Clinical Application and the Free Posterior Thigh Perforator Flap.

OBJECTIVE: The posterior thigh region has been neglected as a donor site for free perforator flaps, likely due to difficulties in positioning the patient during surgery. This study describes the clinical application of the posterior thigh perforator flap based on the third perforating artery of the profunda femoris artery (PFA). METHODS: The free posterior thigh perforator flap based on the third perforating artery of the PFA was used for reconstruction of soft tissue defects in nine patients between February 2010 and May 2014. RESULTS: Flap sizes ranged from 12 × 7 cm to 20 × 13 cm. The len

Varied uses of the medial sural artery perforator flap.

BACKGROUND AND AIM: The aim of the study was to demonstrate how the medial sural artery perforator flap can be used in a wide range of settings and not focused on one area, as previous papers have done. We also wanted to demonstrate that larger flaps can be harvested from the donor site and still closed directly to optimise the appearance of the donor site. METHODS: We describe the use of the medial sural artery perforator flap in 18 patients with defects of the head and neck, upper and lower limbs. RESULTS: All 18 flaps survived, although one suffered some delayed partial loss due to pressu

Superficial circumflex iliac artery perforator flap for tongue reconstruction.

OBJECTIVES: With microvascular reconstruction, different perforator flaps have been introduced for the treatment of head and neck defects. In light of this, the superficial circumflex iliac artery perforator (SCIP) flap was assessed for reconstruction after partial and hemiglossectomy. STUDY DESIGN: A total number of nine patients who received SCIP flap reconstruction for tongue defects were included in this study. Details on clinical features were collected, and postoperative functions and esthetic results were analyzed. RESULTS: All the SCIP flaps survived, and postoperative speech and swa

Quadrilobed superior gluteal artery perforator flap for sacrococcygeal defects.

BACKGROUND: Perforator flaps are used extensively in repairing soft tissue defects. Superior gluteal artery perforator flaps are used for repairing sacral defects, but the tension required for direct closure of the donor area after harvesting of relatively large flaps carries a risk of postoperative dehiscence. This research was to investigate a modified superior gluteal artery perforator flap for repairing sacrococcygeal soft tissue defects. METHODS: From June 2003 to April 2010, we used our newly designed superior gluteal artery perforator flap for repair of sacrococcygeal soft tissue defec

Thoracoacromial artery perforator flap: anatomical basis and clinical applications.

BACKGROUND: Concerns and criticism have been reported on the feasibility and vascular reliability of the thoracoacromial artery perforator flap because of the inconsistent size of the pectoral branch of the thoracoacromial artery and its cutaneous perforators. The authors have investigated its vascular basis through a cadaver dissection and described their clinical experience in the surgical strategy, range of application, and indications for the thoracoacromial artery perforator flap. METHODS: Twelve fresh human cadavers were dissected to define the anatomy of the branches of the thoracoacro

Three-dimensional angiography of the submental artery perforator flap.

BACKGROUND: The submental flap, based on a large branch of the facial artery, is an excellent flap option which is most commonly used for head and neck reconstruction. The purpose of this report is to clarify aspects of the anatomy of the submental flap in order to improve the utility of this flap. METHODS: Ten cadavers were injected with a modified lead oxide-gelatin mixture. Four cadavers were selected for three-dimensional reconstruction using a spiral computed tomography scanner and specialised volume-rendering software. Dissection, angiography and photography of each layer were performed

Perineal reconstruction using a bilobed pudendal artery perforator flap.

BACKGROUND: To reconstruct the perineal region, including the urogenital and anal triangle that differ from each other in tissue characteristics and function, we applied pudendal artery perforator flaps with a bilobed flap design. This bilobed flap design could improve the arc of flap rotation and the mobility of the flap so it could cover wide and deep defects. Moreover, it could preserve the characteristics of each triangle. METHODS: This study enrolled a total of 15 female patients who had undergone perineal reconstruction with pudendal artery perforator flaps. Seven of them had vulva canc

Customized reconstruction with the free anterolateral thigh perforator flap.

