Granuloma annulare with transfollicular perforation.

In recent years the phenomenon of transepithelial elimination has been found to be the underlying mechanism in a variety of skin disorders. "Perforating dermatoses" may show transepidermal or transfollicular elimination or a combination of the two. A case of papular granuloma annulare is reported, where in one lesion the pilary apparatus actively participated in the elimination of necrobiotic collagen from the dermis. To our knowledge, perforating granuloma annulare with a transfollicular pathway has not previously been described. Also, a classification of perforating dermatoses is presented.

Input/Output Relationships for the Primary Hippocampal Circuit.

The hippocampus is the most studied brain region, but little is known about signal throughput-the simplest, yet most essential of circuit operations-across its multiple stages from perforant path input to CA1 output. Using hippocampal slices derived from male mice, we have found that single-pulse lateral perforant path (LPP) stimulation produces a two-part CA1 response generated by LPP projections to CA3 ("direct path") and the dentate gyrus ("indirect path"). The latter, indirect path was far more potent in driving CA1 but did so only after a lengthy delay. Rather than operating as expected f

Anatomical and Radiological Considerations When Colonic Perforation Leads to Subcutaneous Emphysema, Pneumothoraces, Pneumomediastinum, and Mediastinal Shift.

While colonoscopy is generally regarded as a safe procedure, colonic perforation can occur and the risk of this is higher when interventional procedures are undertaken. The presentation may be acute or delayed depending on the extent of the perforation. Extracolonic gas following colonic perforation can migrate to several body compartments that are embryologically related and it has previously been reported in the thorax, mediastinum, neck, scrotum, and lower limbs. This review discusses in detail the anatomical pathways that led to a rare case of widespread subcutaneous emphysema, bilateral p

Granuloma annulare with follicular perforation.

An instance of perforating granuloma annulare was observed in a 42-year-old female patient in which the necrobiotic material in the dermis was eliminated by a hair follicle. This had previously been described by the author in only 1 other case of perforating granuloma annulare. Transepithelial elimination may occur by the transepidermal and/or transfollicular route. Both pathways are possible in perforating granuloma annulare. Some factors are discussed which may be responsible for the predominance of one route over the other in various disorders of transepithelial elimination.

Lacrimal drainage pathway disease-associated keratopathy effectively diagnosed with dacryoendoscopy: a report of two cases.

BACKGROUND: We described two cases of lacrimal drainage pathway disease-associated keratopathy (LDAK) wherein lacrimal irrigation tests revealed no abnormalities, and there were few signs and symptoms suggestive of lacrimal drainage pathway disease. CASE PRESENTATION: Case 1 involved a 66-year-old woman with Sjögren's syndrome who presented with corneal perforation in her left eye. Slit-lamp examination revealed a non-infiltrative corneal ulcer and perforation in the inferior peripheral cornea. LDAK was suspected; however, there were minimal signs of lacrimal drainage pathway disease. Despite

Lesions of the entorhinal cortex impair acquisition of hippocampal-dependent trace conditioning.

Rabbits with the electrolytic lesions of bilateral entorhinal cortex (EC) were trained with the hippocampal-dependent trace conditioning of the nictitating membrane response. The multiple-unit activity of the hippocampal CA1 region was recorded during conditioning. The conditioned stimulus was a tone (1 kHz, 85 dB, 200-ms duration), the unconditioned stimulus was a corneal air puff (3 psi, 150-ms duration), and the interstimulus interval was 750 ms. The EC-lesioned animals showed only 30% conditioned response (CR) by the ninth session while the sham-operated animals showed above 80% CR. The le

Inhibition of phosphatase and tensin homolog induces chemical long-term potentiation in the dentate gyrus of hippocampus.

Phosphatase and Tensin Homolog (PTEN) is a tumor suppressor gene that regulates cell growth, proliferation, and survival. Although the role of PTEN in cellular processes has been well-established, its involvement in synaptic plasticity is still poorly understood. This study examined the impact of bisperoxovanadium (bpV, PTEN inhibitor, 10 µM) on basal synaptic transmission and metaplasticity in the dentate gyrus of the hippocampus. A total of 48 male Wistar rats weighing between 240 and 360 g were used for all experiments. We recorded extracellular postsynaptic potentials (EPSPs) in the dentat

Endogenous opioids regulate long-term potentiation of synaptic inhibition in the dentate gyrus of rat hippocampus.

