Predicting the efficacy of surgery for pain relief in patients with alcoholic chronic pancreatitis.

BACKGROUND: Recurrent pain is the most disabling complication in patients with chronic pancreatitis. Pancreatic surgery is currently considered as last-resort therapeutic option. The aims of this study were to assess pancreatic surgery performance for chronic pain in patients with alcoholic chronic pancreatitis and to determine factors predictive of therapeutic response. METHODS: All patients with chronic pancreatitis who underwent pancreatic surgery for chronic pain were included and divided into 2 groups according to the cause of chronic pancreatitis: alcoholic and any other chronic pancrea

Alcoholic severe acute pancreatitis with positive culture of pancreatic juice treated by nasopancreatic drainage.

BACKGROUND: Severe acute pancreatitis (SAP) is a serious disease associated with alcoholism and has a high mortality rate. Effective treatments have not been established. METHODS: A 58-year-old man was admitted due to alcoholic SAP. Endoscopic retrograde cholangiopancreatography revealed pancreatic calculi at the pancreas head and a stricture in the pancreatic duct from the pancreas head to the body. Endoscopically, nasopancreatic drainage (NPD) was placed through the minor papilla to the pancreas tail beyond the stricture. RESULTS: Pancreatic juice culture was positive for Streptococcus and

Low plasma antibodies specific for phosphatidylethanol in alcohol abusers and patients with alcoholic pancreatitis.

Phosphatidylethanol (PEth) is a group of alcohol-modified phospholipids present in cell membranes after heavy drinking. Our aim was to demonstrate the presence of human plasma antibodies binding to PEth and to address their specificity and value in detecting subjects engaged in heavy alcohol consumption. Antibodies to PEth were analyzed in plasma from heavy drinkers (n=20), patients with alcoholic pancreatitis (n=58) and control subjects (n=24), using chemiluminescent immunoassay. Heavy drinkers and patients with alcoholic pancreatitis demonstrated significantly lower levels of plasma IgG, IgA

CD14 promoter polymorphism in Chinese alcoholic patients with cirrhosis of liver and acute pancreatitis.

AIM: To investigate the relationship between genetic polymorphism of the CD14 promoter and the occurrence of alcoholic cirrhosis and alcoholic pancreatitis, and to challenge the conclusion made earlier that the patients with acute alcoholic pancreatitis and patients with alcoholic cirrhosis of liver are two different subpopulations. METHODS: Using the polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method, we determined the polymorphism of CD14 gene and aldehyde dehydrogenase gene 2 (ALDH 2) in 335 alcoholic patients with different organ complications i.e., cirrh

Diabetes in chronic alcoholic pancreatitis. Role of residual beta cell function and insulin resistance.

Chronic alcoholic pancreatitis (CAP) is often complicated by the onset of diabetes mellitus. The aim of this study was to assess the residual beta cell function (evaluated by means of the glucagon test) and the mean disposal rate of insulin (with the insulin tolerance test) in 66 CAP patients with or without abnormalities of glucose metabolism and in 19 control subjects. On the basis of our data, we conclude that the glucose metabolism abnormalities in chronic pancreatitis occurs as a result not merely of impaired production of endogenous insulin, but also as result of a combination of the lat

Pancreatitis-Related Encapsulated Fluid Collections Associated With a Pancreatic Duct Stent Retained for 13 Years in Chronic Alcoholic Pancreatitis: A Case Report.

Pancreatic duct stenting is an established treatment for ductal decompression in obstructive chronic pancreatitis. However, very long-term stent retention may be associated with late complications, particularly in patients with advanced chronic pancreatitis. A 65-year-old man with chronic alcoholic pancreatitis underwent pancreatic duct stenting for a main pancreatic duct stricture, but follow-up was discontinued, and the stent remained in situ for approximately 13 years. He presented with fever and loss of appetite. Computed tomography showed diffuse pancreatic calcifications and pancreatitis

Different changes in high-energy phosphates in alcoholic acute pancreatitis and taurocholate acute pancreatitis in rats using NMR spectroscopy at 2.0 T.

We studied the pancreatic high-energy phosphates in two models of acute pancreatitis using 31P nuclear magnetic resonance (NMR) in rats for the first time in vivo. Alcoholic pancreatitis was induced by acute ethanol intoxication and an obstruction-hyperstimulation mechanism. Taurocholate pancreatitis was generated by intraparenchymal administration of 1 ml of 1-10% taurocholate-Na+. In addition to the obligate control groups, a simple ischemia experiment was performed. The high-energy phosphates were monitored by 31P NMR spectroscopy at 2.0 T. Additionally, by means of a scoring system, the qu

The Role of Alcohol in Pancreatic Diseases: A Comprehensive Perspective.

Alcohol abuse is a leading cause of global health care concerns and can cause health loss, disease, and death. In addition to the liver, the pancreas is a vital organ for the metabolism of alcohol, especially nonoxidative metabolism, and alcohol abuse is the leading cause of pancreatic diseases worldwide. Epidemiologic investigations have shown that alcohol consumption is linked to a spectrum of multiple pancreatic disorders, including acute pancreatitis, chronic pancreatitis, pancreatic cancer, and diabetes. Numerous in vitro and in vivo studies have elucidated the key mechanisms related to a

[Can the recurrence of alcohol-induced pancreatitis be prevented?].

