Outcome of sex reassignment surgery for transsexuals.

Fifteen years ago the author reviewed the world literature on male transsexualism (Pauly, 1965). Subsequently he summarized the results of sex reassignment surgery for male and female transsexuals (Pauly, 1968), and reviewed the literature on female transsexualism (Pauly, 1974). Very recently, Meyer and Reter (1979) concluded that 'sex reassignment surgery confers no objective advantage in terms of social rehabilitation' as compared with a group of individuals who sought sex reassignment but remained unoperated upon at follow-up. Both groups improved over time and led the Johns Hopkins Gender

[Laparoscopic excision versus open excision for the treatment of choledochal cysts: a systematic review and meta-analysis].

OBJECTIVE: To determine if laparoscopic excision is more effective than open excision in the treatment of choledochal cysts. MATERIAL AND METHODS: A systematic review of randomized clinical trials in 3 databases measuring the efficacy of laparoscopic and open excision of choledochal cysts was performed. The authors considered international and national reports, whose results were analyzed in detail. RESULTS: Mean duration of laparoscopic excision was 51 min, open excision - 35.4 min. Length of hospital-stay after laparoscopic excision ranged between 5 and 74 days, after open excision - betwe

Surgical pathology to describe the clinical margin of debridement of chronic wounds using a wound electronic medical record.

BACKGROUND: Chronic wounds, including diabetic foot ulcers (DFU), pressure ulcers (PU), and venous ulcers (VU) result from multiple physiologic impairments. Operative debridement is a mainstay of treatment to remove nonviable tissue and to stimulate wound healing. Unlike tumor resection, however, operative wound specimens are not routinely sent for pathology. The objective of this study was to describe the pathology present in chronic wounds. STUDY DESIGN: Pathology reports of the skin edge and wound base from 397 initial debridements in 336 consecutive patients with chronic wounds were retro

Diagnosis and Macintosh substitution for chronic anterior cruciate insufficiency.

Of 45 consecutive knees undergoing MacIntosh fascia lata anterior cruciate substitution, only five had accompanying collateral ligament laxity. Over a third of cases (16) had more than ten episodes of major instability causing falls to the ground pre-operatively. The initial injury caused haemarthrosis, usually without external trauma (36), was often accompanied by a sensation of knee disruption (18), and was always associated with immediate disability. The results of 26 lateral substitution operations with minimum 12 month follow-up show abolition of pivot shift jerk in all patients. Incomple

A naturalistic assessment of partial-hospital treatment.

Three hundred fifty-eight consecutive admissions to 13 partial hospitals were evaluated over an 8-month period. The design of the study focused on an unobstrusive measurement process involving clinical judgment of outcome. Analyses revealed that the clinical judgment process generated data which were both reliable and valid. The results of the study indicate that partial-hospital treatment is most effective for chronic and acute schizophrenic conditions, paranoid schizophrenia, and affective disorders and least so far patients diagnosed as adjustment disorders. Patients in the organic and neur

Evaluating Percutaneous Coronary Intervention Safety, Quality, and Appropriateness Across Michigan Using Blinded Cross-Institutional Peer Review.

BACKGROUND: Several quality improvement initiatives have focused on the quality gap in percutaneous coronary intervention (PCI), yet significant variations in quality persist. Our objective was to use a novel blinded peer review system to evaluate PCI quality, safety, and appropriateness across Michigan. METHODS: Single-vessel PCI cases were randomly selected from the Blue Cross Blue Shield of Michigan Cardiovascular Consortium registry across Michigan (2018-2020), and anonymized angiograms and pertinent case records were uploaded to a secure server. Cases were reviewed by blinded interventio

Reflect before you act: providing structure to the evaluation of rehabilitation programmes.

PURPOSE: This paper is concerned with understanding and evaluating potentially diverse rehabilitation programmes. It helps evaluators and programme managers to focus attention on specific aspects of the rehabilitation process and select evaluation questions relevant to each. METHOD: Distinction is made between the rehabilitation programme itself, the programme environment and the relationships between the two. For each of these areas, evaluation questions have been formulated. For services offered to individual clients, questions address whether the status of clients has improved, what interv

Effect of performance improvement programs on compliance with sepsis bundles and mortality: a systematic review and meta-analysis of observational studies.

