The influence of weight-bearing and flexion on 3D joint space width and cartilage thickness in knee osteoarthritis: a rotatable MRI study.

BACKGROUND: Radiographic joint space width (JSW) and MRI-based cartilage thickness are key imaging biomarkers in knee osteoarthritis (OA), yet their weak correlation complicates cross-modality interpretation. This mismatch may potentially be explained by differences in patient posture and joint loading, as radiographs are typically obtained in a weight-bearing and flexed position, whereas MRI scans are acquired in a non-weight-bearing and extended position. METHODS: Using a 0.25 T rotatable MRI scanner, 21 individuals with OA were scanned in four positions, extended and flexed in non-weight-b

Obturator Dislocation of the Hip with Associated Femoral Head Impaction and Medial Wall Fracture of the Acetabulum.

INTRODUCTION: The combination of traumatic obturator dislocations and a femoral head impaction is rare and the treatment challenging. This report describes the successful management of this rare injury in a young patient. CASE REPORT: A 22-year-old truck driver involved in a ski accident sustained an obturator dislocation of the right hip associated with a femoral head impaction in the weight-bearing zone and a medial wall fracture of the acetabulum. After an initial closed reduction within 6 hours after the accident, for the definitive treatment, the hip joint was exposed through a Kocher-La

A new type of elastic fixation, using an encircling and binding technique, for tibiofibular syndesmosis stabilization: comparison to traditional cortical screw fixation.

BACKGROUND: The distal tibiofibular syndesmosis (DTS) is a complex fibrous joint that contributes to the stability and weight-bearing function of the ankle. As such, repair of DTS injury is required, providing fixation strength while maintaining ankle range of motion. The aim of this study was to compare a new elastic fixation technique, using an encircling and binding technique, for DTS stabilization, compared to the traditional cortical bone screw fixation. METHODS: This was a retrospective analysis of 67 patients treated for a DTS injury at our hospital, between June 2019 and June 2021. Of

Deltoid ligament augmentation replacing syndesmotic fixation for the treatment of ankle fractures: a prospective randomized controlled study.

OBJECTIVES: The ankle fracture with both deltoid ligament (DL) rupture and syndesmotic diastasis was treated by fixing syndesmosis after open reduction and plate fixation of the fibular fracture. The present study aimed to evaluate the effect of deltoid ligament augmentation (DLA) replacing syndesmosis fixation for the treatment of such a fracture. METHODS: This randomized controlled trial recruited acute ankle fracture patients with syndesmotic instability and DL rupture. Patients were randomized into DLA and trans-syndesmotic screw fixation (TSSF) groups. The primary outcome measure was the

Sole reconstruction using anterolateral thigh perforator free flaps.

BACKGROUND: The goal of sole reconstruction should be functional and aesthetic. These goals can be achieved by providing the sole with a durable and comfortable weight-bearing surface, adequate contour, protective sensation, and solid anchoring to deep tissue to resist shearing. Various flaps such as fasciocutaneous, musculocutaneous, or split skin grafted muscle flaps have been reported for reconstruction of the weight-bearing foot. The perforator flap, however, deserves attention in the debate for the ideal flap because of its anatomical characteristics. METHODS: Between June of 2002 and Fe

Acute on chronic stress fracture after a medial malleolar osteotomy following osteochondral lesion of the talus treatment.

A patient in his 40s presented at the outpatient clinic with sudden pain and swelling over his medial malleolus, 16 weeks after he had undergone osteoperiosteal autografting with a medial malleolar osteotomy for his tertiary osteochondral lesion of the talus. Postoperatively, the patient was treated using the regular evidence-based rehabilitation protocol of 5 weeks of non-weight-bearing and 5 weeks of partial weight-bearing. However, after the confirmed radiological union the patient experienced an acute on chronic stress fracture through the osteotomy line. The fracture was treated with immo

Total hip arthroplasty with subtrochanteric shortening osteotomy for Crowe grade 4 dysplasia using the direct anterior approach.

