Trends in concurrency, polygyny, and multiple sex partnerships during a decade of declining HIV prevalence in eastern Zimbabwe.

BACKGROUND: Observed declines in the prevalence of human immunodeficiency virus (HIV) infection in Zimbabwe have been attributed to population-level reductions in sexual partnership numbers. However, it remains unknown whether certain types of sex partnerships were more important to this decline. Particular debate surrounds the epidemiologic importance of polygyny (the practice of having multiple wives). METHODS: We analyze changes in reported multiple partnerships, nonmarital concurrency, and polygyny in eastern Zimbabwe during a period of declining HIV prevalence, from 1998 to 2011. Trends

Reducing tokenism in patient and public involvement by integrating the Gothenburg person-centred care framework, relational bridges and impact log - a co-produced position paper.

BACKGROUND: Patient and public involvement (PPI) is widely promoted in health research, care services and governance. It is increasingly framed as a matter of rights, dignity and accountability. Despite strong policy support, PPI remains conceptually diverse, unevenly implemented and persistently vulnerable to tokenism, with PPI contributors often reporting unclear influence and weak feedback. Recent literature has highlighted the need for stronger ethical anchoring of PPI beyond standards, frameworks and reporting requirements. MAIN ARGUMENT: This position paper explores how tokenism in PPI

Aligning medical student training with rural community needs: academic and rural workforce outcomes of a rural longitudinal integrated clerkship in Australia.

INTRODUCTION: The Australian Government continues to provide sustained investment in rural medical education. Despite this investment, rural and remote populations continue to experience health inequities, including persistent workforce shortages and limited access to health services. In response, Griffith University, in partnership with Rural Medical Education Australia, established the Longlook Rural Medical Program in 2010. Longlook is delivered through a Longitudinal Integrated Clerkship model and is designed as a socially accountable rural training program. This study examined academic pe

A public-private partnership to reduce tuberculosis burden in Akwa Ibom State, Nigeria.

BACKGROUND: Tuberculosis (TB) infection and spread are preventable, and TB disease is curable depending on individual and community knowledge of causes of the disease, mode of prevention and cure. An earlier educational intervention carried out in Akwa Ibom State (AKS) of Nigeria in 2006 created awareness of the disease and improved utilization of orthodox medical facilities of residents in 34 communities who had symptoms of TB. OBJECTIVE: The overall aim of this program is to reduce the burden of TB disease in 18 communities of AKS through educational intervention, TB case detection and inte

Redesigning Primary Care Via Health Systems and Communities.

PROBLEM: Traditional primary care practices have not adequately adapted to the clear consensus on the importance of community and social factors in child health. APPROACH: We describe how primary care can improve child and community health through community partnerships that integrate social care into healthcare systems. Specifically, we discuss a National Academy of Medicine heuristic for social care integration into practice and provide examples of primary care/social care integration. Practices/clinicians may feasibly address community and social factors through individual-level social car

Co-creating discovery in basic science: Challenges and opportunities in patient-partnered research for rare liver disease.

BACKGROUND: Patient-partnered basic and translational research is the most productive and meaningful approach to developing informed research questions, generating high-quality data, and producing impactful work. While several organizations have published guidelines for patient-partnered research, few experiential reports in basic science are written from both the patient and researcher perspectives and comment on real-time practical challenges and approaches that ensure the establishment and maintenance of a non-tokenistic, organized, and successful research partnership with patient partners.

Why addressing conflicts of interest is essential to progress in reducing commercially driven health harms: Lessons from tobacco.

Globally and in the UK, significant progress has been made in tackling the harms of tobacco use. This was enabled by addressing the fundamental conflict between the tobacco industry's interests and those of the public, including by rejecting partnerships with the tobacco industry. Conflicts of interest (COIs), like those identified in tobacco control, exist in other areas of public health, yet governments, including the UK, continue to work in partnership with other health-harming industries, including alcohol, ultra-processed food and gambling, despite evidence that partnership approaches whe

Engaging Rural Youth in Community Health Assessment: A Visioning Session Tool for Health Promotion Practice.

Community health assessments (CHAs) are a foundational component of public health planning and accreditation, yet youth are often underrepresented in these processes, particularly in rural communities. Excluding youth perspectives limits opportunities to understand community assets and needs that directly shape young people's health and well-being. This article describes a low-cost, practice-based visioning session tool developed by a local health department to engage rural youth in the CHA process as part of routine public health practice. The visioning session tool uses creative expression t

Experiences on the implementation and maintenance of the Canadian Disability Participation Project: A mixed-methods study.

