ObjectiveThis systematic review synthesizes randomized controlled trial evidence on the effectiveness of nurse-led ACP interventions for patients with advanced cancer.MethodsWe searched PubMed, Embase, CINAHL, Web of Science, and the Cochrane Central Register from inception to January 2026. Eligible studies were randomized controlled trials (RCTs) evaluating nurse-led ACP interventions in adults with advanced or metastatic cancer. Two reviewers independently screened records, extracted data, and assessed risk of bias using the Cochrane Risk of Bias 2 (RoB 2) tool. A narrative synthesis was per

OBJECTIVES: Emergency department (ED) visits offer opportunities for seriously ill patients to formulate future medical care goals, yet ED clinicians lack practical strategies for these conversations. ED GOAL, a behavioral intervention, engages seriously ill yet clinically stable older adults in the ED to address advance care planning (ACP) with their outpatient clinicians. In a randomized trial, goals-of-care documentation was significantly higher in the intervention group compared to controls after three (24.3% vs. 9.9%, p = 0.03) and 6 months (31.4% vs. 12.7%, p < 0.01). INTRODUCTION: Despite a severely degraded quality of life in the advanced stages, chronic obstructive pulmonary disease (COPD) patients rarely discuss end-of-life issues and have little access to palliative care. After knowing this fact, some organisations recommend advance care planning integrated into the respiratory rehabilitation. Considering the important role played by physiotherapists in this rehabilitation, the aim of this study is to understand the professional practices of physiotherapists regarding this planning during rehabilitation and the factors influencing these practices.

OBJECTIVE: The aim of this study is to examine why patients are hospitalised in the last stage of life.
METHODS: Our study was conducted in a large Dutch teaching hospital. We conducted a retrospective chart review of patients aged ≥18 years who died of cancer either during hospitalisation or after discharge to receive terminal care outside the hospital. We collected data about the characteristics of these hospitalisations and indicators of advance care planning.
RESULTS: Of the 264 deceased patients, 56% had died in the hospital and 44% after hospital discharge. Of all patients, 80% had bee

Chronic conditions, whether genetic or acquired, impose a significant burden on health care systems with high utilisation of hospital and emergency department resources. Self-management is increasingly recognised as one of the pillars in models of care for those with long-term medical conditions. Hereditary angioedema (HAE) is a rare genetic disorder inherited in an autosomal dominant fashion. It is characterised by the occurrence of unpredictable attacks of swelling (angioedema) affecting many body parts including subcutaneous tissues, the gut mucosa and the upper airway. For those affected,

PURPOSE: Despite the Institute of Medicine's (IOM) 2005 recommendation, few care organizations have instituted standard survivorship care plans (SCPs). Low health literacy and low English proficiency are important factors to consider in SCP development. Our study aimed to identify information needs and survivorship care plan preferences of low literacy, multi-lingual patients to support the transition from oncology to primary care and ongoing learning in survivorship.
METHODS: We conducted focus groups in five languages with African American, Latina, Russian, Filipina, White, and Chinese medi

BACKGROUND: The survivorship needs of patients living with chronic cancer (CC) and their use of survivorship care plans (SCPs) have been overlooked and underappreciated.
METHODS: A convenience sample of 39,088 SCPs completed for cancer survivors with an Internet-based SCP tool was examined; it included 5847 CC survivors (15%; CC was defined as chronic leukemia and/or recurrent/metastatic cancer of another nature). Patient-reported treatment effects and follow-up care patterns were compared between CC survivors and survivors treated with curative intent (CI). Responses from a follow-up survey

AIMS AND OBJECTIVES: This study aims to evaluate the service impact of the integration of an evidence-based instrument - the Personalised Patient Education Protocol - into an existing postmyocardial infarction care pathway.
BACKGROUND: Recent research indicates that while better patient health outcomes can be achieved when care planning is personalised, delivery staff feel less satisfied and less confident in its provision. To achieve a shift to personalised care, innovations are needed to enable an effective transition for staff.
DESIGN: A service evaluation using a patient survey and nurse

