Imaging of extraradicular biofilm using combined scanning electron microscopy and stereomicroscopy.

Microorganisms are able to survive and induce persistent infection in extraradicular areas. The objective of this study was to evaluate the relationship between extraradicular biofilm and persistent periapical periodontitis. Thirty-five apical samples with different stages of pulp and periapical pathology (vital pulp, pulp necrosis without radiographically visible periapical lesions, chronic periapical periodontitis that had not received root canal therapy and persistent periapical periodontitis) were initially evaluated using scanning electron microscopy. The same samples were then processed

Circadian rhythm disruption aggravates alveolar bone loss in rat apical periodontitis.

BACKGROUND: Circadian rhythm disruption (CRD) affects the expression levels of a range of biological clock genes, such as brain and muscle ARNT-Like-1 (BMAL1), which is considered to be an important factor in triggering or exacerbating inflammatory response. However, the underlying effect of CRD on the pathogenesis of apical periodontitis, a common oral inflammatory disease, currently remains unknown. Exploring the effects and pathogenic mechanisms of CRD on apical periodontitis will be beneficial in providing new ideas for the prevention and treatment of apical periodontitis. METHODOLOGY: Th

The post-endodontic periapical lesion: histologic and etiopathogenic aspects.

Apical periodontitis is produced in the majority of cases by intraradicular infection. Treatment consists in the elimination of the infectious agents by endodontia. Even when carrying out a correct cleansing and filling of canals, it is possible that periapical periodontitis will persist in the form of an asymptomatic radiolucency, giving rise to the post-endodontic periapical lesion. The chronic inflammatory periapical lesion is the most common pathology found in relation to alveolar bone of the jaw. From the histological point of view, it can be classified as chronic periapical periodontitis

Nlrp3 inflammasome drives regulatory T cell depletion to accelerate periapical bone erosion.

AIM: Apical periodontitis is an inflammatory disorder triggered by an immune response to bacterial infection, leading to the periapical tissue damage and alveolar resorption. However, the underlying mechanisms driving this process remain elusive, due to the complex and interconnected immune microenvironment within the local lesion site. In this study, the influence of Nlrp3 inflammasome-mediated immune response on the apical periodontitis was investigated. METHODOLOGY: RNA sequencing, immunohistochemistry and ELISA assay were performed to investigate the activation of Nlrp3 inflammasome signa

Clinical Differential Diagnosis between Nonodontogenic and Endodontic Radiolucent Lesions in Periapical Location: A Critical Review.

In endodontics, accurate diagnoses are important for the selection of appropriate and successful therapy. Several nonendodontic entities in periapical location may resemble those of inflammatory endodontic origin and impact therapeutic approaches. The aim of this study was to review noninflammatory entities mimicking dentoalveolar abscesses or apical periodontitis and to discuss clinical and pathological features. In this review study, the authenticated search engine in PubMed (MEDLINE) database was used to find articles by using "Nonvital Pulp Dentoalveolar Abscess", "Nonvital Pulp And Apical

Immunometabolic, Vascular, and Epigenetic Determinants of Periapical Healing in Diabetes: A Narrative Review.

The bidirectional relationship between diabetes mellitus and apical periodontitis is a central focus within endodontic medicine. While clinical data consistently show a higher prevalence of apical periodontitis and delayed periapical healing in diabetic patients, the mechanisms driving this association remain to be fully clarified. This review aims to synthesize the current evidence to elucidate the specific cellular and molecular pathways through which uncontrolled hyperglycaemia may compromise periapical healing. A narrative review of current literature was conducted, integrating evidence fr

On the causes of persistent apical periodontitis: a review.

Apical periodontitis is a chronic inflammatory disorder of periradicular tissues caused by aetiological agents of endodontic origin. Persistent apical periodontitis occurs when root canal treatment of apical periodontitis has not adequately eliminated intraradicular infection. Problems that lead to persistent apical periodontitis include: inadequate aseptic control, poor access cavity design, missed canals, inadequate instrumentation, debridement and leaking temporary or permanent restorations. Even when the most stringent procedures are followed, apical periodontitis may still persist as asym

[The influence of systemic factors on the prognosis of apical periodontitis].

Apical periodontitis (AP) is an inflammatory disease that occurs in the periapical tissue. The treatments of AP mainly include root canal therapy and endodontic surgery which promote the repair of periapical bone tissue by infections clearing and controlling inside or outside the root canal. The evaluation on efficacies of root canal therapy and endodontic surgery is mainly based on clinical and periapical imaging examinations, and the prognosis relates to multiple factors. The systemic factors of patients can directly or indirectly affect the healing of apical periodontitis. The present revie