Establishing a multidisciplinary network of researchers, trainees, and research users-such as the Canadian Disability Participation Project (CDPP)-is a promising approach to promote and support research partnerships and improve the application of disability research findings. This study aimed to 1) describe the implementation of the CDPP network over time and 2) explore members' experiences and reflections on the implementation and maintenance of the CDPP network and its partnerships. This mixed-methods study used survey data, collected among CDPP researchers, trainees and research users in th

Effects of consumers and health providers working in partnership on health services planning, delivery and evaluation.

BACKGROUND: Health services have traditionally been developed to focus on specific diseases or medical specialties. Involving consumers as partners in planning, delivering and evaluating health services may lead to services that are person-centred and so better able to meet the needs of and provide care for individuals. Globally, governments recommend consumer involvement in healthcare decision-making at the systems level, as a strategy for promoting person-centred health services. However, the effects of this 'working in partnership' approach to healthcare decision-making are unclear. Working

Concurrent partnerships and the spread of HIV.

OBJECTIVE: To examine how concurrent partnerships amplify the rate of HIV spread, using methods that can be supported by feasible data collection. METHODS: A fully stochastic simulation is used to represent a population of individuals, the sexual partnerships that they form and dissolve over time, and the spread of an infectious disease. Sequential monogamy is compared with various levels of concurrency, holding all other features of the infection process constant. Effective summary measures of concurrency are developed that can be estimated on the basis of simple local network data. RESULTS

Promoting community institutional partnerships to improve the health of intimate partner violence survivors experiencing homelessness.

BACKGROUND: Disparities faced by individuals experiencing homelessness pose significant threats to the health and wellbeing of communities. Survivors of intimate partner violence are at heightened risk, with over 80% experiencing homelessness at some point in time. The intersection of homelessness and survivorship creates numerous barriers to care including safety concerns, stable housing, employment and childcare needs. The establishment of community institutional partnerships offers an opportunity to provide healthcare in transitional housing settings. AIMS: The aim of this paper is to disc

Tokenism or true partnership: Parental involvement in a child's acute pain care.

AIMS AND OBJECTIVES: To explore parental involvement in the child's acute pain care and establish ways in which parental preferences for involvement in their child's care can be identified, facilitated and enhanced by nurses. BACKGROUND: Despite growing evidence supporting effective acute pain management in children and the availability of national and international practice guidelines, children still experience acute pain. Involving parents in their child's pain care has been identified as being a central tenet of pain management in children. DESIGN AND METHODS: A qualitative study using an

Improving antipsychotic prescribing practices in nursing facilities: The role of surveyor methods and surveying agencies in upholding the Nursing Home Reform Act.

BACKGROUND: Inappropriate prescribing of antipsychotics is a longstanding challenge to providing high quality care in nursing facilities. The 1987 Nursing Home Reform Act (NHRA) had an initial impact in reducing inappropriate prescribing, but rates returned to pre-NHRA levels. While the recent Partnership to Improve Dementia Care in Nursing Homes (Partnership) initiative has been successful in reducing the prescribing of antipsychotics in skilled nursing facilities/nursing facilities (NF), it is not known how this guidance initiative impacted prescribing practices and other care processes conc

Partnering With Doulas in Clinical Settings.

Fostering positive and strong partnerships between clinicians and doulas is essential and can benefit patients, clinicians, and the workplace. The evidence suggests that, when doulas are part of health care teams, they improve patients' experiences and outcomes. Doula-clinician partnerships have demonstrated improvements in communication and patient-centered care, accountability in the health care team, and continuity of care for individuals who have given birth. Doula support aligns with evidence-based outcomes and reduces costs to the overall health system. Barriers to doula-clinician partne

Standardizing default electronic health record tools to improve safety for hospitalized patients with Parkinson's disease.

Electronic Health Record (EHR) systems are often configured to address challenges and improve patient safety for persons with Parkinson's disease (PWP). For example, EHR systems can help identify Parkinson's disease (PD) patients across the hospital by flagging a patient's diagnosis in their chart, preventing errors in medication and dosing through the use of clinical decision support, and supplementing staff education through care plans that provide step-by-step road maps for disease-based care of a specific patient population. However, most EHR-based solutions are locally developed and, thus