ASSESSMENT: Acute respiratory distress syndrome (ARDS) is a severe form of acute respiratory failure characterized by severe hypoxemia, reduced lung compliance, and bilateral pulmonary opacities not fully explained by cardiogenic causes. We report a case of severe refractory ARDS in the immediate postoperative period following urgent abdominal surgery in a patient with extreme obesity, focusing on nursing assessment and care planning during venovenous extracorporeal membrane oxygenation (VV-ECMO) support. A 47-year-old woman (BMI 71.11 kg/m2) developed severe ARDS (PaO2/FiO2 60 mmHg) after asp

INTRODUCTION: The increasing global cancer burden and advances in treatments have extended patients' life expectancy, leading to greater prognostic uncertainty and a higher demand for palliative care. Advance care planning (ACP) is essential in this context, ensuring that patients' future care aligns with their values and preferences. Oncology case managers, being nurses with specialized training, have emerged as key ACP conversationalists. This study explores the perspectives of healthcare professionals and patients on case managers as ACP conversationalists for advanced cancer patients.

BACKGROUND: Stroke remains a major chronic condition in China, characterized by high incidence, frequent recurrence, and substantial disability. These challenges not only affect patients’ quality of life but also place a heavy burden on families and the healthcare system. Advance care planning provides a structured approach to help patients make informed decisions about their future medical care. However, little is known about how patients with recurrent stroke perceive and prioritize advance care planning services. Gaining insight into their needs and preferences is essential for designing in

KEY POINTS: Higher critical health literacy was associated with informal advance care planning, while higher total health literacy was associated with both informal and formal advance care planning in patients receiving hemodialysis. Patients unsure about treatment preferences had lower total health literacy, and lack of opportunity was the most common barrier to advance care planning engagement. Enhancing health literacy and providing structured opportunities for advance care planning may support patient-centered decision making in dialysis care.
BACKGROUND: Although qualitative studies sugg

Background The majority of those aged 65 and older will visit the emergency department (ED) in the last six months of life. Knowing a patient's goals of care is important, and existing medical records do not always represent them well. We set out to determine the baseline availability of advance directives and goals of treatment in those ED patients at increased risk for mortality. Methods This prospective cohort study included a sample of adult ED patients who had a mortality predictor by an End-of-Life (EOL) Deterioration Index-guided electronic best practice advisory (BPA) or admission to a

INTRODUCTION: Orthodontic treatment in cleft lip and palate and craniofacial anomalies is complex and requires a multidisciplinary approach. There are often multiple possible treatment plans. To properly explain and discuss the various options, such as management of frequently missing incisors or the choice between orthognathic surgery and dental compensation, assessment of burden of treatment must be made and adequately communicated to the patients and families.
PATIENTS AND METHODS: 105 patients affected by cleft lip and palate and craniofacial anomalies were retrospectively collected and d

PURPOSE: People with multiple myeloma are living longer with an incurable disease due to improved treatments. Despite advance care planning (ACP) and palliative care's focus on improving symptom management and quality of life, there remains a perception among patients and clinicians that palliative care is only for end-of-life care, leading to uncertainty around when to introduce these conversations. This study examines the lived experiences of myeloma patients and their carers, exploring their views as to when and how they would like to commence meaningful conversations around palliative and

INTRODUCTION: Venous leg ulcers affect 1% of the population worldwide and more than 3% of the older population. Recurrence rates range from 57-78% within one year after healing. In Sweden, the shift toward specialised primary care increases the need to support self-care planning to enhance healing and prevent recurrences. The study aimed to describe the phenomenon of planning self-care with patients with venous leg ulcers, as experienced by healthcare professionals.
METHODS: Twenty-four lifeworld interviews were conducted with healthcare professionals and analysed using reflective lifeworld r

BACKGROUND: Older adults with serious illness often face treatment decisions shaped by personal values, yet recognising and expressing those values in ways that guide decision-making can be challenging. This study evaluated the acceptability, usability and effectiveness of MyCare, a digital values clarification tool designed to help older adults identify and communicate their care preferences to substitute decision-makers, healthcare providers and support persons.
METHODS: This convergent mixed-methods study explored older adults' experiences with MyCare. Acceptability was assessed using the
