[20 years' experience using anesthesia in pediatric bronchology].

The authors inform about their experience with anesthesia gained in the last 20 years with 50,000 bronchological examinations in children. Different anesthesia methods are discussed. They conclude that local anesthesia in adults and in adolescents is a possible procedure using fiberscope. In infants and children the most preferred anesthesia is halothane-nitrous oxide-oxygen-anesthesia with relaxation by succinylcholine.

Control of intracranial pressure during pediatric neurosurgery anesthesia.

Some of the important points of view of anesthesia in pediatric neurosurgery are: adequate alveolar exchange of oxygen and carbondioxide by means of artificial ventilation; stable cardiovascular function; fluid balance; monitoring, and attempts to keep intracranial pressure low. This last point is achieved by various anesthesiological procedures, e.g., hyperventilation, positive-negative pressure ventilation, rapid induction of anesthesia and choice of suitable anesthetics. Enflurane (Ethrane) increases intracranial pressure less than halothane. The new intravenous anesthetic etomidate lowers

Advantages of Ciprofol with Special Consideration of Pediatric Anesthesia.

The search for an ideal anesthetic has always been a major goal in anesthesiology. In recent years, the introduction of ciprofol has marked a major breakthrough in the pharmacological field, following the introduction of dexmedetomidine. Ciprofol has similar characteristics to propofol but with greater hemodynamic stability. Furthermore, it overcomes one of the most common discomforts associated with propofol: pain at the injection site. These characteristics make it a suitable hypnotic for pediatric use. Although studies on children are still limited, the literature on adults is now substanti

Advantages of Remimazolam in Pediatric Anesthesia: A Narrative Review.

Remimazolam is an ultra-short-acting benzodiazepine developed according to the "soft drug" concept and characterized by rapid onset, predictable offset, organ-independent metabolism, and the availability of a specific antagonist. Due to these pharmacological features, this drug represents a particularly attractive option for pediatric anesthesia and sedation, a field in which traditional agents are often limited by hemodynamic instability, prolonged recovery, and adverse respiratory effects. This narrative review summarizes and discusses the current evidence regarding the use of remimazolam in

Neurodevelopmental Effect of General Anesthesia on the Pediatric Patient.

In this article, the authors review the animal and human data on the recent studies looking at the neurotoxicity of general anesthesia in the pediatric population. Animal studies in rodents and non-human primates demonstrate neurotoxic effects when exposed to general anesthesia at a young age. However, prospective clinical studies in humans do not show significant differences in intelligence quotient outcomes in children younger than 3 years with isolated and/or short exposures. Current studies are investigating alternatives to minimize the potential side effects, including the addition of pro

Anesthesia for diagnostic and therapeutic procedures in pediatric outpatients.

Appropriate sedation of pediatric oncology patients during painful diagnostic and therapeutic procedures is a recognized problem. At the Memorial Sloan-Kettering Cancer Center the pediatric oncologists and anesthesiologists have developed a cooperative program that has provided safe and effective sedation of outpatients. The experience with 100 consecutive outpatients is reported. Patients ranged from 3 months to 17 years of age. Procedures included lumbar puncture and/or intrathecal chemotherapy, multiple bone marrow aspirations, bone marrow biopsies, and removal of indwelling central vein ca

Adverse Events Associated with Airway Management in Pediatric Anesthesia: A Prospective, Multicenter, Observational Japan Pediatric Difficult Airway in Anesthesia (J-PEDIA) Study.

BACKGROUND: The incidence of adverse events and desaturation during airway-securing procedures (a sequence from preoxygenation to completion of tracheal intubation or supraglottic airway placement) under general anesthesia in children remains underexplored. Thus, this study investigated the incidence of adverse and desaturation events and associated risk factors. METHODS: This was a prospective, multicenter, observational study conducted between June 2022 and January 2024 in 10 tertiary care (6 pediatric and 4 university [mixed adult-pediatric]) hospitals in Japan. A standardized data collect

The effect of a pediatric heat and moisture exchanger on dead space in healthy pediatric anesthesia.

BACKGROUND: Heat and moisture exchangers (HME) are often used to maintain humidity of breathing circuits during anesthesia. It is also known to increase dead space ventilation in respiratory distress syndromes. However, the effect of a pediatric HME in healthy pediatric patients has not yet been clarified. The purpose of this study was to evaluate the effect of a pediatric HME on dead space in healthy pediatric patients during anesthesia. METHODS: 20 ASA physical class I pediatric patients, without respiratory impairment, who underwent elective surgery for inguinal hernia or hydrocele with ge

Apnoeic oxygenation in pediatric anesthesia: better safe than sorry!

BACKGROUND: Children, especially neonates and infants, are at particularly high risk of hypoxemia during induction of anesthesia. The addition of nasal apnoeic oxygenation (ApOx) during tracheal intubation should prolong safe apnoea time without desaturation and reduce the risk of hypoxemia. Despite the recommendations in the relevant European guidelines, their implementation in pediatric anesthesia in Germany is not yet known. METHODS: A survey was conducted in July and October 2024 via email to all registered members of the scientific working group on airway management, the scientific worki

Personalized pediatric anesthesia and pain management: problem-based review.

Pharmacogenetics, the genetic influence on the interpersonal variability in drug response, has enabled tailored pharmacotherapy and emerging 'personalized medicine.' Although oncology spearheaded the clinical implementation of personalized medicine, other specialties are rapidly catching up. In anesthesia, classical examples of genetically mediated idiosyncratic reactions have been long known (e.g., malignant hyperthermia and prolonged apnea after succinylcholine). The last two decades have witnessed an expanding body of pharmacogenetic evidence in anesthesia. This review highlights some of th

[Pediatric caudal anesthesia : importance and aspects of safety concerns].

Caudal block is a safe procedure commonly used for pediatric perioperative analgesia. Complications are extremely rare but nevertheless local and systemic contraindications must be excluded. Optimal safety and quality result when strict attention is paid to technical details. These are discussed in detail in this review. A local anesthetic (LA) containing epinephrine allows early detection of inadvertent intravascular LA administration; therefore an epinephrine/LA mixture is recommended at least for the test dose. In terms of safety the choice of LA itself is probably of secondary importance.

Topical Pharyngeal Anesthesia in Sedated Pediatric Patients Undergoing Esophagogastroduodenoscopy.

OBJECTIVES: Pediatric patients undergoing esophagogastroduodenoscopy (EGD) commonly receive procedural sedation for comfort and to facilitate the procedure. EGD with procedural sedation carries the risk of several airway incidents and/or adverse events (AIAE). Topical pharyngeal anesthetics (TPAs) can blunt the airway reflexes and decrease the incidence of laryngospasm but has not been well studied with EGD under procedural sedation. We aimed to study the effect of adding a TPA to propofol-based sedation on the rate of AIAE. METHODS: This is a single-center, retrospective, observational cohor

Pediatric Anesthesia Outside the Operating Room: Safety and Systems.

Anesthesia care performed outside the operating room is a growing area of pediatric anesthesia practice. The anesthesiology team expects to care for children in diverse locations, which include diagnostic and interventional radiology, gastroenterology and pulmonary endoscopy suites, radiation oncology sites, and the cardiac catheterization laboratory. To provide safe, high-quality care the anesthesiologist working in these environments must understand the unique environmental, logistical, and perioperative considerations and risks involved with each remote location. This 2-part review provides
