Metastatic endometrial cancer of the paranasal sinuses.

Endometrial carcinoma is the fourth most common cancer affecting women in the UK. Its most frequent sites of spread are to the pelvic and para-aortic lymph nodes, vagina and peritoneum. We report a case of a 63-year-old woman with known endometrial cancer who presented with left facial swelling and eye displacement. Investigations revealed an expansile soft-tissue density mass arising within the bone, centred on the left zygoma, with exophytic extension into the left maxillary antrum, infratemporal fossa and inferiorly into the orbit. Endoscopic biopsies were taken and histology confirmed meta

[ACTH-producing paraganglioma of the paranasal sinuses].

Paragangliomas are tumours of neuroectodermal origin and hormonally active in rare instances. They are very unusual in the paranasal sinuses. We report on the first case--as far as we know--of a recurrent nasal paraganglioma, which started to produce ACTH after a period of ten years and resulted in Cushing's syndrome. Resection of the tumour normalised ACTH and cortisol secretion.

Paranasal sinuses and nasopharynx CT and MRI.

Neoplastic disease of the nose, paranasal sinuses, the nasopharynx and the parapharyngeal space requires thorough assessment of location and extent in order to plan appropriate treatment. CT allows the deep soft tissue planes to be evaluated and provides a complement to the physical examination. It is especially helpful in regions involving thin bony structures (paranasal sinuses, orbita); here CT performs better than MRI. MRI possesses many advantages over other imaging modalities caused by its excellent tissue contrast. In evaluating regions involving predominantly soft tissue structures (ec

Rosai-Dorfman disease of the paranasal sinuses.

A 57-year-old man presented with a history of nasal obstruction of five to six years duration. 'Nasal polyps' were removed on several occasions. He had previously had an episode of paraplegia which resolved after the removal of a spinal tumour. Histology from both sites was thought to represent a malignant fibrous histiocytoma. On presentation the patient had computed tomographical (CT) evidence of extensive ethmoidal disease, with threatened intracranial extension. He also had evidence of lung and retroperitoneal disease with pancreas and kidney involvement. The ethmoidal disease was consider

Magnetic resonance imaging of the paranasal sinuses.

MRI has been very helpful in evaluating tumors in the paranasal sinuses. MRI can differentiate retained secretions from tumors. The multiplanar capabilities of MRI are also very helpful in identifying tumor margins. Although a specific diagnosis can be suggested in most cases, demonstration of the extent of the disease and specifically the relationship to key surgical landmarks is more important. Structures of thin cortical bone are better seen with CT than MRI. For several reasons, CT is still the examination of choice for evaluation of the osteomeatal complex and inflammatory disease of the

Rare infections of nose and paranasal sinuses.

Two cases of fungal infection of the nose and paranasal sinuses by rhinophycomycosis and actinomycosis are reported. The clinical diagnoses in both cases were wrong and in one of them, a malignancy was suspected. These cases are the first to be reported from Central Nigeria. The authors feel that fungal granuloma should be included on the differential diagnoses list for patients with swellings of the nose and paranasal sinuses in this environment.

Paranasal sinus radiology, Part 4A: Maxillary sinuses.

The maxillary sinus is frequently involved by inflammatory change. It is also the sinus that is most affected by intrinsic or nearby neoplasms. Since neoplasia and inflammatory disease may mimic each other, radiologic examination often becomes the principal means of differentiating them. Principles of radiologic diagnosis, with attention to changes in the various bony margins in maxillary disease processes, form the basis of this article.

Paranasal sinus radiology, Part 2A: Ethmoidal sinuses.

The clinician must have a high level of suspicion in order to detect ethmoidal sinus disease. Survey plain film radiographic examination may provide information about gross changes in ethmoidal sinus transparency and the sinus walls. Conventional tomographic study will often aid evaluation of the ethmoidal sinus walls which may be obscured by overlying structures on plain films. Computerized tomography adds a new dimension to ethmoidal sinus study. By revealing not only bone changes but soft-tissue abnormality as well, more complete understanding of the disease process is gained. Computerized

Primary fungal infections of the paranasal sinuses.

