The role of the tensor veli palatini muscle in the development of cleft palate-associated middle ear problems.

OBJECTIVE: Otitis media with effusion is common in infants with an unrepaired cleft palate. Although its prevalence is reduced after cleft surgery, many children continue to suffer from middle ear problems during childhood. While the tensor veli palatini muscle is thought to be involved in middle ear ventilation, evidence about its exact anatomy, function, and role in cleft palate surgery is limited. This study aimed to perform a thorough review of the literature on (1) the role of the tensor veli palatini muscle in the Eustachian tube opening and middle ear ventilation, (2) anatomical anomali

Free segmental vastus lateralis muscle flap for reconstruction of recalcitrant defects of the cleft hard palate.

INTRODUCTION: Large recalcitrant defects of the anterior palate due to clefting are difficult to close with local tissues. In some cases distant tissue transfer may be the only option. Free segmental vastus lateralis muscle with its long high-caliber pedicle and low donor-site morbidity may be a good option. PATIENT/METHOD: An 8-year-old girl with bilateral cleft lip and palate was evaluated for a defect in the anterior hard palate. She had four failed palate closures resulting in a 3.2 × 2.8 cm defect with severely scarred surrounding palatal tissues and severely hypernasal speech. A vastus

Motor behavior of masticatory muscles in individuals with unilateral trans-incisive foramen cleft lip and palate.

OBJECTIVE: To investigate whether children with complete unilateral cleft lip and palate (CLP) have altered control of masticatory muscles. METHODS: The muscular activity of 66 children with CLP (n = 33) and without CLP (n = 33), aged 6-12 years was assessed during different tasks (rest, isometry, mastication) using electromyography. RESULTS: The activation for the analyzed muscles was higher in children with CLP at rest (R) and inactive period (IP) (p < 0.05). The muscles of children with CLP remained active for longer than those without CLP (p < 0.05), and the length of the masticatory cyc

Denervation-Associated Change in the Palatinus and Levator Veli Palatini Muscles of Dogs with Elongated Soft Palate.

Muscle lesions and decreased numbers of peripheral nerve branches have been reported in the soft palates of dogs presenting with brachycephalic airway obstruction syndrome (BAOS). Myosin adenosine triphosphatase staining was employed to investigate whether muscle lesions in the elongated soft palate (ESP) of dogs with BAOS reflect the presence of denervation. Soft palates were collected from nine brachycephalic dogs during surgical intervention for BAOS and from five healthy beagle dogs as controls. In the control soft palates, myofibres with relatively uniform diameters and a random mosaic pa

The temporalis muscle flap for reconstruction of soft palate and lateral oropharyngeal wall after transoral robotic surgery.

Trans Oral robotic surgery (TORS) is a prominent surgical approach for the resection of oropharyngeal tumors without division of the lip and mandible. The current practice following TORS is to allow the defect to heal by secondary intention, but some defects following TORS are large and complex enough to benefit soft-tissue coverage. In the free flap era, regional flaps are often overlooked albeit they still represent a valid alternative. In terms of cost-effectiveness, the use of alternative pedicled flaps in TORS framework probably reduced the risks of postoperative complications, with conse

Submucous cleft palate: outcomes after primary repair with repositioning of the levator muscle in 51 consecutive patients.

I report the incidence of hypernasal resonance, nasal emission, and fistula after intravelar surgery with retropositioning of the levator muscle by a single surgeon in a consecutive series of 51 patients who presented with symptomatic submucous cleft palate. Intravelar veloplasty with repositioning of the levator muscle was highly effective in that 37/51 patients (73%) achieved either normal or mild and inconsistent resonance (p<0.0001), and 39 (77%) normal or mild and inconsistent nasal emissions (p<0.0001). The fistula rate was 6% (n=3). Both the clinical grade of submucous cleft palate and

The design of non-occlusal intraoral appliances on hard palate and their effect on masseter muscle activity during sleep.

This study aimed to reveal whether masseter muscle activity during sleep is affected by the difference in design of non-occlusal intraoral appliances on hard palate. Eight healthy Japanese participants were selected and wore each of the four types of appliances (horse shoe, thin, thick and medium thick) during sleep for one week with a one week interval without appliance during sleep. A masseter muscle electromyograph (EMG) was recorded during sleep. The EMG activities were analyzed by calculating the number of bursts per hour, episodes per hour, and bursts per episode. The EMG parameters with

Abnormal mitochondria organization and oxidative activity in the palate muscles of long-term snorers with obstructive sleep apnea.

BACKGROUND: Histopathological alterations and a reduced number of capillaries have been observed in the palate muscles of snorers with obstructive sleep apnea syndrome (OSAS). These changes may create a substrate for decreased microcirculation, impaired aerobic metabolism and muscle dysfunction and contribute to upper airway obstruction during sleep. OBJECTIVES: The aim was to analyze mitochondria distribution and oxidative enzyme activity in relation to capillary supply in the palate muscles of patients with a history of long-term snoring and OSAS. METHODS: Palatopharyngeus (PP) and uvula (

A ringed fascia lata graft without peritendinous areolar tissue encircling the levator veli palatini and superior pharyngeal constrictor muscles gradually shrinks to reduce velopharyngeal incompetence, functioning as an intravelar palatal lift.

INTRODUCTION: We have previously reported that fascia lata grafts with peritendinous areolar tissue used to treat severe congenital blepharoptosis gradually shrink within 6 weeks postoperatively and maintain long-term shrinkage of 15.5% on average. Accordingly, it seemed possible that a fascia lata graft without peritendinous areolar tissue would shrink more than the one with peritendinous areolar tissue in a clinical setting. We evaluated this possibility in a patient with Klippel-Feil syndrome having postoperative deep atonic nasopharynx. METHODS: In combination with intravelar veloplasty a
