Abnormalities of the retinal blood supply have been widely implicated in primary open-angle glaucoma (POAG). Impaired blood supply to the retina and optic nerve head (ONH) may be a primary pathophysiologic mechanism contributing to POAG ('vascular hypothesis'). However, the decreased metabolic activity of atrophic tissue is itself known to induce both vascular changes and decreased blood flow due to reduced oxygen demand. Cerebral autoregulation is crucial for maintaining cerebral blood flow and perfusion. In recent years, the importance of cerebral oxygenation in syncope and orthostatic intolerance (OI) has received increased attention. Cerebral tissue oxygenation can be measured by using near-infrared spectroscopy (NIRS), which determines the ratio of oxygenated hemoglobin to total hemoglobin in cerebral tissue. OBJECTIVE: The reliability of pulse oximetry in the lower SaO2-ranges has not been fully investigated. We wanted to investigate pulse oximeter performance in a pig model of perinatal asphyxia.
METHODS: Asphyxia was induced in 22 newborn pigs. Pulse oximetry-values for SpO2, as well as SaO2 from blood gas analyses were recorded multiple times before, during and after asphyxiation. The relationship between SpO2 and SaO2 in normoxia and hypoxia in this model was quantified. Calculations were made for 5% increments of SaO2 increasing successively from ≥95% to ≥20%.

This review will highlight a new technology and recent insights into measuring retinal oxygen saturation in several ophthalmic diseases. A growing body of evidence suggests that disturbances in retinal blood flow and oxygenation are related to several retinopathies and glaucoma, which can severely impair vision. Pulmonary thromboendarterectomy is an uncommon procedure and should be performed with circulatory arrest. One of the major concerns is the postoperative central neurological injuries. Hemoglobinopathies are an uncommon cause of cyanosis and low oxygen saturation on pulse oximetry. However, when they do occur, they can present a complex clinical scenario for the emergency physician. We report the index case of a previously undescribed hemoglobinopathy that presented to the pediatric emergency department. The evaluation and management of the cyanotic/hypoxic child and review of hemoglobinopathies are presented here.

We report a novel use of cerebral oximetry in cardiac surgery using the Fore-Sight absolute cerebral oximeter (CAS Medical Systems Inc, Branford, CT). A patient with a persistent left superior vena cava underwent mitral and aortic valve replacement. We decided to tape and occlude the persistent left superior vena cava and used cerebral oximetry to compare left and right hemispheric oxygen saturation levels to ensure that cerebral perfusion was not impaired. The procedure was uneventful, and the patient was extubated 8 hours later without neurocognitive deficit.

This study examined the potential clinical contribution of noninvasive brain and skeletal muscle oximetry as a diagnostic aid in neurally mediated syncope. Tilt table testing was performed in 15 patients with a history of syncope and 9 healthy volunteers. Spatially resolved reflectance near-infrared spectroscopy was used to examine regional oxyhemoglobin saturation in the frontal cerebral cortex and pectoral muscle. Tissue oxygen tension was measured using a modified Clark oxygen electrode in the gastrointestinal tract of 33 patients undergoing laparatomy. The electrode, which incorporated a thermistor, had a linear response to oxygen tension (r = 0.995, p less than 0.001). OBJECTIVE: We describe a case of persisting readings from a fingertip pulse oximeter (FPO) in an elderly cachectic woman with pancreatic cancer, who fulfilled all classical criteria of death.

PURPOSE: We describe pharyngeal oximetry with the laryngeal mask airway in two patients with low perfusion states where finger oximetry failed.
CLINICAL FEATURES: One patient was a 47-yr-old man with septic shock and the other a 64-yr-old man with multiorgan failure. In both patients, a # 4 laryngeal mask airway with a pediatric pulse oximeter probe was inserted behind the tracheal tube. A good waveform was obtained and oxygen saturation was 0-2% lower than arterial samples.
