Influence of pain anticipation on brain activity and pain perception in Gulf War Veterans with chronic musculoskeletal pain.

Anticipation of a painful experience can influence brain activity and increase sensitivity to experimental somatosensory stimuli in healthy adults, but this response is poorly understood among individuals with chronic musculoskeletal pain (CMP). Studies of brain and perceptual responses to somatosensory stimuli are used to make inferences about central nervous system dysfunction as a potential mechanism of symptoms. As such, we sought to (a) determine the influence of pain anticipation on pain-relevant brain regions and pain perception, and (b) characterize potential differences in these respo

Distinct effects of fibromyalgia pain, depression and anxiety on perception of affective touch and pain.

Gentle stroking that activates C-tactile afferents is typically pleasant, calming and pain relieving. Deep pressure touch frequently elicits similar affective responses. However touch is often less pleasant for individuals with chronic pain. It is important to understand the reasons for this clinical difference because of the close relationship of pain with reduced social connection, and the widespread use of touch-based therapies for pain relief. It is not known whether differences in touch perception in chronic pain relate to pain-specific processes or to comorbid affective symptoms. The cur

Chronic Low Back Pain: Perception and Coping With Pain in the Presence of Psychiatric Comorbidity.

This retrospective study investigated the influence of psychiatric comorbidity on pain perception and coping with pain in tertiary pain clinic patients, 427 treated for chronic low back pain (CLBP) and 629 for other forms of chronic pain (CG). No differences in psychosomatic dimensions were found between the two groups, but Italian Pain Questionnaire dimensions and intensity scores (t = 7.35; p < 0.0001) were higher in CLBP than in CG subjects. According to the Mini-International Neuropsychiatric Interview, CLBP patients also had a higher prevalence of lifetime major depressive episodes (χ2 =

Classification of pain intensity with the pain beliefs and perceptions inventory (PBPI) and the pain catastrophizing scales (PCS).

PURPOSE: The pain beliefs and perceptions inventory (PBPI) and the pain catastrophizing scales (PCS) characterize beliefs or distress dimensions of the pain experience. It is relatively unknown, however, to what degree the PBPI and the PCS are well suited to classifying pain intensity. METHODS: This study applied a receiver operating characteristic (ROC) approach to these instruments against the criterion of a visual analogue scale (VAS) of pain intensity with fibromyalgia and chronic back pain patients (n = 419). RESULTS: The largest areas under the curve (AUC) were moderate and limited to

Peripartal pain perception and pain therapy: introduction and validation of a questionnaire as a quality instrument.

BACKGROUND: Labor pain is difficult to measure. The aim of this proof-of-concept study is to implement and test a questionnaire assessing pain sensation during and after vaginal deliveries. Its key aspect is a highly standardized survey of patient-reported outcome (PRO) by staff not involved in routine care. METHODS: Between January and November 2015 339 women were assessed 24-48 h after spontaneous or operative-vaginal delivery of a singleton. German language skills were a prerequisite to participate. The test-retest reliability was calculated in 38 women 24-36 and 48-72 h postpartum between

Testosterone administration enhances the expectation and perception of painful and non-painful somatosensory stimuli.

The influence of testosterone on pain perception remains inconsistent in the literature. This randomized, placebo-controlled, double-blind, crossover study investigated the effect of testosterone administration on perception and expectation of electrocutaneous stimulus. Thirty healthy male participants received a single dose of testosterone in one session and a placebo in the other session. For each session, they completed a pain-rating task in which a predictability cue was inserted before a painful or non-painful electocutaneous stimulus delivery, while neural activity was simultaneously rec

Neuronal and non-neuronal TRPA1 mediate distinct pain perception mechanisms in Familial Episodic Pain Syndrome models.

Familial Episodic Pain Syndrome (FEPS) is a rare inherited disorder characterized by recurrent episodes of severe upper-body pain triggered by stimuli such as cold, stress, or fasting. A gain-of-function mutation (N855S) in the Transient Receptor Potential Ankyrin 1 (TRPA1) ion channel has been identified in affected individuals, yet the cellular mechanisms driving FEPS pain remain poorly defined. Although TRPA1 has been primarily studied in sensory neurons, emerging evidence suggests that non-neuronal cells may contribute to pain signaling. To define the cell-specific role of mutant TRPA1 (TR

Visual feedback manipulation in virtual reality alters movement-evoked pain perception in chronic low back pain.

Movement-evoked pain in chronic low back pain (LBP) can be influenced by visual cues linked to threatening movements. This study explored whether manipulating visual proprioceptive feedback via virtual reality (VR) alters movement-evoked pain and if individuals with higher pain, kinesiophobia, disability, or catastrophising are more susceptible to these manipulations. Cross-sectional study with 50 patients with non-specific chronic LBP. Participants performed lumbar spine extension until pain onset with and without VR. VR feedback was manipulated to show 10% less (E -) or 10% more (E +) moveme

Impaired insula functional connectivity associated with persistent pain perception in patients with complex regional pain syndrome.

Given that the insula plays a contributory role in the perception of chronic pain, we examined the resting-state functional connectivity between the insular cortex and other brain regions to investigate neural underpinnings of persisting perception of background pain in patients with complex regional pain syndrome (CRPS). A total of 25 patients with CRPS and 25 matched healthy controls underwent functional magnetic resonance imaging at rest. With the anterior and posterior insular cortices as seed regions, we compared the strength of the resting-state functional connectivity between the two gr

Perceiving pain alters body perception: the effects of acute pain on body image and sensory testing.

This study investigated the effects of experimentally induced acute pain on body image and sensory testing. A total of 91 pain-free participants were randomly assigned to 1 of 3 groups: injection (pain), sham injection, or control. Measures were taken 3 times in each group: at baseline, during the testing phase, and once the pain had subsided. The study included both sensory testing and body image measures. Participants in the injection group showed significant increases in body image distortion during the testing phase ( P < 0.001, η 2 = 0.19), alongside larger area of pain distribution ( P <

Make me more comfortable: effects of a hypnosis session on pain perception in chronic pain patients.

INTRODUCTION: Approximately one-quarter of Canadians experience chronic pain, a debilitating condition often necessitating opioid use, which raises concerns regarding dependency and overdose risks. As an alternative, we developed the HYlaDO program (Hypnose de la Douleur, hypnosis of Pain in French), a novel self-hypnosis approach for chronic pain management. The development of this program followed the ORBIT model, a comprehensive framework for designing interventions encompassing several phases ranging from design to efficacy assessment. METHODS: In the present work, we conducted a prelimin

Pain perception in chronic knee osteoarthritis with varying levels of pain inhibitory control: an exploratory study.

Background and aims Pain is a disabling symptom in knee osteoarthritis (KOA) and its underlying mechanism remains poorly understood. Dysfunction of descending pain modulatory pathways and reduced pain inhibition enhance pain facilitation in many chronic pain syndromes but do not fully explain pain levels in chronic musculoskeletal conditions. The objective of this study is to explore the association of clinical variables with pain intensity perception in KOA individuals with varying levels of Conditioned Pain Modulation (CPM) response. Methods This is a cross-sectional, exploratory analysis us
