PURPOSE OF REVIEW: The incidence of patellar instability in pediatric patients ranges is 50-100 in 100,000 patients per year. Risk of recurrent dislocations however has been cited from 8.6% to 88% depending on individual patient factors. This manuscript highlights the demographical, historical, and anatomic factors associated with recurrent patellar instability following a first-time patella dislocation in the pediatric population.
RECENT FINDINGS: In recent years, various studies have focused on identifying risk factors for recurrent patellar instability following a primary patellar dislocat

UNLABELLED: The diagnosis of osteochondral fractures and medial patellar retinaculum injuries that originate as a result of Traumatic Patellar Dislocation (TPD) appears to be crucial for establishing the treatment strategy. Those injuries may be efficiently recognized by means of ultrasonography (USG).
MATERIAL AND METHODS: The analysis of the USG patterns in 22 knees after TPD in 21 patients treated between 2004 and 2007 was proceeded. In all children knee USG examination followed its clinical examination and X ray imaging. The standard USG knee assessment was supplemented by patellar articu

Fifty-eight athletically active study participants with 69 knees that had experienced patellar dislocations were available for followup after being selected for a functional rehabilitation program without antecedent immobilization. Follow-up evaluation was at a minimum of 24 months after onset of treatment and averaged 46.2 months. Good or excellent results occurred in 39 (66%) knees treated after an initial patellar dislocation and in 15 (50%) knees with a chronic history of patellar instability. Twenty-six percent of the 69 knees had experienced recurrent patellar instability at followup. Ov

OBJECTIVE: To investigate the effectiveness of modified single patellar tunnel medial patella femoral ligament (MPFL) reconstruction in the treatment of recurrent patellar dislocation.
METHODS: Between January 2023 and June 2023, a total of 61 patients with recurrent patellar dislocation who underwent MPFL reconstruction with autologous semitendinosus were enrolled and divided into 2 groups using random number table method. In the patellar anchor group, 31 patients were treated with MPFL reconstruction with double medial patellar anchors, and 30 patients in the patellar tunnel group were trea

BACKGROUND: Recurrent patellar dislocation (RPD) is a multifactorial disorder, with tibial tuberosity lateralization (TTL) as a major contributor. Although the tibial tuberosity-trochlear groove (TT-TG) distance is widely used to assess TTL, it is influenced by knee flexion and trochlear dysplasia. Based on our clinical observation of increased external rotation of the patellar tendon insertion line in RPD, we hypothesized that this abnormality alters patellar vector forces. Currently, no parameter accounts for the rotational orientation of the patellar tendon insertion line. This study evalua

The frequency of recurrent dislocation of the patella is increased by the young age of onset, the presence of patella alta and permanent subluxation of the patella. The natural history of the patellar syndrome appears to be fairly favourable in everyday activities, but discomfort is often marked in the intermediate term in sports activities. Isolated femoropatellar osteoarthritis rarely causes major functional impairment of gait. Medial or lateral femorotibial osteoarthritis occurs secondarily. Painful cases often present with subchondral anomalies of the lateral patellar facet, lateral tibial

Approximately one-third of skeletally mature patients with primary patellar dislocation will experience recurrent patellar instability over time. Because of the multifactorial combination of features contributing to overall stability of the patellofemoral joint, first-time patella dislocation presents a challenge to the treating physician. A detailed patient history, focused physical examination, and appropriate diagnostic imaging are essential for identifying risk factors for recurrent instability. Individual risk factors include young patient age, patella alta, trochlear dysplasia, and later

Knee dislocation is a relatively rare condition of all orthopaedic injuries. Accompanying multiple ligament injuries are common after knee dislocations. A 41-year-old male presented to the emergency department suffering from right knee dislocation in June 2013. The patient had anterior cruciate ligament, medial collateral ligament (MCL), medial patellofemoral ligament (MPFL) rupture, and lateral meniscal tear. A single-bundle anatomic reconstruction, medial collateral ligament reconstruction, medial patellofemoral ligament reconstruction and meniscus repair were performed in single session. At

Congenital patellar dislocation (CPD) is a rare deformity in children that involves a laterally displaced patella. While potentially identified in early childhood using diagnostic imaging techniques, it is often misdiagnosed at birth, creating pain and mobility issues as the child grows. Dislocation of the patella is permanent and manually irreducible, often manifesting with flexion contracture of the knee, genu valgum, external tibial torsion, and foot deformity. Surgical correction is the treatment of choice in order to prevent future sequelae. We herein present a case of CPD in a four-year-

BACKGROUND: Patellar dislocation occurs when the patella disengages completely from the trochlear (femoral) groove. Following reduction of the dislocation, conservative (non-surgical) rehabilitation with physiotherapy may be used. Since recurrence of dislocation is common, some surgeons have advocated surgical intervention rather than non-surgical interventions. This is an update of a Cochrane review first published in 2011.
OBJECTIVES: To assess the effects (benefits and harms) of surgical versus non-surgical interventions for treating people with primary or recurrent patellar dislocation.

Chondral fractures of the patella are associated with acute dislocation of the patella. Osteochondral fracture in patellar dislocation is located in the medial facet of the patella. This article presents a case of a 15-year-old female ballerina with isolated displaced osteochondral fracture of the patella without patellar dislocation. She had no history of trauma. A Merchant's view of both knees showed mild subluxation of the patella, a small fragment on the lateral aspect of the knee, and a small defect of the centromedial patella. Axial magnetic resonance imaging (MRI) revealed an osteochond

OBJECTIVE: To investigate the procedure and effectiveness of medial patellofemoral ligament (MPFL) reconstruction for the treatment of recurrent patellar dislocation. METHODS; Between June 2005 and September 2007, 29 patients with recurrent patellar dislocation underwent MPFL reconstruction with allograft semitendinosus or allograft anterior tibialis tendon. There were 6 males and 23 females with an average age of 20.3 years (range, 13-45 years). The patients suffered from 2-10 times patellar dislocation preoperatively. The average time between last dislocation and surgery was 43.9 months (ran

BACKGROUND: The treatment of acute patellar dislocation in children is controversial. Some investigators have advocated early repair of the medial structures, whereas others have treated this injury nonoperatively. The present report describes the long-term subjective and functional results of a randomized controlled trial of nonoperative and operative treatment of primary acute patellar dislocation in children less than sixteen years of age.
METHODS: The data were gathered prospectively on a cohort of seventy-four acute patellar dislocations in seventy-one patients (fifty-one girls and twent

Patellofemoral instability is a disabling condition that occurs in adolescence. Recurrence after patellar dislocation has been reported in 2-50% of patients. We compared the distal femur in patients with patellofemoral instability with distal femura of a healthy cohort using mathematical quantification of two-dimensional shape variation at the same position in different knees. One hundred eight computed tomography scans from 54 patients with patellofemoral instability were compared with 197 computed tomography scans from 102 subjects with normal knees. We used principal components analysis to

Irreducible lateral patellar dislocation is rare. It has previously been described with rotation around a vertical axis. We describe a case of irreducible lateral patellar dislocation in a 66-year-old lady with an impaction fracture of medial facet of patella locked on the lateral femoral condyle. Closed reduction was unsuccessful and the patient was treated by total knee replacement. Such a case has not been described previously in the literature.