There has been an increase in the number of primary and secondary fungal infections of the paranasal sinuses in recent years. Aspergillosis and mucormycosis are the most common fungal infections involving the paranasal sinuses. In the past, fungal infections usually occurred in patients with uncontrolled metabolic diseases or compromised defense mechanisms as an acute, rapidly progressing process, often leading to the patient's demise. Primary fungal infections may also be aggressive in otherwise healthy individuals. Physicians should be aware of these microorganisms in order to establish an e

[A case of paranasal sinusitis-cavernous sinusitis with ophthalmoplegia externa].

A 15-year-old man was admitted because of diplopia and bilateral ptosis which occurred a few days after initial clinical signs, such as fever up, nausea, vomiting and headache. His pupils were anisocoric (Rt. phi 3.5 mm < Lt. phi 6.0 mm). In his left eye, light reflex was absent and its movements were limited in all directions. Brain MRI revealed the findings of paranasal sinusitis in bilateral ethmoidal and sphenoidal sinuses and swelling of bilateral cavernous sinus. Combination of intravenous antibiotic therapy and drainage improved his clinical symptoms and MRI findings. It was diagnosed a

Metastases to nasal cavity and paranasal sinuses.

The sinonasal cavities are rare locations for metastases. Metastases to these locations are usually solitary and produce similar symptoms to those of a primary sinonasal tumor. Nasal obstruction and epistaxis are the most frequent symptoms. The maxillary sinus is most frequently involved. The most common primary tumor sites to spread to this region originate in the kidney, breast, thyroid, and prostate, although any malignancy could potentially lead to a metastasis to the paranasal sinuses. The patient's prognosis is usually poor because of the fact that the sinonasal metastasis is usually ass

The phylogenetic significance of anthropoid paranasal sinuses.

In this study, the phylogenetic significance of anthropoid paranasal sinus anatomy is explored. New information reported in recent years has precipitated new hypotheses of sinus homology and more than doubled the number of anthropoid genera for which confident assessments of sinus identity can be made. As a result, it is likely that the phylogenetic meaning of commonly cited characters such as the ethmoid and frontal sinuses will change. The traditional method of "character mapping" is employed to test hypotheses of sinus homology and to reconstruct the ancestral states for sinus characters in

Endodermal sinus tumor of the paranasal sinuses.

BACKGROUND: We report a rare case of endodermal sinus tumor (EST) of the paranasal sinus in a 59-year-old man with a 6-week history of nasal obstruction. METHODS AND RESULTS: High-resolution MRI showed a right nasoethmoid mass with obstructive changes in the sphenoid, frontal, and right maxillary sinuses and orbital and intracranial extension. Histologic analysis showed 2 distinct histologic features: poorly differentiated carcinoma and EST. Immunohistochemical studies showed strong keratin staining in both components and restricted positivity of alpha-fetoprotein in the endodermal sinus-like

Haemangioendothelioma of paranasal sinuses with intracranial extension.

Maxillary sinus is a rare site for the occurrence of haemangioendothelioma. The present case is being reported in a 45 year old Hindu Male. The main complaint ware bilateral nasal obstruction und a mass protruding from he right nostril along with episodes of headache, vomiting and nasal bleeding. The. C.T. scan showed evidence of erosion of bony walls of maxillary antrum with intracranial extension of the tumor. There was no evidence of regional lymph nodes or distance metastasis. Histology of the tissue revealed a highly cellular and vascular tumor the diagnosis of haemangioendothelioma of th

Paecilomyces fungus infection of the paranasal sinuses.

Fungal infections caused by Paecilomyces species are very rare and occur in adult patients with impaired host defences or following foreign body implants. They are found worldwide in soil and decaying vegetation. We describe a case of an 8 year old child who came with complaints of left sided nasal obstruction and discharge, telecanthus, diplopia and epiphora. On examination and investigation he had pansinusitis caused by P. lilacinus. An endoscopic sinus surgery was done and the patient was on oral Itraconazole for 6 months. To our knowledge, this is the first case reported in the paediatric