Long-term potentiation (LTP) of excitatory transmission in the hippocampus has been extensively studied as a synaptic model of learning and memory. Here we report a new form of LTP in which inhibitory synaptic signals are potentiated following tetanic stimulation of an opioid-containing excitatory pathway in the presence of opioid antagonists. The lateral perforant path (LPP) was stimulated at the dentate outer molecular layer of hippocampal slices. Evoked synaptic currents were recorded from dentate granule cells using whole-cell voltage-clamp techniques. A high-frequency stimulus train (100

Inducing a long-term potentiation in the dentate gyrus is sufficient to produce rapid antidepressant-like effects.

Recent hypotheses propose that one prerequisite to obtain a rapid antidepressant (AD) effect would reside in processes of synaptic reinforcement occurring within the dentate gyrus (DG) of the hippocampus independently from neurogenesis. However, to date no relationship has been established between an increased DG synaptic plasticity, and rapid AD-like action. To the best of our knowledge, this study shows for the first time that inducing a long-term potentiation (LTP) within the DG by stimulating the perforant pathway (PP) is sufficient to induce such effects. Thus, Sprague-Dawley rats having

Severe ischemic cytomegalovirus proctocolitis with multiple perforation.

Cytomegalovirus (CMV) typically causes gastrointestinal infections in immunocompetent patients. Colonic perforations secondary to CMV are exceeding rare. We describe a 88-year-old male presenting with a week-long history of intractable abdominal discomfort, bloating, nausea and diarrhea. Flexible sigmoidoscopy revealed significant ulceration with yellowish slough. Emergency surgery was performed subsequently in view of multiple perforations in the rectosigmoid junction. CMV gastrointestinal infections demonstrated an ischemic process secondary to vasculitis, which accelerated the pathway to co

Laparoscopic Repair of Colorectal Perforations Induced by Compressed Air Pressure: A Case Report.

A compressed air nozzle has the potential to result in lethal injuries when handled inappropriately. Owing to the rarity of colorectal perforations due to barotrauma, no clear pathway to managing them has been established. We report an incident of a 33-year-old male patient who presented with tension pneumoperitoneum due to rectosigmoid perforations after being subjected to transanal compressed air insult. An emergency laparoscopic exploration with primary repair of the rectal perforation and Hartmann procedure were performed resulting in a smooth postoperative course. We hereby conclude that

Selective disruption of synaptic NMDA receptors of the hippocampal trisynaptic circuit in Aβ pathology.

Synaptic dysfunction is an early feature in Alzheimer's disease (AD) pathogenesis and a major morphological correlate of memory deficits. Given the main synaptic location of N-methyl-D-aspartate receptors (NMDARs), their dysregulation has been implicated in these pathological effects. Here, to detect possible alterations in the expression and synaptic localisation of the GluN1 subunit in the brain of amyloidogenic APP/PS1 mice, we employed histoblot and SDS-digested freeze-fracture replica labelling (SDS-FRL) techniques. Histoblots showed that GluN1 expression was significantly reduced in the

Lentiviral Expression of Rabies Virus Glycoprotein in the Rat Hippocampus Strengthens Synaptic Plasticity.

Rabies virus (RABV) is a neurotropic virus exclusively infecting neurons in the central nervous system. RABV encodes five proteins. Among them, the viral glycoprotein (RVG) plays a key role in viral entry into neurons and rabies pathogenesis. It was shown that the nature of the C-terminus of the RABV G protein, which possesses a PDZ-binding motif (PBM), modulates the virulence of the RABV strain. The neuronal protein partners recruited by this PBM may alter host cell function. This study was conducted to investigate the effect of RVG on synaptic function in the hippocampal dentate gyrus (DG) o

Interventional management of acute perforated cholecystitis: When is percutaneous transhepatic cholecystostomy a reasonable therapeutic option?

In the case of acute perforated cholecystitis, rapid control of the infection site is important for patient outcomes. For free perforation with generalized peritonitis, emergency laparoscopic cholecystectomy remains the gold standard therapy. Percutaneous transhepatic cholecystostomy (PTC) is an alternative damage-control option, particularly for frail, septic, or profoundly comorbid patients, either as a bridge to (early or interval) cholecystectomy or, in truly inoperable patients, as definitive palliation. Gallbladder perforations are categorized on the basis of the Niemeier classification.

Calcium channel blockade attenuates abnormal synaptic transmission in the dentate gyrus elicited by entorhinal amyloidopathy.

Entorhinal-hippocampal network is one of the earliest circuits which is affected by Alzheimer's disease (AD). There are numerous data providing the evidence of synaptic deficit in the dentate gyrus (DG) of AD animal model. However, there is little known about how entorhinal cortex (EC) amyloidophaty affects each excitatory and/or inhibitory transmission in the early stage of AD. On the other hand, it is believed that calcium dyshomeostasis has a critical role in the etiology of AD. Here, the effect of the EC amyloid pathogenesis on excitatory or inhibitory post synaptic currents (EPSC and IPSC
