Surgery for Malignant Submandibular Gland Neoplasms.

For many decades, surgery has been the primary treatment for malignant submandibular gland neoplasms. Nonetheless, due to the heterogeneity and rarity of submandibular gland malignant tumors and the high frequency of chronic benign processes in this region, management can be complex. Preoperative investigations, such as fine-needle aspiration and imaging, are critical to achieve the correct diagnosis so that appropriate surgery can be planned. In general, for malignant submandibular gland neoplasms, the minimal treatment necessary is excision of the submandibular gland with level I lymph node

[ORL].

This 2014 version of news in ENT--established by reading more than 10,000 abstracts published in 2014--will focuss this year on two main topics that are trendy: Robotic surgery in ENT, and e-cigarette. Of course, other subjects will be treated as well, in the format of guidelines, like for otitis externa, tinnitus, ear grommets, Bell's palsy, children's sinusitis, topical treatments in sinusitis, or dangers of radiation on kids. Some interesting mobile applications will be as well presented.

[Manifestation of tuberous sclerosis in the ENT area].

Tuberous sclerosis (Bourneville-Pringle's disease) is a rare, largely autosomal dominant neurocutaneous disease. The disease can also result from spontaneous mutations. Although strongly variable in its manifestation, manifestations are typically characterized by involvement of the central nervous system (early childhood seizures), skin (facial angiofibromas) and kidneys (angiomyolipomas). In the case described, a 67-year-old female patient complained exclusively of obstructed nasal breathing that was found to be due to angiofibromas in the nasal vestibule. Oral fibromas were asymptomatic, whi

[An unusual form of neoplastic lymphangitis in an irradiated field].

The authors describe an exceptional form of neoplastic lymphangitis occurring in old people some years after surgery and irradiation of a cancer (parotid, breast), the special character of which is that it draws exactly the fields of irradiation. This lymphangitis is at first smooth and later covered with neoplastic nodes which join together. Evolution is in any case the death after some weeks or months. Different pathogenetic hypotheses are discussed, involving the mechanisms of biology of cancer and radiations.

Update on management of frontal sinus osteomas.

PURPOSE OF REVIEW: Endoscopic approaches are used increasingly as an alternative to external approaches in removal of frontal sinus osteoma. The purpose of this article is to review and highlight the recent articles on the management of frontal sinus osteoma over the past 24 months. RECENT FINDINGS: The evolution in endoscopic sinus surgery has allowed rhinologists to expand its indications in managing a wide range of frontal sinus pathologies, including frontal sinus osteomas. The surgeons are attempting endoscopic removal of higher grades of osteomas from frontal sinus, previously thought t

[Otolaryngological cancer pain at the after-effects stage].

The standards of pharyngolaryngeal tumor treatment have changed over the years in an attempt to prevent laryngeal mutilation (partial surgery, endoscopic surgery, sequential radiotherapy, and chemotherapy). Pain induced by these treatments is frequent and varies from one treatment to another. Chemoradiotherapy induces less pain but often more severe pain, since 20% of these situations are not controlled by strong opioids. Pain from mucositis, although nonspecific to otolaryngic cancer, is more frequent and more severe, and prevention and treatment remain poorly defined. Pain from postradiother

Endoscopic sinus surgery for benign and malignant nasal and sinus neoplasm.

PURPOSE OF REVIEW: Endoscopic sinus surgery has become widely accepted as the standard of treatment for chronic inflammatory diseases of the paranasal sinuses unresponsive to medical treatment. With increased skill with endoscopic surgical technique, advanced technologies such as intraoperative imaging systems, and a better understanding of the complex anatomy of the paranasal sinuses and surrounding vital structures, many otolaryngologists have increasingly applied their expertise in endoscopic sinus surgery to the resection of nasal and sinus neoplasms. The following represents a review of t

Surgery for Malignant Sublingual and Minor Salivary Gland Neoplasms.