Between September 2006 and June 2011, 1521 primary total hip arthroplasties were performed using the direct anterior approach on a standard surgical table. In 12 hips, subtrochanteric shortening osteotomy was required. Full weight bearing was allowed 1 week after surgery. The mean follow-up period was 3.7 years. All patients limped preoperatively, but no patient had a positive Trendelenburg or Duchenne limp at the last follow-up. On the basis of our findings, we believe that the direct anterior approach is a safe and reproducible technique for total hip arthroplasty with subtrochanteric shorte

[The ankle joint fork and tarsus].

Severe soft tissue injury occurring in sprains and dislocations may lead to painful disability after plaster-immobilisation for 6-12 weeks. Therefore, primary operation is indicated for ligamentous repair, osteochondral fracture-fixation and internal K-wire stabilization of dislocations after reduction. After 1 week of immobilisation, the patient should begin active daily muscle exercises; in the 2nd week patients are mobilized with support of a dynamic, partial weight-bearing splint, allowing continued muscle exercises until weight-bearing is possible 6-8 weeks after surgery. In 23 patients u

Management of ankle sprains.

One hundred fifty-seven patients between the ages of 15 and 65 were evaluated for ankle injury. Patients without fractures were assigned to one of two treatment groups. Patients in the plaster splint group were non-weight-bearing. Patients in the early mobilization group used an elastic bandage and were permitted to begin partial weight bearing as soon as tolerated while using crutches. The talar tilt (stress) test was not found to be a useful routine radiographic study. Of the patients with follow-up, no significant benefit was obtained from the plaster splint. Aside from a previous ankle inj

A surgical ankle sprain pain model in the rat: effects of morphine and indomethacin.

Ankle sprain is a frequent injury in humans that results in pain, swelling and difficulty in walking on the affected side. Currently a suitable animal model resembling human ankle sprain is lacking. Here, we describe an animal ankle sprain model induced by ankle ligament injury (ALI) in rats. Cutting combinations of the lateral ankle ligament complex produced pain, edema and difficulty of weight bearing, thereby mimicking severe (grade III) ankle sprain in humans. Analgesic compounds, morphine and indomethacin, significantly reversed the reduced weight bearing, thus indicating that reduction o

Forearm support is associated with reduced upper arm muscle activity during direction-specific isometric wrist exertions in young healthy adults.

Upper arm muscles exhibit high muscle activity during distal upper limb motor tasks, which may contribute to musculoskeletal disorders of the upper arm. More specifically, elbow flexor muscle activity is elevated during upwards and leftwards wrist exertions, while downwards exertions produce high elbow extensor activity. While some evidence suggests that forearm supports may reduce the activity of these upper arm muscles, this has not been extensively investigated, nor is it clear how support might interact with wrist exertion direction. Thus, the purpose of the present study was to determine

Load-bearing capacity and wear characteristics of short fiber-reinforced composite and glass ceramic fixed partial dentures.

The aim of this study was to evaluate load-bearing capacity and wear performance of experimental short fiber-reinforced composite (SFRC) and conventional lithium-disilicate CAD/CAM fabricated fixed partial dentures (FPDs). Two groups (n = 12/group) of three-unit CAD/CAM fabricated posterior FPDs were made. The first group used experimental SFRC blocks, and the second group fabricated from lithium-disilicate (IPS e.max CAD). All FPDs were luted on a zirconia testing jig with dual-curing resin cement. Half of FPDs per group were quasi-statically loaded until fracture. The other half experienced

Sit-To-Stand Lift: Effects of Lifted Height on Weight Borne and Upper Extremity Strength Requirements.

To prevent back and shoulder injuries to nursing personnel, sit-to-stand lifts are used to transfer partial-weight-bearing patients with upper extremity strength. However, the weight-bearing capacity and upper extremity strength required of patients have not been defined. The objectives of this study were to determine: (a) the percentage of body weight borne by the patient when lifted to different heights in the lift and (b) whether a patient needs upper extremity strength. Nineteen healthy normal-weight volunteers (ages 19-39) were transferred from a hospital bed to a wheelchair using (a) a g
