Navigating regulatory challenges in pediatric epilepsy pharmacotherapy: pathways to safe and effective treatments.

INTRODUCTION: Drug development for epilepsy, the most frequent chronic neurologic childhood condition, faces fundamental challenges. One of these is the concept of children as 'therapeutic orphans' and the resulting pediatric requirements of the US Food and Drug Administration (FDA) and the European Medicines Agency (EMA). Developing new antiseizure medications (ASMs) is expensive. Public coffers and reimbursement institutions are increasingly struggling with fundings. Commercial drug developers are progressively skeptical about the risks of developing new ASMs, which makes a critical review o

MicroRNAs and Long Non-coding RNAs in Genetic Diseases.

Since the discovery and classification of non-coding RNAs, their roles have gained great attention. In this respect, microRNAs and long non-coding RNAs have been firmly demonstrated to be linked to regulation of gene expression and onset of human diseases, including rare genetic diseases; therefore they are suitable targets for therapeutic intervention. This issue, in the context of rare genetic diseases, is being considered by an increasing number of research groups and is of key interest to the health community. In the case of rare genetic diseases, the possibility of developing personalized

Nelarabine: new drug. T-lymphoblastic leukaemia/lymphoma: more evaluation needed.

1) Acute T-lymphoblastic leukaemia and T-lymphoblastic lymphoma are closely related malignant haemopathies. There is a better prognosis for children with these disorders than for adults. There is no consensus treatment in case of relapses. However, only haematopoietic stem cell transplantation following chemotherapy offers a chance of long-term survival; 2) Nelarabine is a purine analogue closely related to clofarabine, fludarabine and cytarabine. It is marketed for the treatment of children and adults with one or the other of these two malignant haemopathies, after failure of at least two lin

Orphan drug propranolol for infantile hemangioma: ten-year real-world safety data from the FAERS database.

OBJECTIVE: Propranolol, a non-selective β-adrenergic receptor blocker, is the first FDA-approved orphan drug for the treatment of infantile hemangioma (IH). While it has demonstrated substantial therapeutic benefits, the safety profile of propranolol in infants remains insufficiently characterized, particularly in real-world settings. METHODS: A retrospective pharmacovigilance analysis of FAERS reports from Q1 2015 to Q4 2024 was conducted, applying four signal detection algorithms (ROR, PRR, BCPNN, MGPS) to identify disproportional AE signals related to propranolol in IH. RESULTS: A total of

Value-based decision-making for orphan drugs with multiple criteria decision analysis: burosumab for the treatment of X-linked hypophosphatemia.

OBJECTIVE: Burosumab is an orphan medicinal product (OMP) approved in Europe for the treatment of X-linked hypophosphatemia (XLH). The aim of this study was to assess the value of burosumab versus conventional therapy for the treatment of paediatric XLH, through a multi-criteria decision analysis (MCDA) framework for health technology assessment (HTA) of OMPs in Portugal. METHODS: The MCDA framework considered 14 criteria related to disease burden, therapeutic value and economic burden. A multidisciplinary panel of national stakeholders participated in a two-phase exercise. In the first phase

[Pharmaceutical development concerning diseases predominating in tropical regions: the concept of indigent drugs].

When the WHO certified the eradication of smallpox in 1981, there was a general impression that the fight against infectious diseases which began with Jenner and Pasteur was entering a phase of achievement: poliomyelitis, dracunculasis, leprosy, Chagas' disease and neonatal tetanus were also responding to eradication campaigns. However, in 1995, infectious diseases are still an important cause of mortality and morbidity and the rising incidence of emerging or re-emerging diseases remains a matter of great concern. Although this situation can be explained, at least partly, by the deterioration

Revealing the macromolecular targets of complex natural products.

Natural products have long been a source of useful biological activity for the development of new drugs. Their macromolecular targets are, however, largely unknown, which hampers rational drug design and optimization. Here we present the development and experimental validation of a computational method for the discovery of such targets. The technique does not require three-dimensional target models and may be applied to structurally complex natural products. The algorithm dissects the natural products into fragments and infers potential pharmacological targets by comparing the fragments to syn

Pricing and reimbursement of orphan drugs: the need for more transparency.

Pricing and reimbursement of orphan drugs are an issue of high priority for policy makers, legislators, health care professionals, industry leaders, academics and patients. This study aims to conduct a literature review to provide insight into the drivers of orphan drug pricing and reimbursement. Although orphan drug pricing follows the same economic logic as drug pricing in general, the monopolistic power of orphan drugs results in high prices: a) orphan drugs benefit from a period of marketing exclusivity; b) few alternative health technologies are available; c) third-party payers and patien

[Drugs for 'neglected diseases': a bitter pill].

