Magnetic resonance imaging of normal and abnormal brain perfusion.

The brain is highly susceptible to injury from any process that deprives its cells of oxygen, glucose, and other essential nutrients, and is therefore critically dependent on blood flow or perfusion. With hypoperfusion or ischemia, blood flow to the brain may be diminished to a level incompatible with normal function, eventually leading to acute ischemic stroke. Magnetic resonance (MR) imaging is one of several imaging methods used to evaluate brain perfusion and stroke. Using a multisequence approach comprised of conventional spin echo, inversion recovery, and heme-sensitive gradient echo pul

Perfusion lung imaging in the adult respiratory distress syndrome.

In 29 perfusion lung scans (PLS) of 19 patients with ARDS, 20 of which were obtained within six days from the onset of respiratory symptoms, perfusion abnormalities were the rule. These included focal, nonsegmental defects, mostly peripheral and dorsal, and perfusion redistribution away from the dependent lung zones. PLS were scored for the presence and intensity of perfusion abnormalities and the scores of perfusion redistribution were validated against numerical indices of blood flow distribution per unit lung volume. PLS scores were correlated with arterial blood gas values, hemodynamic par

Myocardial perfusion imaging with 99m-Tc or 113m-In macroaggregated albumin: correlation of the perfusion image with clinical, angiographic, surgical, and histologic findings.

Scintillation camera myocardial perfusion images were performed in 77 patients with proved or suspected ischemic heart disease following the intracoronary injection of 1.5 mCi 99m-Tc or 113m-In macroaggregated albumin. Perfusion images were classified as normal (36) or abnormal (41), and the location of abnormality was noted. Thirty-seven out of 41 patients with abnormal images had prior myocardial infarction based on history (30), ECG Q-waves (27), local contraction pattern abnormality (23), or direct surgical (9) or histologic (4) inspection, either singly or in combination. Three out of fiv

Role of Perfusion-Weighted Imaging in a Diffusion-Weighted-Imaging-Negative Transient Ischemic Attack.

BACKGROUND AND PURPOSE: The absence of acute ischemic lesions in diffusion-weighted imaging (DWI) in transient ischemic attack (TIA) patients makes it difficult to diagnose the true vascular etiologies. Among patients with DWI-negative TIA, we investigated whether the presence of a perfusion-weighted imaging (PWI) abnormality implied a true vascular event by identifying new acute ischemic lesions in follow-up magnetic resonance imaging (MRI) in areas corresponding to the initial PWI abnormality. METHODS: The included patients underwent DWI and PWI within 72 hours of TIA and also follow-up DWI

Asymmetrical cerebral perfusion demonstrated by noninvasive arterial spin-labeling perfusion imaging in a patient with corticobasal degeneration.

This is to report arterial spin-labeling (ASL) regional cerebral blood flow (rCBF) mapping findings in a case of corticobasal degeneration (CBD). A 68-year-old man gradually developed limb-kinetic apraxia, myoclonus, and rigidity in the left hand and was diagnosed as having CBD. Magnetic resonance imaging and single photon emission computed tomography revealed atrophy and decreased blood flow, respectively, in the right hemisphere, findings that were compatible with CBD. In addition, ASL rCBF mapping demonstrated asymmetrical hypoperfusion areas in locations typical for CBD, proving its potent

The diagnostic and prognostic value of near-normal perfusion or borderline ischemia on stress myocardial perfusion imaging.

BACKGROUND: Data on the diagnostic and prognostic value of subtle abnormalities on myocardial perfusion imaging (MPI) are limited. METHODS AND RESULTS: In a retrospective single-center cohort of patients who underwent regadenoson SPECT-MPI, near-normal MPI was defined as normal left ventricular ejection fraction (LVEF ≥ 50%) and a summed stress score (SSS) of 1-3 vs SSS = 0 in normal MPI. Borderline ischemia was defined as normal LVEF, SSS = 1-3, and a summed difference score (SDS) of 1 vs SDS = 0 in the absence of ischemia. Coronary angiography data within 6 months from MPI were tabulated. P

Serial investigation of perfusion disturbances and vasogenic oedema in hypertensive encephalopathy by diffusion and perfusion weighted imaging.

Serial MRI including diffusion and perfusion imaging was performed in a patient with hypertensive encephalopathy. At admission, the patient was disorientated and presented with seizures and cortical blindness. Perfusion imaging showed a marked reduction in blood volume and flow, with corresponding vasogenic oedema in the occipital, posterior temporal, and, to a lesser extent, frontal lobes. The clinical symptoms disappeared rapidly following treatment, whereas the disturbed circulation pattern and vasogenic oedema resolved more slowly. A complete normalisation was seen after 1 year.

