Pediatric PCA: the role of concurrent opioid infusions and nurse-controlled analgesia.

OBJECTIVES: We designed a clinical study to determine: a) the safety and efficacy of patient-controlled analgesia (PCA) therapy in children and adolescents undergoing major operations, b) if the use of a concurrent opioid infusion improved the efficacy of conventional PCA therapy, and c) if nurse control of the PCA device was a useful alternative in the intensive care unit (ICU) setting. DESIGN: Subjects were randomly assigned to receive morphine sulfate for postoperative pain relief via intermittent PCA boluses on demand or PCA plus a continuous infusion (PCA + CI). Children (n = 12) who wer

Type A behavior in children: what a pediatric nurse practitioner needs to know.

Type A behavior has been shown in adults to be a predictor of heart disease equal in magnitude to cigarette smoking and cholesterol level. This article reviews current thinking on Type A behavior and its relationship to cardiovascular health. Particular emphasis is given to current literature about Type A behavior in adolescents, school-aged children, and preschoolers. Discussion covers the importance of the primary subcomponents of Type A behavior (i.e., competitiveness, hostility, and impatience). Clinical cautions and applications of this research are included.

Phenomenological Study of the Lived Experiences of Nurses Managing Pediatric Postoperative Pain.

BACKGROUND: Postoperative pain management in children is a critical aspect of pediatric care. This aspect of pediatric care is important because it can lead to psychological stress and distress, impacting both immediate and future emotional well-being in children if not managed well. PURPOSE: To explore the experiences of nurses during the management of postoperative pain in the pediatric population at the Tamale Teaching Hospital. METHODS: A hermeneutic-phenomenological approach was used, with a sample size estimated at three (3) to ten(10) participants. Data collection involved interviews

Nurses and Midwives Mistakes and Errors in Pediatric Health Outcomes in Southwestern Uganda.

BACKGROUND: According to the World Health Organization, nearly 1 in 10 patients suffer harm during healthcare, resulting in over 3 million deaths in low- and middle-income countries. Alarmingly, more than half of these incidents are preventable, with medication errors being the primary cause. Children are particularly vulnerable to these errors and other adverse patient events. Therefore, this study explored nurses and midwives' mistakes and errors in paediatric health outcomes in southwestern Uganda. METHODS: We adopted a phenomenological qualitative research design. Purposive and consecutiv

Nurse-Performed Bladder Ultrasound Effect on Pediatric Bladder Catheterization Success.

BACKGROUND AND OBJECTIVES: Bladder catheterization (BC) is a routine procedure, but unsuccessful attempts due to inadequate bladder volume are common and stressful for children and caregivers. Physician-performed bladder point-of-care ultrasound (POCUS) improves BC success rates, but the effect of nurse-performed POCUS remains understudied. METHODS: We randomized children under 24 months of age to receive either nurse-performed POCUS before BC or standard blind BC in the pediatric emergency department to compare dry catheterization rates. A simplified POCUS technique using a single bladder me

Simulation for Student Registered Nurse Anesthetists: Common Pediatric Anesthesia Complications.

Student registered nurse anesthetists (SRNAs) are required by the Council on Accreditation to provide anesthetics to a minimum of 30 patients ages 2-12 years and 10 patients younger than 2 years. Pediatric anesthesia can prove to be stressful because children are at higher risk for morbidity and mortality during the perioperative period compared with adults. Simulation allows SRNAs the opportunity to review and develop skills in a safe and supportive environment. The purpose of this project was to provide a high-fidelity pediatric simulation for SRNAs prior to their pediatric rotation to impro

Spanish Validation of Pediatric Nurses Knowledge and Attitudes Survey Regarding Pain.

BACKGROUND: Although pain is considered a sign/symptom codified by ICD-10, it is not frequently found in the clinical reports of children who have suffered pain due to disease or interventions. It has a negative effect on their well-being, affecting different spheres of their life. Having questionnaires adapted and validated in different languages makes it possible to compare results between different centers and countries, as well as to adapt training to the deficits found in a given population. AIM: To validate the Spanish version of Pediatric Nurses Knowledge and Attittudes Survey Regardin

Nursing Interventions and Intracranial Pressure Change in Pediatric Patients With Severe Traumatic Brain Injury.

BACKGROUND: Nursing interventions in the care of pediatric patients with severe traumatic brain injury (TBI) can have a direct effect on intracranial pressure (ICP), yet they have been largely underexplored. Early evidence is therefore needed to describe these relationships and to determine intervention that promotes neuroprotection and recovery. OBJECTIVES: The aim of this study was to examine nursing interventions within the first 72 hours of pediatric severe TBI and their effects on ICP. METHOD: This is a retrospective review of pediatric patients admitted for severe TBI using a quasi-exp

Strategic threat management: an exploration of nursing strategies in the pediatric intensive care unit.

Part of the work of a critical care nurse is to manage the threats that arise that could impede efficient and effective job performance. Nurses manage threats by employing various strategies to keep performance high and workload manageable. We investigated strategic threat management by using the Threat-Strategy Interview. Threats frequently involved technology, staff, or organizational components. The threats were managed by a toolbox of multifaceted strategies, the most frequent of which involved staff-, treatment- (patient + technology), examination- (patient + clinician), and patient-orien

Critical Cardiopulmonary Event Series: Four Simulations for Pediatric ICU Fellows, Critical Care Nurses, and Pediatric Residents.

INTRODUCTION: Critical cardiopulmonary events arising from congenital or acquired heart diseases are infrequent in some pediatric critical care units but can be associated with significant morbidity and mortality when encountered. We developed four simulation cases for interprofessional pediatric critical care teams (fellows, residents, and nurses) to provide participants with high-acuity cardiopulmonary scenarios in safe learning environments. The included cases were coarctation of the aorta, Kawasaki disease, myocarditis, and tetralogy of Fallot. METHODS: The simulations were typically 15 m

How far from the ideals of narrative medicine are current practices in pediatric nursing? An empirical analysis of nurse-child-parent communication.

OBJECTIVE: To evaluate how nurses in three hospitals in China communicate in triadic pediatric interactions (nurses-pediatric patients-parents), data were collected and analyzed to identify the extent to which current communication patterns are adequate for the goals of narrative medicine, a patient-story-centered approach to medicine that advocates deep patient engagement. METHOD: Qualitative ethnographic observation and discourse analysis were combined for this empirical investigation. Audio recordings of 34 triadic interactions by three nurses in three hospitals across China were collected
