Alignment strategies in total knee arthroplasty and the patellofemoral joint: A systematic review.

BACKGROUND: Different alignment strategies in total knee arthroplasty (TKA) may affect the patellofemoral joint. Mechanical alignment (MA) is commonly used but may alter native anatomy. Newer strategies such as kinematic alignment (KA), restricted kinematic alignment (rKA), and functional alignment (FA) aim to better restore native joint mechanics. This study provides an overview of the effects of alignment strategies on patellofemoral outcomes after TKA. METHODS: A literature search in July 2025 identified studies comparing patellofemoral outcomes in TKA using different alignment strategies.

Assessment and management of torsional malalignment in paediatric patients with patellofemoral joint instability.

PURPOSE: Torsional malalignment is an important but frequently underrecognized contributor to patellofemoral instability (PFI) in children and adolescents. This review aimed to provide a clinically focused overview of the development, assessment, imaging evaluation, and contemporary management of femoral and tibial torsional abnormalities associated with PFI. METHODS: A literature search of PubMed and Embase was performed from database inception, using the terms ((patella) OR (patellofemoral)) AND ((rotation) OR (torsion)). Titles and abstracts were screened by two authors, followed by full-t

Development and validation of a kinematically-driven discrete element model of the patellofemoral joint.

Quantifying the complex loads at the patellofemoral joint (PFJ) is vital to understanding the development of PFJ pain and osteoarthritis. Discrete element analysis (DEA) is a computationally efficient method to estimate cartilage contact stresses with potential application at the PFJ to better understand PFJ mechanics. The current study validated a DEA modeling framework driven by PFJ kinematics to predict experimentally-measured PFJ contact stress distributions. Two cadaveric knee specimens underwent quadriceps muscle [215 N] and joint compression [350 N] forces at ten discrete knee positions

The association of patellofemoral joint morphology with chondromalacia patella: a quantitative MRI analysis.

The relationship of patellofemoral congruency with chondromalacia patellae (CP) was retrospectively evaluated. Lateral patellar tilt angle (LPTA), sulcus angle (SA), trochlear depth (TD), and patella angle (PA) were measured at 301 knee magnetic resonance images and compared between groups with and without CP. In the CP group, LPTA and TD were significantly low (P<.01), SA was high (P<.01), while PA showed no difference (P>.05). The parameters were also compared between groups with mild and severe CP, and no significant difference was found (P>.05). Our results demonstrate that patellar tilt a

Ruptures of the extensor mechanism of the knee joint. Clinical results and patellofemoral articulation.

The functional results of 28 cases of rupture of the quadriceps and patellar tendons are reported. Excellent or good results were noted in 15 of 18 quadriceps and 7 of 10 patellar tendons. Radiographic comparison with the opposite knee disclosed incongruences between the patella and the femoral groove in the tangential view and/or cranial-caudal position of the patella in the lateral view in 13 of the quadriceps tendon ruptures and seven of the patellar tendon ruptures. Patients with residual pain had patellofemoral incongruity but since two-thirds of the patients with incongruity were asympto

Radionuclide imaging of the patellofemoral joint in young adults with anterior knee pain.

Young patients with symptoms of anterior knee discomfort represent a most difficult and often enigmatic clinical group, in large part because of the highly subjective nature of the condition. A primary clinical research goal over the past several years has been a search for reliable objective indicators of a presumed underlying pathologic process to account for the symptoms. We believe that the use of the bone scan along with other clinical and experimental data has provided a new and previously unappreciated perspective of a dynamic osseous process occurring in many such patients. With furthe

The role of radiography in the evaluation and treatment of common anarthrotic disorders of the patellofemoral joint.

Patellofemoral disorders are among the most common knee problems in the world. Although they are often considered as a group, pathologically the individual disorders are as variable as fingerprints. Treatment of resistant cases requires a detailed understanding of the disorder mechanism to be corrected. Many radiographic techniques, incorporating plain radiography, arthrography, CT, and MRI have been formulated to determine formats of chondromalacia, instability, and malalignment. Many of the techniques reviewed herein reveal distinct abnormalities but do not necessarily provide insight into t

Influence of patellofemoral joint reaction force changes at small knee flexion angles on patellofemoral pain during single limb landing in active females.

BACKGROUND: Excessive patellofemoral joint (PFJ) loading is considered a primary stimulus for patellofemoral pain (PFP) development. However, little is known about how changes in PFJ loading influence PFP in active females, who are affected more frequently than males. PURPOSE: Examine if changes in the patellofemoral joint reaction force (PFJRF) caused by forward trunk lean are associated with changes in self-reported PFP and hip and knee kinematics during single limb landing. Examine if and to what extent PFJRF reductions are related to changes in self-reported PFP. METHODS: Twenty-six acti

Patellofemoral Joint (PFJ) Arthroplasty in Young Patients: Mid-term Results and Functional Outcomes.

Purpose Isolated patellofemoral arthritis is a well-recognized variant of knee osteoarthritis. The cause of isolated patellofemoral arthritis can be traumatic, secondary to malalignment, degenerative, or idiopathic. This study aims to evaluate whether the results of our third-generation patellofemoral arthroplasty (PFA) support its continued use as a viable treatment option for isolated patellofemoral arthritis. Methods A retrospective cohort study was conducted using the electronic NHS trust database to identify all patients who underwent PFA between September 2008 and April 2014. Each patien

Patient-specific gait pattern in individuals with patellofemoral instability reduces knee joint loads.

Patellofemoral instability is influenced by morphological factors and associated with compensational alterations in gait pattern. Recent simulation studies investigated the impact of knee morphology on the stability and loading of the patellofemoral joint but neglected the patient-specific gait pattern. The aim of this study was to investigate the impact of patient-specific gait pattern on muscle forces and joint loading in individuals with patellofemoral instability. Musculoskeletal simulations with a model including a twelve degrees of freedom knee joint were performed based on three-dimensi

Influence of patellofemoral joint degeneration on clinical outcomes after medial unicompartmental knee arthroplasty.

BACKGROUND: Patellofemoral joint (PFJ) degeneration has traditionally been regarded as a contraindication to unicompartmental knee arthroplasty (UKA). More recently, some researchers have proposed that PFJ degeneration can be ignored in medial UKA, and others have proposed that this change should be reviewed in PFJ degenerative facets and severity. This study aimed to systematically evaluate the effect of PFJ degeneration on patient-reported outcome measures (PROMs) and revision rates after medial UKA. METHODS: Electronic databases (PubMed, Embase, Web of Science, etc.) were searched for stud

Influence of step length and landing pattern on patellofemoral joint kinetics during running.

Elevated patellofemoral joint kinetics during running may contribute to patellofemoral joint symptoms. The purpose of this study was to test for independent effects of foot strike pattern and step length on patellofemoral joint kinetics while running. Effects were tested relative to individual steps and also taking into account the number of steps required to run a kilometer with each step length. Patellofemoral joint reaction force and stress were estimated in 20 participants running at their preferred speed. Participants ran using a forefoot strike and rearfoot strike pattern during three di

Patellofemoral joint force and stress during the wall squat and one-leg squat.

PURPOSE: To compare patellofemoral compressive force and stress during the one-leg squat and two variations of the wall squat. METHODS: Eighteen subjects used their 12 repetition maximum (12 RM) weight while performing the wall squat with the feet closer to the wall (wall squat short), the wall squat with the feet farther away from the wall (wall squat long), and the one-leg squat. EMG, force platform, and kinematic variables were input into a biomechanical model to calculate patellofemoral compressive force and stress as a function of knee angle. To asses differences among exercises, a one-f
