparison of a conventional and nested PCR for diagnostic confirmation and genotyping of Orientia tsutsugamushi.

A nested polymerase chain reaction (PCR) targeting the 56-kDa antigen gene is currently the most commonly used molecular technique for confirmation of scrub typhus and genotyping of Orientia tsutsugamushi. In this study, we have compared the commonly used nested PCR (N-PCR) with a single-step conventional PCR (C-PCR) for amplification and genotyping. Eschar samples collected from 24 patients with scrub typhus confirmed by IgM enzyme-linked immunosorbent assay were used for DNA extraction following which amplifications were carried out using nested and C-PCR methods. The amplicons were sequence

A case series of possibly recrudescent Orientia tsutsugamushi infection presenting as pneumonia.

Orientia tsutsugamushi remains latent in humans after scrub typhus. Pneumonia occurs as a complication of scrub typhus in the late-phase disease. However, pneumonia may also occur as a presenting manifestation of O. tsutsugamushi infection. We reviewed the cases of 3 patients with atypical pneumonia who presented at our hospital and were later confirmed to have O. tsutsugamushi infection by serology, nested polymerase chain reaction (PCR), and cell culture. All patients were young adults with no history of scrub typhus, and none claimed to have recently been exposed to areas where scrub typhus

Dissemination of Orientia tsutsugamushi and inflammatory responses in a murine model of scrub typhus.

Central aspects in the pathogenesis of scrub typhus, an infection caused by Orientia (O.) tsutsugamushi, have remained obscure. Its organ and cellular tropism are poorly understood. The purpose of this study was to analyze the kinetics of bacterial dissemination and associated inflammatory responses in infected tissues in an experimental scrub typhus mouse model, following infection with the human pathogenic strain Karp. We provide a thorough analysis of O. tsutsugamushi infection in inbred Balb/c mice using footpad inoculation, which is close to the natural way of infection. By a novel, highl