[Utility of digital perfusion images of the lung in mitral stenosis: the pathophysiological significance of the distribution of pulmonary perfusion].

The relationship between pulmonary hemodynamics (cardiac index; CI and the mean pulmonary artery wedge pressure; mPw) and the redistribution of pulmonary perfusion were studied in 300 patients with mitral stenosis using computerized Tc99m-MAA perfusion images (digital perfusion images: DPI), which consisted of isocount areas. Various types of DPI were obtained and were classified in six grades according to patterns of 100-70% of the isocount area in the right anterior DPI, i.e. grade-0 (normal), grade-1 (increased perfusion in the upper zone), grade-2 (uniform distribution), grade-3 (disappear

Deep Learning-Based Image Registration in Dynamic Myocardial Perfusion CT Imaging.

Registration of dynamic CT image sequences is a crucial preprocessing step for clinical evaluation of multiple physiological determinants in the heart such as global and regional myocardial perfusion. In this work, we present a deformable deep learning-based image registration method for quantitative myocardial perfusion CT examinations, which in contrast to previous approaches, takes into account some unique challenges such as low image quality with less accurate anatomical landmarks, dynamic changes of contrast agent concentration in the heart chambers and tissue, and misalignment caused by

Perfusion MR imaging of an intracranial collision tumor confirmed by image-guided biopsy.

We present a patient with a new intracranial mass lesion that was initially interpreted as a metastasis on conventional anatomic MR imaging. On dynamic, contrast-enhanced, susceptibility-weighted perfusion MR imaging, however, there were regional hemodynamic differences within the lesion. Image-guided open biopsy targeting these regions uncovered a collision tumor between a typical meningioma and a metastatic breast carcinoma. In cases where conventional anatomic MR imaging is ambiguous, physiology-based neuroimaging methods provide complementary physiologic information useful for discriminati

Beyond perfusion: a review of peptide radiopharmaceuticals for cardiovascular imaging.

Cardiology is continually evolving towards increased personalization with targeted diagnostics and therapeutics. Peptide-based radiopharmaceuticals have emerged as a valuable tool for noninvasive, receptor-specific imaging, addressing limitations of traditional perfusion-based radiotracers like [15O]H2O, [13N]NH3, [82Rb]RbCl and [99mTc]Tc-Sestamibi, which lack molecular specificity. While these conventional tracers provide crucial insights into myocardial perfusion and ventricular function, receptor-targeted imaging can illuminate the molecular mechanisms underlying cardiovascular diseases. Rapid Decomposition of 99mTc-MAA Complex During Lung Perfusion Imaging.

A 30-year-old woman with a recent episode of dyspnea was presented. The lung perfusion scan using Tc-MAA (macro-aggregated albumin) initially revealed an acceptable lung-to-background activity ratio implying a proper radiopharmaceutical preparation and radiolabeling efficiency. Unexpectedly, later during the scan, rapidly increasing concentration of activity was observed in salivary glands, thyroid and stomach. Rapid breakdown of Tc-MAA complexes was considered as a likely explanation for the increase in the circulatory level of the free pertechnetate.

Lung Perfusion Imaging in Tetralogy of Fallot: A Case Report.

Congenital heart diseases, such as tetralogy of fallot (TOF), are the most common human birth defects that may cause pulmonary diseases. Lung perfusion scintigraphy (LPS) has an important role in evaluating pulmonary involvement in patients with these defects, both as part of the diagnostic work-up and for follow-up to guide best therapeutic strategy. Herein, we report a 10-year-old female patient with TOF who underwent LPS two years after cardiac surgery. The scan showed hypoperfusion of the left respect to the right lung and abnormal uptake of Tc-99m-macroaggregated albumin in the kidneys an

Uncertainty of Myocardial Perfusion Imaging in Chest Pain Risk Stratification.

BACKGROUND: Chest pain is a common presenting symptom in the emergency department (ED). Although the diagnostic workup for chest pain is well established, the best time to perform invasive cardiac catheterization in patients with low to moderate risk of coronary artery disease is still unclear, particularly if noninvasive tests such as the electrocardiogram (ECG) and nuclear myocardial perfusion scan show nonsignificant findings. CASE REPORT: We present the case of a 52-year-old female who presented to the ED with acute-onset chest pain that had started early in the morning while she was slee
