Medial patellofemoral ligament anatomy: is it a predisposing factor for lateral patellar dislocation?

PURPOSE: Because the medial patellofemoral ligament (MPFL) is the primary restraint to lateral dislocation of the patella, we aimed, in this controlled study, to verify whether the MPFL with different measurements could be considered another predisposing factor for patellar dislocation. METHODS: A group of 100 consecutive individuals without the criteria for patellar dislocation (trochlear dysplasia, patella alta and lateral patellar tilt) was recruited as a control group and underwent magnetic resonance imaging (MRI) study and another group of 50 patients with patellar instability. Femoral c

Mechanical tensile properties of the quadriceps tendon and patellar ligament in young adults.

We analyzed mechanical tensile properties of 16 10-mm wide, full-thickness central parts of quadriceps tendons and patellar ligaments from paired knees of eight male donors (mean age, 24.9 years). Uniaxial tensile testing was performed in a servohydraulic materials testing machine at an extension rate of 1 mm/sec. Sixteen specimens were tested unconditioned and 16 specimens were tested after cyclic preconditioning (200 cycles between 50 N and 800 N at 0.5 Hz). Mean cross-sectional areas measured 64.6 +/- 8.4 mm2 for seven unconditioned and 61.9 +/- 9.0 mm2 for eight preconditioned quadriceps t

Unusual patellar tendon injury in an adolescent runner with generalised ligamentous laxity.

A case is reported of an acute traction injury of the patellar tendon in a boy of 14 with joint hypermobility. Such injuries are unusual but early diagnosis and surgical repair lead to a good long term outcome. In adolescents participating in sports, the awareness of the possibility of a rare knee extensor mechanism injury is essential for a successful outcome.

Effect of Patient Height and Sex on the Patellar Tendon and Anterior Cruciate Ligament.

BACKGROUND: Graft-tunnel mismatch is an avoidable complication in anterior cruciate ligament (ACL) reconstruction. Patient height and sex may be predictors of patellar tendon length (PTL) and intra-articular ACL length (IAL). Understanding these relationships may assist in reducing graft-tunnel mismatch during ACL reconstruction with bone-patellar tendon-bone (BTB) autograft. PURPOSE: To determine the association of patient height and sex with PTL and IAL. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: Magnetic resonance imaging (MRI) studies were obtained on the health

Cadaveric study of the secondary medial patellar restraints: patellotibial and patellomeniscal ligaments.

PURPOSE: To detail the anatomy of the medial patella stabilizers, medial patellotibial (MPTL), and medial patellomeniscal ligaments (MPML), focusing on the points of origin and insertion, length, thickness, width, and fibres orientation to study the frequency of anatomical variations and the anatomy of these ligaments, thereby improving surgical techniques. METHODS: Thirty dissected knees were analysed. A digital caliper was used to measure the length, thickness, and width, as well as the mid-point of the ligaments insertion and the distance from the MPTL insertion to the articular surface of

Acute rupture of the anterior cruciate ligament and patellar tendon in a collegiate athlete.

In rare incidences of combined ruptures of the ACL and patellar tendon, either the patellar tendon ruptures or the associated ACL tear is often initially missed. Even when recognized, there is no established treatment regimen. We report a case of an intercollegiate football player with a combined rupture of the ACL and patellar tendon that was successfully treated by primary augmented repair of the patellar tendon along with ACL reconstruction. Similar to other reported cases, the mechanism of injury involved forceful eccentric contraction of the quadriceps against a fixed foot. Superior displ

Reconstruction of the medial patellofemoral ligament for patellar instability using a semitendinosus autograft.

Between 1999 and 2001 thirty knees underwent a semitendinosus tendon plasty to recreate the medial patellofemoral ligament for recurrent patellar dislocation. The mean follow-up was 38 months. The mean improvement of the patellofemoral congruence angle after surgery was 14 +/- 7 degrees. All patients ended up with a full range of motion, except one patient, whose flexion was limited to 120 degrees due to superficial wound infections. Dislocation did not recur. According to the Larsen and Lauridsen outcome score the clinical results were excellent in 27 patients, good in 2 and fair in one. In c

[Tendinopathy of the patellar ligament secondary to transtendineal arthroscopy of the knee. Ultrasonographic evaluation].

During knee arthroscopy, the transtendinous approach best visualizes articular structures, even though it may cause patellar tendinitis, following the surgical procedures. Thirty patients (22 men and 8 women) who had undergone transtendinous arthroscopy of the knee were submitted to clinical and US follow-up at 3, 6, 12 months. This monitoring was aimed at correlating clinical and instrumental findings of the above iatrogenic condition. Painful symptoms were present in 30% of the cases in group I, in 10% of group II and in no patient in group III. US demonstrated thickening of the patellar ten

Effect of anterior cruciate ligament reconstruction with patellar tendon or prosthetic ligament on the morphology of the other ligaments of the knee joint. An experimental study in dogs.

