Surgical approaches for implantation of three-piece inflatable penile prostheses include the infrapubic and the penoscrotal. The penoscrotal approach avoids possible injury to the dorsal sensory nerves, provides easier and more complete corporeal exposure, and allows the pump to be anchored in the scrotal pouch. A variant of the penoscrotal approach, the transverse penoscrotal incision, is described.

PURPOSE: Lateral extrusion of a penile prosthesis cylinder is an uncommon but distressing problem. A technique is described to reseat the cylinder in a more medial and secure position under the glans penis by creating a new cavity for the cylinder behind the back wall of the fibrotic sheath containing it.
MATERIALS AND METHODS: A total of 14 patients underwent corporoplasty using this technique with followup as long as 2 years.
RESULTS: All patients had a satisfactory result with no recurrence of the original problem.

PURPOSE OF REVIEW: The inflatable penile prosthesis (IPP) was introduced in 1973. Since that time, the fundamental design of the IPP has not changed, but numerous improvements to the device, surgery, and peri-operative management have resulted in a modern IPP with excellent reliability, infection control, safety profile, and user experience.

BACKGROUND: Currently the Ambicor is the only 2-piece inflatable penile prosthesis (IPP) available in the United States. By eliminating the separate reservoir, this provides certain advantages for patient and surgeon. Nevertheless, it composes a small percentage of all IPPs implanted in the United States.
