A 58-year-old female who underwent an aortic valve replacement 3 years ago was admitted to our hospital. She developed dyspnea in a month. Urgent reoperation was scheduled because an echocardiography demonstrated severe aortic valve regurgitation, and because a cinefluoroscopy showed that a leaflet of the prosthetic valve was restricted. Pannus formation from left ventricular outflow tract caused prosthetic valve insufficiency. An ATS 18 aortic prosthesis was replaced after removing old sewing cuff and surrounding tissue of the aortic annulus as much as possible. However, the coronary orifices

OBJECTIVE: Mechanical valvular prostheses have the advantage of longevity but carry a risk of thrombosis which is dependant on valve design, materials and host-related interface. While pannus is common to both biologic and mechanical valves, acute prosthetic thrombosis is mostly a complication of mechanical valves; therefore we investigated to find rates and risk of these obstructive complications.
METHODS: Between 1/1/70 and 31/12/97, 2680 patients received at least one mechanical prosthesis in the aortic or mitral or tricuspid position and a total of 3014 operations were performed. Follow-u

Guinea-pigs were repeatedly infected with guinea-pig inclusion conjunctivitis agent. Reinfection caused severe conjunctival inflammation, and repeated reinfection led to chronic inflammation lasting for many months. This was followed by the development of pannus, follicles on the palpebral conjunctivae, scarring of the lower palpebral conjunctiva, and deformities of the lower lid. Reinfection was accompanied by small numbers of inclusion-bearing cells, small numbers of polymorphonuclear cells, and high numbers of mononuclear cells. There was no increase in the level of serum antibodies. The ch

BACKGROUND: Early bioprosthetic valve dysfunction (BVD) due to pannus formation is uncommon in elderly patients, and only a limited number of cases have been reported.
CASE SUMMARY: An 84-year-old man presented with exertional dyspnoea 3 years after surgical aortic valve replacement (SAVR) with a 19 mm Epic™ valve (Abbott, Santa Clara, CA, USA). Transthoracic echocardiography demonstrated progressive BVD, and cardiac computed tomography (CT) revealed sub-aortic pannus formation. Re-operative SAVR was performed using a 19 mm INSPIRIS RESILIA® valve (Edwards Lifesciences, Irvine, CA, USA), and

PURPOSE: To investigate the cardiac computed tomographic (CT) findings and clinical implications of subprosthetic pannus in patients who have undergone aortic valve replacement.
MATERIALS AND METHODS: The institutional review board approved this retrospective study, and the need to obtain written informed consent was waived. From April 2011 to March 2012, 88 patients (mean age, 63 years; 45 men) with a prosthetic aortic valve who underwent cardiac CT were retrospectively selected. Dynamic cardiac CT images were analyzed by using a multiplanar reformatted technique. The presence or absence of

Polyfibromatosis is a rare fibrosing condition characterized by fibromatosis in different body areas and by keloid formation, and which can be associated with arthropathy and osteolysis. Familial occurrence has been described, but the cause remains unknown. Here, we describe a patient with characteristics of polyfibromatosis with arthropathy who had in addition severe conjunctival fibrosis, distinctive face, gingival overgrowth, and pigmented keloids. We discuss the resemblances and differences with polyfibromatosis and descriptions of other, similar patients. We conclude that at present it re

The cartilage-pannus junction has been studied in multiple sections from 23 rheumatoid joints. Changes suggesting a metaplastic reaction of the articular cartilage, termed transitional fibroblastic zone, were commonly found in hips and knees, but were rarely present in metatarsophalangeal joints, in which an invasive pannus with cartilage degradation in close association with inflammatory cells was seen. Thus when multiple sections from rheumatoid joints were examined a transitional fibroblastic zone was found in 1/15 (7%) sections from metatarsophalangeal joints compared with 29/57 (51%) and

PURPOSE: Anti-tumor necrosis factor (TNF)-alpha inhibitors are commonly used in the treatment of inflammatory bowel disease (IBD). Despite a beneficial risk-profile, dermatologic and ocular complications may particularly develop at higher doses. Here, we report a case of bilateral corneal pannus in a patient with Crohn's disease, which occurred following a prolonged adalimumab overdose.
OBSERVATIONS: A 19-year-old male with Crohn's disease presented with progressive bilateral blurred vision, conjunctival injection, and light sensitivity. Clinical examination, including best-corrected visual a

BACKGROUND: Coronary obstruction from pannus after surgical aortic valve replacement (SAVR) is rare. We report a case of severe bioprosthetic aortic valve stenosis with concomitant ostial left main coronary artery (LMCA) obstruction, treated with SAVR alone.
CASE SUMMARY: A 73-year-old man with prior coronary artery bypass grafting and SAVR presented with acute heart failure. Multimodality imaging-echocardiography, angiography, and computed tomography-revealed severe bioprosthetic stenosis and critical LMCA obstruction from pannus. Redo SAVR included valve explantation, pannus excision at the

