Non-surgical treatment of maxillary lateral incisor double dens invaginatus type Ⅲ with apical periodontitis.

Dental invagination is an abnormality of the crown or root development induced during tooth germ development when the enamel-forming apparatus or epithelial root sheath overpopulates and folds into the papilla. In severe cases, the invaginated channels are connected to the pulp and periodontal tissues, often causing endodontic and periapical diseases. The complex anatomical pattern of this disease adds difficulty in its preoperative diagnosis and clinical operation. In this paper, we report a case of non-surgical treatment assisted by cone beam CT and microscopy for maxillary lateral incisor d

[The influence of systemic factors on the prognosis of apical periodontitis].

Apical periodontitis (AP) is an inflammatory disease that occurs in the periapical tissue. The treatments of AP mainly include root canal therapy and endodontic surgery which promote the repair of periapical bone tissue by infections clearing and controlling inside or outside the root canal. The evaluation on efficacies of root canal therapy and endodontic surgery is mainly based on clinical and periapical imaging examinations, and the prognosis relates to multiple factors. The systemic factors of patients can directly or indirectly affect the healing of apical periodontitis. The present revie

Assessing the Accuracy of Oral Examination and Radiology in the Diagnosis of Pathological Dental Changes in Alpacas (Vicugna pacos).

As alpacas (Vicugna pacos) gain popularity globally, the need for improved knowledge about the species grows, including a better understanding of dental pathologies increasingly recognized in domesticated populations. Oral examination and radiology are commonly used tools for the detection of periapical infections and other dental diseases. In this diagnostic accuracy study, we aim to calculate the sensitivity and specificity of these two modalities to determine their agreement with histology in the detection of periodontal (periapical and periradicular) and pulpal pathologies. Seven alpacas w

Midkine expression in human periapical granulomas.

INTRODUCTION: The expression of midkine (MK), a heparin-binding growth factor, is increased in various human tumors, making it a promising tumor marker and target for tumor therapy. MK is also related to the regulation of the development and etiology of chronic or autoimmune diseases; however, the involvement of MK in apical periodontitis has never been examined. This study compared the localization of MK-expressing cells and MK messenger RNA expression in periapical granulomas with healthy gingival tissues. METHODS: Periapical lesions were removed surgically from chronic apical periodontitis

Periapical actinomycosis: a review.

Actinomycosis has increasingly been recognized as a cause of persistent or recurrent periapical disease associated with endodontically treated teeth. This case report shows the classic clinical picture of periapical actinomycosis: persistent periapical disease with recurrent sinus tracts. Although there was no pain or swelling after clinically acceptable initial endodontic treatment, a periapical lesion developed. After retreatment, the periapical lesion persisted, and a sinus tract developed. The sinus tract healed with antibiotic therapy but recurred within a few months. This cycle of sinus

Endodontic medicine: connections between apical periodontitis and systemic diseases.

The prevalence of apical periodontitis (AP) in Europe has been reported to affect 61% of individuals and 14% of teeth, and increase with age. Likewise, the prevalence of root canal treatment (RCT) in Europe is estimated to be around 30-50% of individuals and 2-9% of teeth with radiographic evidence of chronic persistent AP in 30-65% of root filled teeth (RFT). AP is not only a local phenomenon and for some time the medical and dental scientific community have analysed the possible connection between apical periodontits and systemic health. Endodontic medicine has developed, with increasing num

Relationship between prostaglandin E2 concentrations in periapical exudates from root canals and clinical findings of periapical periodontitis.

In this study the relationship of prostaglandin E2 (PGE2) concentrations in periapical exudates with clinical findings of teeth with periapical periodontitis is discussed. Periapical exudate samples were obtained from root canals of 77 endodontically involved teeth during routine root canal treatment by the quantitative sampling method using paper points. PGE2 concentrations in periapical exudates (PE-PGE2) were determined by radioimmunoassay. Significantly higher levels of PGE2 were found in periapical exudates from teeth with radiolucent areas than from teeth without radiolucent areas (236.8

[Analysis and management of the influence of periapical lesion size on endodontic microsurgery].

Endodontic microsurgery is a critical treatment option that is often considered when conventional root canal therapy fails or when complex periapical diseases cannot be resolved through non-surgical treatment. The clinical outcomes of endodontic microsurgery are influenced by multiple factors. Among these, the size of the periapical lesion is considered a key variable that can significantly impact surgical design, operative difficulty, postoperative infection control, and tissue healing. This article systematically discusses the mechanisms by which the size of periapical lesions affects the ef

The Dilemma Of Management Of Cystic Lesions; An Uncertain Way Forward: A Case Report.

Periapical diseases ranges from mild granulomatous lesions to large cystic ones, with the treatments corresponding to their respective pre-operative diagnoses. However, the determination of cause of periapical radiolucency is impossible on pre-operative clinical and radiographic examinations. We present a case highlighting the difficulties encountered in treating a periapical cyst using the current evidence in literature. It demonstrates the uncertainty involved in treating such lesions, owing to the impossible nature of determining the histopathological nature of the cyst, i.e., being either

Intraosseous injection of clindamycin phosphate into the chronic apical lesion of lower molar--a case report.

