Oximetry-derived perfusion index as an early indicator of CT-guided thoracic sympathetic blockade in palmar hyperhidrosis.

AIM: To test the hypothesis that the oximetry-derived perfusion index (PI) recorded in the index finger may provide earlier objective evidence for correct positioning of the needle tip during computed tomography (CT)-guided thoracic sympathetic blockade than skin temperature in palmar hyperhidrosis. MATERIALS AND METHODS: Forty-four CT-guided thoracic sympathetic blockades were prospectively performed in both hands of 22 patients. Prior to chemical blockade, PI and skin temperature were recorded at 1 min intervals until 20 min after lidocaine injection. Repeated measures analysis of variance

Single SPECT measures of cerebral cortical perfusion reflect time-index estimation of dementia severity in Alzheimer's disease.

UNLABELLED: To determine the relationship between cerebral cortical blood flow loss and the temporal development of the dementia in Alzheimer's disease (AD), SPECT was studied in a cross section of AD patients with a broad range of impairment. METHODS: Thirty patients with a diagnosis of probable AD had their mini-mental state examination scores transformed into time-index values to give an estimation of dementia severity relative to the developmental time course. SPECT images were obtained using 99mTc-ethyl cysteinate dimer and a 3-head camera. Cortical surface perfusion was analyzed, includ

Relationship Between Pulse Oximetry Perfusion Index and Anesthetic Brachial Plexus Block for Upper Limb Surgery in Adults.

Introduction Brachial plexus block is widely used as a regional anesthetic technique in upper extremity surgery. However, its routine assessment using clinical criteria, such as sensation and motor function, is limited by the subjective nature of the assessment and the dependence on the patient. The perfusion index, obtained by pulse oximetry, could be an objective, early method of predicting block success. Objective To establish the relationship between the increase in perfusion index and the success of anesthetic brachial plexus block in adult patients undergoing upper extremity surgery. Met

Perfusion Collateral Index versus Hypoperfusion Intensity Ratio in Assessment of Collaterals in Patients with Acute Ischemic Stroke.

BACKGROUND AND PURPOSE: Perfusion-based collateral indices such as the perfusion collateral index and the hypoperfusion intensity ratio have shown promise in the assessment of collaterals in patients with acute ischemic stroke. We aimed to compare the diagnostic performance of the perfusion collateral index and the hypoperfusion intensity ratio in collateral assessment compared with angiographic collaterals and outcome measures, including final infarct volume, infarct growth, and functional independence. MATERIALS AND METHODS: Consecutive patients with acute ischemic stroke with anterior circ

The oxygen reactivity index indicates disturbed local perfusion regulation after aneurysmal subarachnoid hemorrhage: an observational cohort study.

BACKGROUND: Cerebral autoregulation (CA) can be impaired in patients with delayed cerebral ischemia (DCI) after aneurysmal subarachnoid hemorrhage (aSAH). The Pressure Reactivity Index (PRx, correlation of blood pressure and intracranial pressure) and Oxygen Reactivity Index (ORx, correlation of cerebral perfusion pressure and brain tissue oxygenation, PbtO2) are both believed to estimate CA. We hypothesized that CA could be poorer in hypoperfused territories during DCI and that ORx and PRx may not be equally effective in detecting such local variances. METHODS: ORx and PRx were compared dail

Low peripheral perfusion index values may indicate the need for antivenom in the early phase of scorpion envenomation.

BACKGROUND: Scorpion envenomation may cause local symptoms as well as life-threatening situations including shock, and death. Antivenom administration is the cornerstone of therapy, meticulous patient selection is needed because of the possible side effects of the antivenom including anaphylaxis. Peripheral perfusion index (PPI) is a simple, non-invasive test that is widely used for the detection of peripheral hypoperfusion. We aimed to investigate the value of using PPI to predict the need for antivenom administration for scorpion envenomation. METHODS: This prospective observational study w

Baseline Perfusion Index as a Predictor of Post-spinal Hypotension in Elective Caesarean Section: A Prospective Observational Study.

Introduction Spinal anaesthesia is the preferred technique for cesarean delivery, but it is frequently associated with hypotension, leading to adverse maternal and fetal outcomes. Early prediction remains a clinical challenge. Perfusion index (PI), a non-invasive marker of peripheral perfusion, has emerged as a potential predictor. Methodology This prospective observational study included 100 parturients undergoing elective lower segment caesarean section under spinal anaesthesia. Baseline PI was recorded preoperatively, and patients were categorized into two groups: PI ≤3.5 and PI >3.5. Hypot

The ability of pulse oximetry-derived peripheral perfusion index to detect fluid responsiveness in patients with septic shock.

BACKGROUND: Fluid challenge test is a widely used method for the detection of fluid responsiveness in acute circulatory failure. However, detection of the patient's response to the fluid challenge requires monitoring of cardiac output which is not feasible in many settings. We investigated whether the changes in the pulse oximetry-derived peripheral perfusion index (PPI), as a non-invasive surrogate of cardiac output, can detect fluid responsiveness using the fluid challenge test or not. METHODS: We prospectively enrolled 58 patients with septic shock on norepinephrine infusion. Fluid challen

The changes of non-invasive hemoglobin and perfusion index of Pulse CO-Oximetry during induction of general anesthesia.

BACKGROUND: We hypothesized that induction of general anesthesia using sevoflurane improves the accuracy of non-invasive hemoglobin (SpHb) measurement of Masimo Radical-7® Pulse CO-Oximetry by inducing peripheral vasodilation and increasing the perfusion index (PI). The aim of this study is to investigate the change in the SpHb and the PI measured by Rad7 during induction of general anesthesia using sevoflurane. METHODS: The laboratory hemoglobin (Hblab) was measured before surgery by venous blood sampling. The SpHb and the PI was measured twice; before and after the induction of general anes

Delayed contrast enhancement and perfusable tissue index in hypertrophic cardiomyopathy: comparison between cardiac MRI and PET.

UNLABELLED: Delayed contrast enhancement (DCE) visualized by cardiac MRI (CMR) is a common feature in patients with hypertrophic cardiomyopathy (HCM), presumed to be related to myocardial fibrosis. The pathophysiologic basis of hyperenhancement in this patient group, however, remains unclear as limited histologic comparisons are available. The present study compares the perfusable tissue index (PTI), an alternative marker of myocardial fibrosis obtained by PET, with DCE-CMR in HCM. METHODS: Twenty-one patients with asymmetric septal HCM, 12 chronic myocardial infarction (MI) patients, and 6 a

A marked decrease in bispectral index with elevation of suppression ratio by cervical haematoma reducing cerebral perfusion pressure.

A 77-year-old man with a ruptured abdominal aortic aneurysm undergoing aneurysmectomy were anaesthetised with ketamine under bispectral index (BIS) monitoring, which is a clinical EEG monitor for measurement of depth of anaesthesia/sedation. First marked BIS reduction with elevation of suppression ratio (SR) was observed following severe hypotension by deflation of the aortic occlusion balloon. The re-inflation and rapid blood transfusion improved haemodyanamics and BIS and SR. At second marked BIS reduction with SR elevation, a heavy cervical swelling due to a massive subcutaneous haematoma a