Neglected diseases are diseases restricted to poor areas; diseases for which there exist no commercial incentives to invest in the development of new treatments. Patients suffering from these diseases often have no access to essential drugs; this can be a matter of life and death. Lack of research and patent protection play a crucial role. Both are cost driven; greed in the West curtails the availability of life-saving drugs for all. Treatment options for trypanosomiasis and visceral leishmaniasis are lacking. It is recommended that drugs for the treatment of 'neglected diseases' be developed

Gastric neuroendocrine tumors: prevalence in Europe, USA, and Japan, and rationale for treatment with a gastrin/CCK2 receptor antagonist.

OBJECTIVE: Gastric carcinoids (neuroendocrine tumors) arise from enterochromaffin-like cells in the gastric mucosa. Most are caused by hypergastrinemia. The objectives were to determine if their prevalence in Europe, USA and Japan meets the criteria for an orphan disease and to justify treatment with a gastrin/CCK2 receptor antagonist. METHODS: We obtained data from European and USA cancer registries, and searched PubMed. RESULTS: Prevalence per 10,000 population obtained from cancer registries was: median 0.32 (range 0.09-0.92) for Europe; and 0.17 for the USA, equivalent to 4812 for the wh

Towards regulatory endorsement of drug development tools to promote the application of model-informed drug development in Duchenne muscular dystrophy.

Drug development for rare diseases is challenged by small populations and limited data. This makes development of clinical trial protocols difficult and contributes to the uncertainty around whether or not a potential therapy is efficacious. The use of data standards to aggregate data from multiple sources, and the use of such integrated databases to develop statistical models can inform protocol development and reduce the risks in developing new therapies. Achieving regulatory endorsement of such models through defined pathways at the US Food and Drug Administration and European Medicines Aut

Applying the win ratio method in clinical trials of orphan drugs: an analysis of data from the COMET trial of avalglucosidase alfa in patients with late-onset Pompe disease.

BACKGROUND: Clinical trials for rare diseases often include multiple endpoints that capture the effects of treatment on different disease domains. In many rare diseases, the primary endpoint is not standardized across trials. The win ratio approach was designed to analyze multiple endpoints of interest in clinical trials and has mostly been applied in cardiovascular trials. Here, we applied the win ratio approach to data from COMET, a phase 3 trial in late-onset Pompe disease, to illustrate how this approach can be used to analyze multiple endpoints in the orphan drug context. METHODS: All po

[Access to orphan drugs for the treatment of spinal muscular atrophy in Spain].

INTRODUCTION: Spinal muscular atrophy (SMA) is a rare disease whose diagnosis and treatment are complex. In Spain, there are two orphan medicines that are currently financed by the state, nusinersen and onasemnogene abeparvovec and, a third in process, risdiplam. The objective was to detect possible causes of inequity in the diagnosis and treatment of SMA in Spain. MATERIALS AND METHOD: Descriptive study realized in two phases: a first phase of bibliographic revision and a second phase of semi-structured interviews with clinical experts in SMA in Andalusia, Castilla-La Mancha, Catalonia and M

Reassessing R&D outcomes of leading pharmaceutical companies.

This analysis investigated 118 new drugs approved by the FDA for leading pharmaceutical companies between 2002 and 2012 and their commercial outcome through 2024. Focusing on R&D effectiveness, we found that 67% generated sufficient revenues to recover estimated standard R&D costs. R&D profitability varied by sourcing strategy, orphan drug designation status and therapeutic area, whereas therapeutic novelty and regulatory innovativeness showed no meaningful difference. Overall, the findings indicate that R&D productivity of large pharmaceutical companies might be more effective than previously

Valuation of Medical Innovation in Orphan Diseases with a Focus on Small Investors and Limited Diversifiable Risks.

This paper assesses the impact of uncertainty for investors on the economic valuation of medical innovation projects for orphan drugs or rare diseases. Conventionally, investor evaluation uses the deterministic discounted cash flow (DCF) method with an appropriate sensitivity analysis that captures some level of uncertainty. In healthcare, and particularly for rare diseases, the levels of uncertainty in financial outcomes (return on investment and net present value (NPV)) are broader than the ones normally captured by the DCF formula. Uncertainties include R&D costs, the approval process (leve

Priced Out of Treatment: The Exorbitant Cost of Antiparasitic Drugs in the United States.

Parasitic infections are often perceived as diseases of low-resource countries, yet they impose a significant and overlooked burden in the United States-especially among immigrants, low-income individuals, and other marginalized populations. This perspectives article examines the case of Mr. X, a landscaper in Houston diagnosed with neurocysticercosis, who faced unaffordable drug costs despite the availability of effective, off-patent treatments. Through his story, we explore the broader issue of pricing for commonly and not-so-commonly prescribed antiparasitic medications, highlighting the ro