Periapical disease is the result of bacteria, their product, and the host response to them. Early histological studies of diseased periapical tissue have not been able to demonstrate viable bacteria in the lesions studied. Recent reports indicate that many of periapical lesions are indeed infected before and after endodontic treatment. The validity and applicability of the microbial delivery overcome many disadvantages that we see with systemic drugs. In this case report we presented a novel approach of managing chronic diffuse periapical lesion of lower molar based on specific selection of in

Distribution of mast cells and macrophages and expression of interleukin-6 in periapical cysts.

INTRODUCTION: Mast cells and macrophages are important components of the inflammatory infiltrate found in inflammatory periapical diseases. Several cytokines participate in the mechanisms of inflammation, tissue repair, and bone resorption associated with periapical cysts. The aim of the present study was to evaluate the distribution of mast cells and macrophages and the expression of interleukin-6 (IL-6) in periapical cysts. METHODS: Thirty periapical cysts were selected for the study, and clinical, demographic, and gross information from the cases was obtained from the laboratory records. F

Exploring the Influence of Genetic Single-Nucleotide Polymorphism (SNPs) on Endodontic Pathologies: A Comprehensive Review.

A considerable portion of the global population is affected by pulpitis and periapical lesions. While the impact of infections caused by various microbes and host effector molecules in pulpal and periapical diseases is widely recognized, disease susceptibility and progression are also influenced by the dynamic interaction between host genetic factors and environmental influences. Apical periodontitis occurs as an inflammatory response to microorganisms present in the root canals of infected teeth. Initially functioning as the body's defense mechanism, this response often progresses to chronic

Macrophages in periapical lesions: Potential roles and future directions.

Periapical lesions are infectious diseases that occur in the apical region of teeth. They result in the destruction of alveolar bone and are usually accompanied by swelling, pain, and possible systemic impacts. A complex interaction between pathogens and the host immune system determines the development, progression, and outcome of periapical lesions. The lesions, if not treated promptly, may cause resorption of bone tissue, destruction of the periodontal ligament, and loss of the affected teeth, all of which can severely worsen the quality of life of patients, often at considerable economic c

The presence of autophagy in human periapical lesions.

INTRODUCTION: Autophagy, a lysosome- or endosome-mediated self-degradation process, participates in diverse neurodegenerative diseases, cancer, and inflammatory diseases associated with apoptosis. This study aims to identify the presence of autophagy in human periapical lesions and its possible colocalization with apoptosis. METHODS: Forty-seven samples of human periapical lesions diagnosed as radicular cysts (RCs, n = 23) or periapical granulomas (PG, n = 24) were examined. The inflammatory infiltrates of lesions were evaluated by hematoxylin-eosin staining, and the presence of autophagy was

Higher expression of galectin-3 and galectin-9 in periapical granulomas than in radicular cysts and an increased toll-like receptor-2 and toll-like receptor-4 expression are associated with reactivation of periapical inflammation.

INTRODUCTION: Cysts and periapical granulomas are inflammatory reactions that develop in response to periapical infection by microbial species in dental root canal. It is known that toll-like receptors (TLRs) are pathogen recognition molecules and that galectins are lectins that can be associated with the inflammatory process, stimulating or inhibiting the immune system. The objective of this study was to evaluate the in situ expression of TLRs and galectins in radicular cysts and periapical granulomas. METHODS: We analyzed 62 cases (30 radicular cysts, 27 periapical granulomas, and 5 control

Silencing of Ac45 Simultaneously Inhibits Osteoclast-Mediated Bone Resorption and Attenuates Dendritic Cell-Mediated Inflammation through Impairing Acidification and Cathepsin K Secretion.

Endodontic disease is characterized by inflammation and destruction of periapical tissues, leading to severe bone resorption and tooth loss. ATP6AP1 (Ac45) has been implicated in human immune diseases, yet the mechanism underlying how Ac45 regulates immune response and reaction in inflammatory diseases remains unknown. We generated endodontic disease mice through bacterial infection as an inflammatory disease model and used adeno-associated virus (AAV)-mediated Ac45 RNA interference knockdown to study the function of Ac45 in periapical inflammation and bone resorption. We demonstrated that the

Assessment of maxillary posterior teeth pathologies and its association with maxillary sinus alterations-A Cone Beam Computed Tomography study.

BACKGROUND: Due to the fact that the roots of the maxillary posterior teeth are relatively close to the sinus floor, maxillary sinusitis is frequently caused by odontogenic infections. Mucosal thickness may result from the periapical pathology spreading to the sinuses. This study uses cone-beam computed tomography (CBCT) images to assess the relationship between maxillary sinus mucosal alterations and periapical diseases of maxillary posterior teeth. MATERIAL AND METHODS: CBCT images were used in this study to assess 302 maxillary posterior teeth with periapical pathologies. Changes in the si
