Long-term experience with biocompatible ossicular implants.

The senior author's 8-year personal experience with biocompatible ossicular implants is reviewed. Four hundred sixty-one consecutive operations, in which high-density polyethylene sponge ossicular replacement prostheses were used, are grouped according to the Bellucci classification of chronic otitis media. The prostheses used were the drum-to-footplate prosthesis (TORP, total) and the drum-to-stapes prosthesis (PORP, partial). Each group's short- and long-term hearing results are compared. Prosthesis extrusion and persistent or recurrent conductive hearing loss are the most common causes of o

Total Ossicular Chain Reconstruction: Is Autologous Material Better Than a TORP?

INTRODUCTION: Total ossicular chain reconstruction may be performed using autologous cartilage or bone, or with a total ossicular replacement prosthesis (TORP). It is unclear if one method offers better hearing outcomes. The goal of this study was to compare hearing outcomes following total ossicular chain reconstruction with autologous material or TORPs. METHODS: A single-institution prospective database was queried to identify patients who underwent total ossicular chain reconstruction and had available audiometric tests. Preoperative and postoperative air-bone gaps (ABG) at 0.5 to 4 kHz we

[Implantable hearing aids].

The purpose of these hearing aids is to improve the quantity and quality of amplification of the sound wave. Their operating principle is based on the existence of a transducer which transforms the electric signal captured by the microphone into a mechanical, vibrating wave. This transducer can act either directly on the temporal bone by short-circuiting the middle ear (and is then called a bone conduction implant) or directly on the ossicular chain (in which case it is called an ossicular transduction implant). A) There are 2 types of bone conduction implant: the "Audiant Bone Conduction" sys

Factors influencing hearing outcomes in pediatric patients undergoing ossicular chain reconstruction.

OBJECTIVE: Ossicular chain disruption in children leads to conductive hearing loss. Few studies have focused on factors influencing successful results in pediatric ossicular chain reconstruction (OCR). We aim to determine whether demographic or surgical factors affect hearing outcomes in pediatric OCR. METHODS: We conducted a retrospective chart review of 120 patients undergoing OCR at our institution, a tertiary care hospital, between 2003 and 2014, with median length of follow-up of 2.2 years (range 0.1-9.3 years). Pediatric patients (<18 years old at time of surgical procedure) who had cur

The "boomerang" malleus-incus complex in congenital aural atresia.

"Boomerang" malleus-incus fusion deformity is identified on axial high-resolution CT in a subset of patients with congenital aural atresia, and it is associated with an absent incudostapedial joint and stapes capitulum and attachment of the incus to the tympanic segment of the facial nerve canal. Twelve patients with this deformity were identified on a retrospective review of imaging from a cohort of 673 patients with congenital aural atresia, with surgical confirmation in 9 of these patients. Eight of 9 patients underwent partial ossicular replacement prosthesis reconstruction with improvemen

Impact of Prosthesis Length on Tympanic Membrane's and Annular Ligament's Stiffness and the Resulting Middle Ear Sound Transmission.

HYPOTHESIS: Prosthesis' length creates tension in ossicular reconstructions, which directly effects the middle ear sound transmission. BACKGROUND: Relatively long prostheses are often used to stabilize the middle ear reconstruction to prevent dislocation. Thereby, tension on the flexible components such as the tympanic membrane (TM) and the annular ligament (AL) is increased. Only little is known on the amount of displacement-related stiffening of the TM and AL, as well as the consecutive reduction in middle ear transfer function (METF). METHODS: An expandable total ossicular replacement pro

Alternative fixation of an active middle ear implant at the short incus process.

INTRODUCTION: Since 1996, the preferred approach for positioning the active middle-ear implant Vibrant Soundbridge© is a mastoidectomy and a posterior tympanotomy. With this device, placement of the floating mass transducer (FMT) on the long incus process is the standard method for treatment of mild-to-severe sensorineural hearing loss in the case of normal middle-ear anatomy. The aim of this study was to determine the vibrational effectiveness of FMT placement at the short incus process. MATERIALS AND METHODS: An extended antrotomy and a posterior tympanotomy were performed in 5 fresh human

Revision ossiculoplasty.

Although ossiculoplasty, also known as ossicular chain reconstruction (OCR), was attempted initially in the early 1900s, it was not until the 1950s that it became commonplace and relatively well understood. Since then, there have been numerous technologic advances and a gain in the understanding of ossiculoplasty. However, successful OCR with resulting long-term stability can be a daunting task. Typically, the most common condition requiring revision OCR is chronic suppurative otitis media (COM) with or without cholesteatoma. Primary and revision OCR are performed also for blunt and penetratin

Experimental Simulation of Clinical Borderline Situations in Temporal Bone Specimens After Ossiculoplasty.

OBJECTIVES: One reason for insufficient hearing improvement with a distinct air-bone gap after ossiculoplasty with implantation of partial or total ossicular replacement prostheses can be the dislocation or minimal shifting of the prosthesis. The aim of this study was the simulation of common clinical borderline situations with minimal shifting of the prosthesis in temporal bone specimens after ossiculoplasty. It was furthermore the goal to identify these specific situations through imaging by cone beam computed tomography (cbCT) and direct visual inspection using the operation microscope. Add

Investigation of the coupling quality of partial prostheses at the stapes head.

INTRODUCTION: The success of partial ossicular chain reconstructions in cases of conductive hearing loss depends on many factors. One important factor is the coupling between the prosthesis and the stapes head, which has not been explicitly investigated previously. Prostheses with different forms of attachment to the stapes are available, namely clip and bell type PORP. We present a standardized method to assess the quality of the prosthesis-stapes connection. The coupling quality of different prostheses is compared using measurements on a specimen model. METHODS: This study delineated six gr

[Endoscopically guided reconstruction of the ossicular chain-an introduction].

The term "endoscopic ossiculoplasty" refers to surgical methods with the intention to reconstruct the ossicular chain using endoscopic vision. Apart from malformations and injuries, inflammatory processes cause the majority of indications for ossicular reconstruction. This article offers a commented overview of current literature and preliminary personal experience.

[The omega connector--a module for jointed coupling of titanium total prostheses in the middle ear].

BACKGROUND: Hearing improvement after reconstruction of a defect ossicular chain depends on material, design and - crucially - coupling of the prosthesis. Coupling a total ossicular replacement prosthesis to the stapes footplate can be problematic and lead to prosthesis instability. In order to solve this problem, the Omega Connector was developed, a module allowing the middle ear surgeon to couple a titanium total prosthesis to the stapes footplate in a flexible, angle-variable manner. MATERIAL AND METHOD: The Omega Connector is made of pure titanium and consists of three compenents: head, n
