Pelvis radiographs in children with cerebral palsy: effects of patient positioning on calculating migration percentages.

BACKGROUND: Hip displacement in children with cerebral palsy (CP) is monitored by measuring migration percentage on anteroposterior pelvis radiographs. However, proper positioning for radiography in children with spasticity is difficult. The reliability and accuracy of migration percentage as a function of patient positioning is unknown. OBJECTIVE: To determine the effects of patient positioning on migration percentage measurements in children with CP. MATERIALS AND METHODS: We identified children with CP (≤18-year-old) with pelvis CT and anteroposterior pelvis radiograph obtained <6 months

Cochlear implant positioning and fixation using 3D-printed patient specific surgical guides; a cadaveric study.

HYPOTHESIS: To develop and validate the optimal design and evaluate accuracy of individualized 3D- printed surgical guides for cochlear implantation. BACKGROUND: Positioning and fixation of the cochlear implant (CI) are commonly performed free hand. Applications of 3-dimensional (3D) technology now allow us to make patient specific, bone supported surgical guides, to aid CI surgeons with precise placement and drilling out the bony well which accommodates the receiver/stimulator device of the CI. METHODS: Cone beam CT (CBCT) scans were acquired from temporal bones in 9 cadaveric heads (18 ear

Awake intubation and awake prone positioning of a morbidly obese patient for lumbar spine surgery.

We describe the awake fibreoptic intubation followed by awake prone positioning of a woman weighing 180 kg with body mass index of 62 kg.m(-2) requiring emergency lumbar discectomy for cauda equina syndrome. This approach was taken to overcome difficulties in transfering an anaesthetised patient on to the operating table. Following topical anaesthesia with lidocaine and low-dose remifentanil infusion, oral intubation was performed using a split oral airway to guide the fibrescope. The patient positioned herself on the Jackson operating table, following which general anaesthesia was induced. Th

Improved accuracy of alignment with patient-specific positioning guides compared with manual instrumentation in TKA.

BACKGROUND: Coronal malalignment occurs frequently in TKA and may affect implant durability and knee function. Designed to improve alignment accuracy and precision, the patient-specific positioning guide is predicated on restoration of the overall mechanical axis and is a multifaceted new tool in achieving traditional goals of TKA. QUESTIONS/PURPOSES: We compared the effectiveness of patient-specific positioning guides to manual instrumentation with intramedullary femoral and extramedullary tibial guides in restoring the mechanical axis of the extremity and achieving neutral coronal alignment

CT-guided adrenal biopsy: comparison of ipsilateral decubitus versus prone patient positioning for biopsy approach.

OBJECTIVES: To compare ipsilateral decubitus and prone patient positioning for performing computed tomography guided adrenal biopsy using the requirements for out-of-plane approach (OOP) and the needle insertion time (NIT) as a surrogate for procedure complexity. METHODS: The study included 106 adrenal biopsies performed in 104 patients with lesions measuring ≤ 4 cm that were divided into two groups: Ipsilateral decubitus (Group I) and prone (Group II) positions. The frequency of use of an OOP biopsy path and the NIT were recorded as well as diagnostic yield, adverse events and transgression

Prone positioning improves survival in severe ARDS: a pathophysiologic review and individual patient meta-analysis.

Prone positioning has been used for over 30 years in the management of patients with acute respiratory distress syndrome (ARDS). This maneuver has consistently proven capable of improving oxygenation in patients with acute respiratory failure. Several mechanisms can explain this observation, including possible intervening net recruitment and more homogeneously distributed alveolar inflation. It is also progressively becoming clear that prone positioning may reduce the nonphysiological stress and strain associated with mechanical ventilation, thus decreasing the risk of ventilator-induced lung

[Phenotype positioning on chromosomes in a patient with the syndrome of partial trisomy 7p21.2-->pter].

By using the techniques of human chromosome G-banding, high resolution banding and fluorescence in situ hybridization (FISH), we investigated a patient with the karyotype of partial trisomy 7p21.2-->pter. Combining with the comparative review of the clinical data in 14 cases with partial trisomy 7p syndrome from reported literatures,we searched for the correlation between the karyotype and the phenotype and between the karyotype and the associated gene in the patients with partial trisomy 7p syndrome. The results indicated that the 7p21.2-->p22 is the critical segment of partial trisomy 7p syn

[Development and evaluation of a positioning system for radiotherapy patient based on structured light surface imaging].

This paper aims to propose a noninvasive radiotherapy patient positioning system based on structured light surface imaging, and evaluate its clinical feasibility. First, structured light sensors were used to obtain the panoramic point clouds during radiotherapy positioning in real time. The fusion of different point clouds and coordinate transformation were realized based on optical calibration and pose estimation, and the body surface was segmented referring to the preset region of interest (ROI). Then, the global-local registration of cross-source point cloud was achieved based on algorithms

Awake prone positioning in a patient with respiratory impairment due to subarachnoid hemorrhage: a case report.

BACKGROUND: Prone positioning has been shown to be an effective rescue strategy in severe acute respiratory distress syndrome and was widely used during the coronavirus disease 2019 pandemic, both in mechanically ventilated and in awake patients. Subarachnoid hemorrhage is often associated with respiratory failure. Prone positioning has been used in brain-injured patients, but concerns relating to neurological complications from intracranial hypertension still remain. CASE PRESENTATION: We report the case of a 59-year-old Italian patient with subarachnoid hemorrhage who safely underwent awake

Impact of patient positioning on hemodynamic assessment: A comparison of supine and upright right heart catheterization.

The impact of patient positioning on PCWP measurement and PH classification is underexplored. This study evaluates the differences in hemodynamic assessments between supine and upright RHC. We conducted a retrospective analysis of a single-center iCPET registry, comparing sequential hemodynamic measurements in the supine and upright positions in 1307 patients. Compared to upright RHC, supine RHC showed consistently higher mean PAP (21 ± 8.5 mmHg vs. 16 ± 8 mmHg) and PCWP (12 ± 4.7 mmHg vs. 6.4 ± 4.9 mmHg). The differences in mPAP and PCWP measurements between supine and upright RHC persisted r

The Effects of Patient Positioning on the Outcome During Posterior Cranial Fossa and Pineal Region Surgery.

Background: Surgery on posterior cranial fossa (PCF) and pineal region (PR) carries the risks of intraoperative trauma to the brainstem structures, blood loss, venous air embolism (VAE), cardiovascular instability, and other complications. Success in surgery, among other factors, depends on selecting the optimal patient position. Our objective was to find associations between patient positioning, incidence of intraoperative complications, neurological recovery, and the extent of surgery. Methods: This observational study was conducted in two medical centers: The Ohio State University Wexner Me

Perivascular axillary brachial plexus block and patient positioning: the influence of a lateral, head-down position.

The aim of this study was to examine the effect of a 20 degrees Trendelenburg position on the blockade of nerves that exit the brachial plexus proximally in patients undergoing single-injection axillary brachial plexus block. After a pilot study of eight cadavers suggested that a head-down and lateral position would encourage the proximal spread of local anaesthetic, 72 patients undergoing elective surgery were divided into two equal groups: a Supine group and a Modified Position group (lateral position, 20 degrees head-down tilt). Patients were left in the allocated position for 30 min after