INTRODUCTION: Bioprosthetic valve failure after mitral valve replacement is a well-recognized phenomenon, with pannus overgrowth being one notable cause. The standard treatments include redo surgical mitral valve replacement and the less invasive transcatheter treatment, mitral valve-in-valve. However, the feasibility and safety of performing mitral valve-in-valve when pannus overgrowth has reduced the valvular opening to a mere pinhole remains uncertain.
CASE PRESENTATION: A 76-year-old woman, who had previously undergone mitral valve replacement, was admitted for congestive heart failure. S

Structural valve deterioration represents the most important concern regarding the implantation of a biologic heart valve in young patients. Several strategies have been proposed to improve prosthesis design in order to optimize performance and durability. This report describes an early and specific mode of valve degeneration of prostheses with outward-mounted pericardial leaflets resulting from pannus overgrowth on the aortic side. This led to leaflet retraction in two consecutive cases 4 and 6 years after the first implantation of a Trifecta valve (St. Jude Medical, St. Paul, MN).

A 78-year-old woman was admitted to our institution with progressive dyspnea. She had previously been diagnosed with rheumatic heart disease and had undergone cardiac surgery for mechanical mitral valve replacement ten years previously. Transesophageal echocardiography revealed blockage of the mechanical prosthesis and the patient was scheduled for surgery, in which a thrombus was removed from the left atrial appendage. A partial thrombosis of the mechanical prosthesis and circumferential pannus overgrowth were concomitantly detected. Prosthetic heart valve blockage is a rare but life-threaten

Ankylosing spondylitis is one of the commonest inflammatory diseases of the axial skeleton and can be complicated by atlanto-axial instability. This serious and likely underestimated complication can be easily overlooked. However, there are clear features which can help alert suspicion to initiate the appropriate investigations with imaging that is very effective at diagnosing and assessing this complication. The authors report an unusual case where odontoid pannus formation, akin to that seen in rheumatoid arthritis, was the underlying cause.

PURPOSE: To determine the epithelial lineage of origin in corneal pannus tissue surgically removed from patients with total limbal stem cell (SC) deficiency.
METHODS: The lineage of origin of the entire conjunctivalized pannus removed from eight corneas with a diagnosis of total limbal SC deficiency was characterized by anti-keratin (K)-3 and anti-K19 monoclonal antibodies. The protein and mRNA of epithelial outgrowth from segments of five such pannus specimens were analyzed by Western blot and reverse transcription-polymerase chain reaction, respectively.
RESULTS: Cross sections of all eigh

Bioprosthetic valve thrombosis was previously considered to be a relatively rare complication of surgical or transcatheter bioprosthetic valve replacement. Although echocardiograms can reliably show the characteristic findings of prosthetic valve stenosis, differentiating between thrombus formation and pannus overgrowth as the underlying cause of prosthetic valve dysfunction can be challenging. We present the case of a 75-year-old man who underwent transthoracic Doppler echocardiography in the presence of an elevated valvular gradient 2 years after his aortic valve had been surgically replaced

Pannus formation is a known complication of valve replacement surgery. However, few cases describe pannus formation in the mitral position, and they are mostly associated with prosthetic valves rather than only annuloplasty. We describe the case of a 62-year-old woman who first underwent reductive annuloplasty using a Carpentier-Edwards 28 mm ring prosthesis in 2009 that performed well on regular follow-up for 5 years. In 2014, the patient presented with significant weight gain, shortness of breath (New York Heart Association functional class III) and signs of severe mitral stenosis. Echocardi

Relatively little attention has been paid to lumbar spine involvement in rheumatoid arthritis (RA), and indeed it is generally considered to be an uncommon and usually clinically minor manifestation of the disease. We describe a case of acute right lower extremity weakness secondary to compression of multiple lumbar nerve roots by a large interforaminal rheumatoid pannus, and review the literature on this complication and other lumbar spine involvement in RA.

BACKGROUND: Retro-odontoid pannus is a reactive fibrocartilaginous mass that develops posterior to the odontoid process, causing ventral compression of the cervicomedullary junction. Although classically associated with rheumatoid arthritis, it is increasingly recognized in non-inflammatory conditions (e.g., diffuse idiopathic skeletal hyperostosis [DISH]) driven by chronic atlantoaxial instability. Over the past two decades, posterior stabilization alone has emerged as the preferred first-line approach, inducing spontaneous pannus regression by eliminating pathological motion.