From April of 2003 through September of 2006, 70 free anterolateral thigh (ALT) flaps were transferred for reconstructing soft-tissue defects. The overall success rate was 96%. Among 70 free ALT flaps, 11 were elevated as cutaneous ALT septocutaneous vessel flaps. Fifty-seven were harvested as cutaneous ALT myocutaneous "true" perforator flaps. Two flaps were used as fasciocutaneous perforator flaps based on independent skin vessels. Fifty-four ALT flaps were used for lower extremity reconstruction, 11 flaps were used for upper extremity reconstruction, 3 flaps were used for trunk reconstructi

[Combined latissimus dorsi-thoracodorsal artery perforator-transpositional free flap].

BACKGROUND: Reconstruction or complete cover of extended but polygonal defects is limited by the size of transplantable tissue. One of the largest composite tissue components to be transplanted is the myocutaneous latissimus dorsi flap. Under certain circumstances even this large-scale flap is not sufficient for complete defect cover. Based on experiences with the thoracodorsal artery perforator (TAP) flap, the skin island adjacent to the latissimus muscle may be raised, pedicled on the perforator vessels penetrating the underlying muscle. Thus this island may easily be transposed or rotated t

Reconstruction of axillary contractures with thoracodorsal perforator island flap.

The axilla is one of the most frequently sites affected by contractures after severe burns. These contractures often cause cosmetic problems and functional deficiency. A variety of therapeutic methods such as skin grafting, Z-plasties, local flaps, island flaps, and free flaps, have been reported for treatment of the contractures. Each has its own advantages and drawbacks. In this clinical study we report 15 cases of post burn axillary contractures treated with thoracodorsal perforator-based cutaneous flaps. The flaps were harvested in range of sizes up to as large as 27 cmx15 cm. All flaps su

Sequential chimeric deep circumflex iliac artery perforator flap and flow-through anterolateral thigh perforator flap for one-stage reconstruction of complex tissue defects.

BACKGROUND: Simultaneous reconstruction of composite bone and large soft-tissue defect areas is challenging for reconstructive surgeons. This study presents a microsurgical sequential chimeric flap composed of a flow-through anterolateral thigh perforator (ALTP) flap and a deep circumflex iliac artery perforator (DCIAP) chimeric flap for one-stage reconstruction of a complex tissue defect in limbs. METHODS: A retrospective analysis was performed among patients who underwent reconstruction of bone composite and large soft-tissue defect areas in limbs using a sequential chimeric flap. A flow-th

Moving forwards: the anterior supraclavicular artery perforator (a-SAP) flap: a new pedicled or free perforator flap based on the anterior supraclavicular vessels.

Since the original publication in 1997 the supraclavicular island flap (SIF) has evolved and is now a safe and reliable procedure for head and neck reconstruction. The purpose of this communication is to give an overview of the current indications and outcomes. An analysis of the literature shows documented use in over 400 patients. Flap size is around 10 × 30 cm; in the case of pre-expansion the width can be up to 15 cm. In larger case series the major complication rate is <5%. We have transferred 127 flaps in 79 patients. To unify the widely varying nomenclature and in the light of the new c

Investigation of flow velocity in recipient perforator artery for a reliable indicator for the flap transfer with perforator to perforator anastomosis.

INTRODUCTION: In flap transfer with perforator to perforator anastomosis (FTPPA), encountering poor pulsation and blood flow in a recipient perforator intraoperatively often makes FTPPA impossible. This study sought to identify color and spectral Doppler ultrasonography (CSDUS) parameters that can aid reliable preoperative selection of a recipient perforator artery. PATIENTS AND METHODS: The study enrolled 38 patients with lower extremity lymphedema who underwent vascularized lymphatic tissue transfer with perforator to perforator anastomosis for physiological lymphatic flow reconstruction. Reliable harvesting of a large thoracodorsal artery perforator flap with emphasis on perforator number and spacing.

BACKGROUND: Thoracodorsal artery perforator flaps are useful for covering extensive defects because of their generous donor-site dimensions. However, large flaps increase the risk of partial flap necrosis, and a reliable means of sizing thoracodorsal artery perforator flaps has not been devised. The authors reviewed a series of large thoracodorsal artery perforator flap transfers performed under various reconstructive conditions to evaluate surgical outcomes and placed emphasis on flap design and harvesting technique. METHODS: A consecutive series of large thoracodorsal artery perforator flap