Malignant sublingual gland neoplasms are rare, early-stage neoplasms presenting as painless non-ulcerated masses in the antero-lateral floor of the mouth. The majority of patients present with advanced disease, with symptoms of pain or anaesthesia of the tongue. Malignant minor salivary gland neoplasms are more common, the majority (>80%) of which present in the oral cavity, most frequently in the palatal area, as painless masses or as obstructive symptoms in the head and neck region. The most frequent pathologies are adenoid cystic carcinoma and mucoepidermoid carcinoma (>85%), with the major

[Immunoglobulin G4 (IgG4)-related disease. A review of head and neck manifestations].

Immunoglobulin G4 (IgG4)-related disease (also known as hyper-IgG4 disease) is a recently defined emerging condition with highly heterogeneous clinicopathological features and variable disease manifestations. This disorder is characterized by unifocal or multifocal (multiorgan) involvement by tumefactive plasma cell-rich inflammatory infiltrates associated with prominent fibrosclerosis. This not uncommonly interferes with organ function resulting in diverse clinical symptoms. The autoimmune pancreatitis represents the prototype of this disease; however, to date almost all organs have been repo

Preoperative vestibular ablation with gentamicin and vestibular 'prehab' enhance postoperative recovery after surgery for pontine angle tumours--first report.

CONCLUSIONS: Preoperative gentamicin in combination with vestibular 'prehab' offers a possibility to reduce postoperative malaise and speed up recovery and may be used for patients undergoing such surgery when there is remaining vestibular function. OBJECTIVES: Removal of pontine angle tumours in a patient with remaining vestibular function causes symptoms of acute vestibular loss. A simultaneous cerebellar lesion can cause a combined vestibule-cerebellar lesion. PATIENTS AND METHODS: Twelve patients with pontine angle tumours but with near normal vestibular function were treated with intrat

[Incidence of respiratory tract system tumors in Italy: data from the Tumor Registry].

Nine population-based cancer Registries are active in Italy, covering about one tenth of the population. Six Registries (Torino, Varese, Trieste, Genova, Parma, and Forlì) are located in the North of Italy, two Registries (Firenze and Latina) in the Center and one (Ragusa) in the South. Incidence data (total number of registered cases; age-specific, age-standardized, truncated, and cumulative rates) and indices of reliability (% of cases with histological verification of diagnosis; % of cases registered only as a result of death certification; and mortality/incidence ratio) are tabulated for t

Carotid Body Tumor: Clinical Features.

Although carotid body tumor (CBT) is a rare neoplasm, it should always be considered in differential diagnosis of lateral neck masses. We shared the 10 years of CBT experience in our clinic and started a discussion on CBT with literature support. A total of 21 patients with CBT diagnosis in Fırat University Hospital, Otorhinolaryngology Clinic, participated in the study. Patients were evaluated based on demographical data and particularities of the tumor. Participant patients were 19 women and 2 men, and their ages were between 21 and 79 (mean age 54.06 ± 12.48). Orbital Epstein-Barr virus associated smooth muscle tumor in an immunocompromised patient with T-cell deficiency.

Epstein-Barr Virus-associated smooth muscle tumors (EBV-SMTs) are rare neoplasms that primarily occur in immunocompromised individuals. Orbital EBV-SMTs are an exceptionally rare subset of these tumors which may cause mass effect that leads to functional impairment. We report a case of a patient with systemic lupus erythematosus on long-term corticosteroids who was later found to have T cell deficiency and markedly reduced CD4 and CD8 counts. She presented with a rapidly enlarging right orbital mass causing proptosis, optic nerve compression, and visual dysfunction. Imaging revealed an extraco

Lateral hypopharyngectomy with laryngeal preservation reconstructed with inlay fascio-cutaneous free flaps: clinical and functional outcomes.

OBJECTIVE: Lateral hypopharyngectomy (LH) is one of the organ-preservation surgical strategies available for treatment of selected naïve early squamous cell carcinoma (SCC), as well as for rarer non-SCC tumours and persistent/recurrent/second primaries after chemoradiation of the lateral wall of the piriform sinus. Its reconstructive methods have been the subject of different approaches without a general consensus. The aim of the present study is to describe a retrospective series of LH reconstructed by inlay fascio-cutaneous free flaps, reporting on oncological and functional outcomes. METHO