The effect of anterior cruciate ligament replacement on the other ligaments of the knee, including the medial or lateral collateral and posterior cruciate ligaments, was studied histologically in the canine knee. The anterior cruciate ligament was replaced either with the medial third of the patellar tendon or with a prosthetic ligament. Histologic analysis was performed 3 months after prosthetic ligament implantation and 1 year after patellar ligament reconstruction. In all ligaments, numerous pathologic alterations were found. Degenerative changes such as glycosaminoglycan accumulation, meta

Concomitant Patellar Tendon Reconstruction With Hamstring Autografts and Medial Patellofemoral Ligament Reconstruction With Quadriceps Autograft in the Setting of Severe Patellar Tendinopathy With Recurrent Lateral Patellar Instability.

Chronic injuries to the extensor mechanism present unique challenges to physicians because of their complex nature and the need for a multifactorial treatment approach. When conservative treatment is unsuccessful in the setting of patellar tendinopathy, surgical intervention may be required to improve symptoms. For patients with concomitant lateral patellar instability, surgical treatment is also indicated to prevent future dislocations and worsening patellofemoral symptoms. Current literature presents numerous patellar tendon and medial patellofemoral ligament arthroscopic and open reconstruc

Ultrasonographic examination of the patellar ligament after capsular and fascial imbrication for the treatment of cranial cruciate ligament rupture in dogs.

INTRODUCTION: Cranial cruciate ligament (CCL) rupture is the most common orthopedic condition in dogs, with many surgical options available for its treatment. Thickening of the patellar ligament after capsular and fascial imbrication (CFI) is a frequently reported complication, but its clinical significance remains unclear. METHOD: This prospective study evaluated patellar ligament structural and thickness changes after CCL repair using CFI. Forty-six dogs with CCL ruptures treated at the Small Animal Clinic of Freie Universität Berlin between July 2013 and April 2015 were included. Clinical,

Physeal-Sparing Medial Patellofemoral Ligament Reconstruction With Patellar Tendon Shortening and Guided Growth.

BACKGROUND: In skeletally immature patients, alternative surgical approaches should be undertaken for the treatment of patellofemoral instability that respect the open physis. Remaining growth can be exploited for correction of genu valgum, a known anatomic risk factor of patellofemoral instability. INDICATIONS: A skeletally immature 14-year-old boy with bilateral genu valgum and patella alta presented with symptomatic, recurrent left knee patellar instability precluding sports participation and causing dysfunction in his activities of daily living. TECHNIQUE DESCRIPTION: A midline approach

Bone patellar tendon bone graft for ulnar collateral ligament reconstruction: a biomechanical study.

BACKGROUND: Ulnar collateral ligament (UCL) tears are a common injury among overhead athletes, leading to a lengthy recovery period after surgical reconstruction. To explore alternatives to decrease recovery time, we compared the biomechanical properties of the standard palmaris reconstruction with a novel bone-patellar tendon-bone (BTB) reconstruction with interference screw fixation. METHODS: Sixteen fresh frozen cadaver upper extremities and 8 cadaver knees were obtained (elbows 77 ± 9 years, male:female 1:3 ratio; knees 59 ± 7 years, male:female 3:1 ratio). Eight upper extremities were ra

Midterm Results after Implant-Free Patellar Fixation Technique for Medial Patellofemoral Ligament Reconstruction.

Problems and complications concerning the patellar fixation in medial patellofemoral ligament reconstruction (MPFLR) have been reported. The purpose of this retrospective study was to systematically analyze the surgical technique for MPFLR with a V: -shaped patellar bonetunnel for implant-free fixation of an autologous gracilis tendon, allowing early functional rehabilitation, regarding restoration of the patellofemoral stability, patient satisfaction, return to sports, and technique-specific complications. In 2010, 128 cases of consecutive isolated MPFLR were performed. All these cases were i

Hamstring tendons or bone-patellar tendon-bone graft for anterior cruciate ligament reconstruction?

In anterior cruciate ligament (ACL) reconstruction, success depends on several factors: patient selection; surgical technique, taking account of associated meniscal and ligamentous lesions; and postoperative rehabilitation. Improved hamstring-tendon graft preparation and fixation techniques now provide results in terms of laxity and retear comparable to patellar tendon graft, which has long been the gold-standard for patients with strong functional demand, despite its higher risk of extension stiffness and anterior pain. Apart from a few exceptional preferential indications, such as hamstring

Patellar tracking after isolated medial patellofemoral ligament reconstruction: dynamic evaluation using computed tomography.

PURPOSE: Medial patellofemoral ligament (MPFL) reconstruction offers good clinical results with a very low rate of instability recurrence. However, its in vivo effect on patellar tracking is not clearly known. The aim of this study is to investigate the effects of MPFL reconstruction on patellar tracking using dynamic 320-detector-row CT. METHODS: Ten patients with patellofemoral instability referred to isolated MPFL reconstruction surgery were selected and subjected to dynamic CT before and ≥6 months after surgery. Patellar tilt angles and shift distance were analysed using computer software