BACKGROUND: Several reports suggest that implementation of the Surviving Sepsis Campaign (SSC) guidelines is associated with mortality reduction in sepsis. However, adherence to the guideline-based resuscitation and management sepsis bundles is still poor. OBJECTIVE: To perform a systematic review of studies evaluating the impact of performance improvement programs on compliance with Surviving Sepsis Campaign (SSC) guideline-based bundles and/or mortality. A consensus process on the use of exercises and after action reports to assess and improve public health emergency preparedness and response.

INTRODUCTION: The objective of disaster preparedness is to ensure that appropriate systems, procedures, and resources are in place to provide prompt, effective assistance to disaster victims, thus facilitating relief measures and rehabilitation of services. Disaster preparedness efforts include the identification of possible health scenarios based on the probability of hazards and vulnerability of the population as a basis for creating a disaster plan. Exercises that simulate emergency response, involving the health and other sectors, have been suggested as useful tools to test the plans on a

Expanding developmental and behavioral services for newborns in primary care: implications of the findings.

BACKGROUND: In two other papers in this issue, the rationale, development, implementation, experimental design, approach to evaluation, and early results of a program to deliver developmental and behavioral services to all infants in primary care practice were described. Positive effects were seen for parental satisfaction, including decreased disenrollment, provider satisfaction, parenting practices, and health outcomes. METHODS: In the present article, the results are reviewed and implications of our findings for the delivery of care, families, healthcare systems, and further research are d

Effects of perceived working alliance quality and congruence on naturalistic psychotherapy outcome: A response surface analysis.

Objective. Patient- and therapist-reported (i.e., dyadic) working alliance evaluations have been identified as robust predictors of psychotherapy outcome. However, it remains unclear how their congruence relates to outcome and how treatment duration, a variable factor in naturalistic psychotherapy, influences this relationship. This study examined the effects of dyadic alliance judgments and their congruence on multidimensional outcomes, further assessing the role of treatment duration. Method. N = 353 outpatients undergoing cognitive behavioral therapy and their N = 95 therapists provided all

Quality assurance in primary care: a strategy based on outcome assessment.

We describe an outcome-based approach to quality assurance in primary care and present data from an initial study made to explore its usefulness. A questionnaire, which asked patients to report on the status of their problem in terms of the amount of symptoms, activity limitation, and anxiety it caused, was mailed to adults who had been seen a month previously for upper respiratory tract infection, sore throat, or urinary tract infection. Outcome standards developed for these conditions indicated that patients should report no symptoms, activity limitation, or anxiety. Of the 127 patients who

Do patients internalize the positive regard they are offered? A dyadic test of a Rogerian condition.

OBJECTIVE: Positive regard (PR) reflects a therapist's unconditional prizing of their patient, which meta-analytically correlates positively with patient improvement. However, most research has been limited to single-participant ratings of PR at a specific time, which neglects the dyadic and dynamic nature of PR (i.e., fundamental to benefitting from therapist-offered PR is that a patient internalizes it). Testing this premise, we hypothesized that therapist-offered PR at one session would predict patient-felt PR at a subsequent session (two sessions later), which would in turn predict the pat

Public service announcements: their effect on smoking.

The reasons for choosing the two-stage hierarchy of effects model in Table 5 and the process model of commitment in Table 6 to test are twofold. First, the two-stage hierarchy of effects model allows for reconsideration of the decision once it is made. Secondly, the process model of commitment seemed to be most appropriate in this context, since few commitment models have been specifically applied to marketing and the process model suggested offered the most complete description of commitment of those offered. Other models, such as the EKB model, would be more difficult to operationalize and w

Avoidable death as a measure of quality.

Age standardized mortality rates (SMR) vary between 43 and 162 per 100,000 inhabitants from cervical cancer in different areas in England, between 19 and 250 from tuberculosis, between 31 and 249 for asthma, between 0 and 263 from rheumatic heart disease, from 0 to 379 for acute respiratory disease, from 18 to 279 for abdominal hernias, and from 0 to 228 for appendicitis. Essentially similar differences were found for some of these diseases between five EEC countries. Many differences persist when correlated against social variables, and high quality health services should respond to possible
