The purpose of this paper is to present a surgical method for microtia(small auricle, small external audiotory meatus and small tympanic membrane). 7 cases received fenestration of semicircular canal. During the operation it was found that the patients had congenital absence of oval windows and stapes, significant low position of horizontal facial canal covering the oval windows. It was impossible to perform labyrinthotomy in these patients. The only choice of operation is fenestration of semicircular canal. 7 ears underwent the operation as described. 6 weeks-21 months follow-up showed that a

Three patients with a middle ear malformation characterized by the absence of the oval window and complicated by facial nerve anomalies were treated by surgery. Hearing ability of all the patients was improved. A variety of facial nerve anomalies were observed. Fenestration of the vestibule above the facial nerve was performed in the case of 2 of the 3 patients. In the other patient, fenestration of the scala vestibuli below the facial nerve was performed. In the case of all the 3 patients, hearing ability improved after the surgery and remained stable for a long time. Surgery for this disease

BACKGROUND: The aim of this study was to analyze malformed petrous bones with computed tomography and to develop a radiologic score which can help to judge the indication for operative reconstructions.
METHODS: One hundred forty-two petrous bones in 71 patients with unilateral or bilateral microtia, atresia of the external auditory canal, and malformations of the middle ear were evaluated with high-resolution CT.
RESULTS: In 97% of patients with severe auricular dysplasia, there was dysplasia of the middle ear ossicles; in 70% the stapes was malformed. In 32% the oval window was occluded, an

OBJECTIVES: Obliteration of the round window (RW) in cases of otosclerosis presents a significant clinical challenge due to its association with more severe hearing loss and a poorer prognosis for functional recovery after stapes surgery. The objective is to assess and characterize the occurrence of RW involvement in otosclerosis cases and to identify patterns of disease progression that may indicate a potential for RW obliteration.
METHODS: We selected archival temporal bones from donors with otosclerosis. We evaluated the degree of RW obliteration using a semi-quantitative scale and the loc

A 7-year-old boy with a history of purulent meningitis and watery rhinorrhea was studied using computed tomography (CT) and magnetic resonance imaging (MRI). He had a common cavity in the left inner ear. With high-resolution heavily T2-weighted MRI, leakage of the inner ear fluid into the middle ear at the oval window area through a congenital perilymphatic fistula could be visualized. Surgery to close the fistula showed a perforation in the stapes footplate.

INTRODUCTION: Active middle ear implants (aMEI) are being increasingly used for hearing restoration in congenital aural atresia. The existing gradings used for CT findings do not meet the requirements for these implants. Some items are expendable, whereas other important imaging factors are missing. We aimed to create a new grading system that could describe the extent of the malformation and predict the viability and challenges of implanting an aMEI.
METHODS: One hundred three malformed ears were evaluated using HRCT of the temporal bone. The qualitative items middle ear and mastoid pneumati

In this study, we aimed to compare the outcomes of satisfaction of the patients who used hearing aids preceding the vibrant sound bridge (VSB) application on middle ear windows (14 oval window and 5 round window). Nineteen adult patients with conductive or mixed hearing loss were included in the study. All patients used behind the ear hearing aids on the site which was selected for VSB application. The patients used hearing aids for at least 3 months before the VSB operation. The floating mass transducer (FMT) was placed on one of the middle ear windows (oval or round) in VSB operation. The pa

Transcanal endoscopic ear surgery (TEES) will become a very useful therapeutic option. A perilymphatic fistula (PLF) is defined as sudden sensorineural hearing loss and/or vertigo caused by leakage of the perilymph through a fistula from the oval window and/or round window. We report a case of PLF after electric acoustic stimulation (EAS), a kind of cochlear implant, successfully treated by TEES. A 38-year-old man presented to our hospital with vertigo and hearing loss (HL). His vertigo was induced by Valsalva maneuvers. Eight months ago, he underwent EAS for his right ear for congenital senso

BACKGROUND: Silver nanoparticles (Ag NPs) displayed strong activities in anti-bacterial, anti-viral, and anti-fungal studies and were reportedly efficient in treating otitis media. Information on distribution of AgNPs in different compartments of the ear is lacking.
OBJECTIVE: To detect distribution of Ag NPs in the middle and inner ear and transportation pathways after transtympanic injection.
METHODS: Contrast effect of Ag NPs in the micro CT imaging was assessed in a phantom. AgNPs at various concentrations (1.85 mM, 37.1 mM, and 370.7 mM) were administered to rat middle ear using transty

11 children (13 ears) have been operated upon for otosclerosis. In ten cases the stapes footplate was found totally fixed, with histological verification of the diagnosis of otosclerosis. Conservative surgery of the oval window was possible in only four cases. All other patients required total removal of staples or footplate. Findings indicate that clinical otosclerosis occurring in childhood results in an otosclerotic process in the oval window region with total fixation of the footplate. Thus, correction usually requires removal of at least the entire footplate.

The paper first reviews our present understanding of the functional morphology of the odontocete (toothed whale) ear. The tympano-periotic complex forming the ear region consists of a ventral bowl-shaped tympanic bone in direct contact with the surrounding soft tissues and the incident sound, and a dorsal periotic bone containing the inner ear. Apparently sound brings the tympanic bone, and especially its thin tympanic plate, into vibration. The ossicles in the air-filled middle ear cavity form a bridge from the tympanic plate to the periotic bone connecting the vibrating plate to the oval win

Congenital anomalies of the middle ear have been considered rare. The tremendous volume of middle ear surgery performed during the last 15 years has brought attention to numerous anomalies of the facial nerve, ossicles, and middle ear cleft, suggesting that middle ear anomalies are not as rare as once believed. This paper discusses anomalies of the descending and horizontal facial nerve which have been previously reported. The literature of anomalies of the ossicles is reviewed. A very rare anomaly of the carotid artery presenting in the middle ear is described and its surgical treatment outli

A 35-year-old man had a 5-year history of fluctuating hearing loss in his only hearing ear. History and diagnostic tests indicated a perilymph fistula, a diagnosis subsequently confirmed by exploration. Primary and secondary repairs temporarily ameliorated symptoms. A spinal fluid to middle ear fluid pathway was identified by radioactive tracer. A patent cochlear aqueduct indicated on computed tomography scan was found and repaired through a posterior cranial fossa approach. Hearing was preserved, remaining relatively stable during the 2-year follow-up period.

HYPOTHESIS: Driving the oval window directly with an active middle ear implant (AMEI) can produce high levels of input to the inner ear.
BACKGROUND: Treatment of otosclerosis bypasses the stapes with a piston that penetrates the vestibule. Although this treats the conductive component of hearing loss, it does not treat the sensorineural part, which can be improved using an additional conventional hearing aid. Active middle ear implants have been proposed to be an alternative in treating otosclerosis in cases of mixed hearing losses.
METHODS: Seven temporal bones were prepared to expose the s