70% of pancreatitis cases are considered to be induced by alcohol. Half of those fallen ill with alcohol-induced acute pancreatitis will have relapses. A prospective follow-up study showed that the level of dependence on alcohol constitutes the most important risk factor. Continued drinking was shown to be a dose-responsive risk factor for relapse; abstinence provided for a complete protection against renewed pancreatitis. In a randomized study, a semi-annual meeting with a healthcare professional specialized in substance abuse problems significantly reduced new episodes of acute pancreatitis.

[Endogenous toxemia in pancreatic necrosis of alcoholic etiology].

Operation for pancreonecrosis was carried out in 197 patients, in 52 (26.4%) of them the disease was of alcoholic etiology. The disease was marked by severe clinical manifestations with the development of respiratory, hemodynamic, and peritoneal syndromes and their complications, and by psychic disorders. The endo-intoxication structure was studied in 13 patients by mass-spectrometry proceeding from alcohol metabolism in the organism. Eleven intermediate products were identified: ethanol, acetaldehyde, normal butyraldehyde and isovaleric aldehyde, diethylamine, acetone, isopropanol, methyl iso

Recurrent Acute-on-Chronic Pancreatitis in a Chronic Alcoholic With Pancreatic Divisum: A Complex Case.

Pancreatic divisum, a rare developmental anomaly of the pancreas, is infrequently observed in young individuals and is a recognized cause of recurrent acute pancreatitis, ultimately progressing to chronic pancreatitis. Chronic alcoholism is a prevalent and significant etiological factor contributing to both recurrent acute pancreatitis and chronic pancreatitis. This case report details the presentation of a 28-year-old patient with a history of chronic alcoholism, exhibiting recurrent acute pancreatitis evolving into chronic pancreatitis. Diagnostic evaluation revealed the presence of pancreat

Battle-scarred pancreas: role of alcohol and pancreatic stellate cells in pancreatic fibrosis.

Pancreatic stellate cells (PSC) are now recognized as the key mediators of pancreatic fibrosis, a characteristic feature of chronic pancreatitis. The role of PSC in alcoholic pancreatic fibrosis has been examined in vivo (using pancreatic tissue from patients with alcohol-induced chronic pancreatitis and from animal models of experimental pancreatitis) and in vitro (using PSC in culture). These studies indicate that PSC are activated early in the course of pancreatic injury and are the predominant source of collagen in the fibrotic pancreas. The factors responsible for mediating PSC activation

Pancreatic fibrosis in patients with alcoholic dependence syndrome.

Pancreatic fibrosis in patients with alcoholic dependence syndrome was studied histopathologically. In 30 of 41 autopsied patients with alcoholic dependence syndrome, fibrosis was observed despite the absence of clinical pancreatitis. The fibrosis was categorized into three types, according to Martin's classification: intralobular sclerosis in 15 cases, perilobular sclerosis in seven cases, and a mixed intralobular and perilobular sclerosis in the remaining eight cases. The type of the fibrosis was not related to the duration of alcohol abuse. Alcoholic liver cirrhosis was coexistent in 23 of

[Changes in pancreatic secretion in alcoholic liver disease].

By means of consecutive pancreatic stimulation, we have investigated the presence of changes of pancreatic function in alcoholic patients, with and without alcoholic liver disease, in order to detect functional alterations and possible association of hepatic and pancreatic disease. The patients were 49 chronic alcoholics (8 patients without liver disease, 11 hepatic steatosis, 9 alcoholic hepatitis and 21 alcoholic cirrhosis) and 15 non alcoholic subjects (8 normal controls and 7 cases of non alcoholic cirrhosis). In all the cases two consecutive stimulations were carried out: first with secre

Disproportionate steatorrhoea in alcohol-induced calcific pancreatitis.

The pancreatograms, as assessed by endoscopic retrograde cholangiopancreatography, of 51 patients with alcohol-induced calcific pancreatitis (AICP), were compared after division of the patients into three groups according to their insufficiency patterns. The first group, consisting of 18 patients, was labelled as having 'disproportionate steatorrhoea' characterized either by overt steatorrhoea and mild or no diabetes or by mild steatorrhoea and a normal glucose tolerance test result. The second group of 15 patients had severe diabetes associated with overt, mild or no steatorrhoea and constitu

Pancreatic Secretory Trypsin Inhibitor (SPINK1) Gene Mutation in Patients with Acute Alcohol Pancreatitis (AAP) Compared to Healthy Controls and Heavy Alcohol Users without Pancreatitis.

Only 3-5% of heavy alcohol users develop acute alcohol pancreatitis (AAP). This suggests that additional triggers are required to initiate the inflammatory process. Genetic susceptibility contributes to the development of AAP, and SPINK1 mutation is a documented risk factor. We investigated the prevalence of the SPINK1(N34S) mutation in patients with AAP compared to heavy alcohol users who had never suffered an episode of pancreatitis. Blood samples for the mutational analysis from patients with first episode (n = 60) and recurrent AAP (n = 43) and from heavy alcohol users without a history of
