Extracardiac Mass After Pericardiectomy: Mediastinal Fat Replacement Secondary to Progressive Constrictive Pericarditis.

We describe an extracardiac mass in a 72-year-old man with a history of pericardiectomy for constrictive pericarditis. Imaging studies revealed that progressive shrinkage of the residual pericardium compressed mitral valve anteriority, and fat replacement of the secondarily enlarged posterior space of the atrioventricular groove appeared as a space-occupying lesion. (Level of Difficulty: Intermediate.).

Anti-inflammatory properties of amniotic membrane patch following pericardiectomy for constrictive pericarditis.

BACKGROUND: Since constrictive pericarditis is most often idiopathic and the pathophysiology remains largely unknown, both the diagnosis and the treatment can be challenging. However, by definition, inflammatory processes are central to this disease process. Amniotic membrane patches have been shown to possess anti-inflammatory properties and are believed to be immune privileged. Due to these properties, amniotic membrane patches were applied intraoperatively in a complicated patient presenting with constrictive pericarditis. CASE PRESENTATION: A patient with a history of multiple cardiac sur

Microwave Device-Assisted Pericardiectomy for Immunoglobulin G4-Related Constrictive Pericarditis: A Case Report.

INTRODUCTION: Immunoglobulin G4 (IgG4)-related constrictive pericarditis (CP) is a rare condition with no standardized treatment. Surgical pericardiectomy is challenging due to dense adhesions and the risk of injury to the phrenic nerves and heart. Conventional energy devices carry risks of thermal damage. We report a novel application of a microwave ablation device (Acrosurg; Nikkiso, Tokyo, Japan) for radical pericardiectomy, highlighting its advantages in preventing nerve injury and ensuring hemostatic control. CASE PRESENTATION: A 79-year-old male patient with recurrent heart failure due

Model for end-stage liver disease predicts mortality after pericardiectomy for constrictive pericarditis.

OBJECTIVES: Prognosis after pericardiectomy for constrictive pericarditis (CP) is affected by the aetiology of the constriction as well as by concomitant cardiac and non-cardiac disease, including liver dysfunction. However, few data exist on the risk stratification that accounts for liver function in patients with CP. We evaluated the effectiveness of the model for end-stage liver disease (MELD) score, an established measure of liver dysfunction, in predicting long-term survival and identifying other risk factors for death. METHODS: A total of 79 patients who underwent pericardiectomy for CP

The Hemodynamic Reversal of Delayed-Onset Radiation-Induced Constrictive Pericarditis Obtained by Pericardiectomy.

Constrictive pericarditis (CP) is often underdiagnosed, partly because of the lack of established diagnostic criteria. Exaggerated ventricular interdependence during inspiration is a hallmark physiological feature of CP. We herein report the case of a 55-year-old patient with CP who demonstrated a hemodynamic improvement following pericardiectomy. This case is characterized by two features: (1) delayed-onset CP 36 years after undergoin thoracic radiotherapy, which developed without pericardial calcification in contrast to severe aortic calcification, and (2) a postoperative improvement of insp

Early experience with pericardiectomy for constrictive pericarditis in Ile-Ife, Nigeria: a retrospective analysis.

BACKGROUND: Constrictive pericarditis is a rare but important cause of diastolic heart failure. Its uniqueness lies in the fact that surgery (pericardiectomy) remains the mainstay of treatment. Globally, the leading causes of constrictive pericarditis include idiopathic, mediastinal irradiation, post cardiac surgery and tuberculosis. METHODS: Patients who underwent pericardiectomy at our tertiary hospital between January 2019 and December 2024 were retrospectively studied with the aim of discussing our experience with the procedure and its outcomes. Data including baseline demographics, preop

Combinational elastography prior to pericardiectomy to assess liver condition in patients with constrictive pericarditis.

UNLABELLED: A 68-year-old man was diagnosed with pericarditis associated with immunoglobulin G4-related disease and was administered prednisolone 2 years prior to presentation. During the process of tapering off from prednisolone 1 year later, edema of the lower legs and pleural effusion worsened. He gradually developed dyspnea on exertion, and laboratory examinations revealed elevated liver enzyme levels. Diuretics were administered; however, the symptoms did not resolve. Transthoracic echocardiography and cardiac catheterization revealed findings consistent with those of constrictive pericar

Constrictive Pericarditis With Massive Calcification in Systemic Sclerosis Treated With Pericardiectomy: A Case Report.

Calcific constrictive pericarditis is a very rare complication of systemic sclerosis. This is the first report of surgically treated calcific constrictive pericarditis in systemic sclerosis. A 53 years-old woman, affected by limited systemic sclerosis, had a diagnosis of calcific constrictive pericarditis. She had a medical history of congestive heart failure since 2022. The patient was treated with pericardiectomy. Via a median sternotomy, the pericardium was dissected and removed from the midline to the left phrenic nerve, thus freeing the heart. Three months after the pericardiectomy, there

The Importance of Complete Pericardiectomy and the Role of the Apical Suction Device in Chronic Constrictive Pericarditis.

BACKGROUND: The aim of this study was to analyze the preoperative attributes and clinical impacts of complete pericardiectomy in chronic constrictive pericarditis. METHODS: A total of 26 patients were treated from January 2001 to December 2013. The pericardium was resected as widely as possible. When excessive bleeding or hemodynamic instability occurred intraoperatively, a cardiopulmonary bypass (CPB; n=3, 11.5%) or an apical suction device (n=8, 30.8%) was used. Patients were divided into 2 groups: those who underwent ≥ 80% resection of the pericardium (group A, n=18) and those who underwen

[Prominent mitral L wave in a patient with partial pericardiectomy: a possible new etiology].

The middiastolic flow of mitral L wave may result from pathologies that impair the diastolic function of the heart. Echocardiographic examination of a 48-year-old female patient with a three-year history of partial pericardiectomy showed mild left ventricular hypertrophy, mild mitral regurgitation, and mitral annular calcification. During pulse-Doppler examination, a prominent forward transmitral flow (mitral L wave) was noted. The patient developed supraventricular tachycardia attacks on simultaneous electrocardiographic monitoring, during which the mitral L wave disappeared, but E and A wave

Rheumatoid arthritis as a cause of cardiac compression. Favourable long-term outcome of pericardiectomy.

In order to clarify the significance of rheumatoid arthritis (RA) as a cause of cardiac compression, we scrutinized pericardiectomy files of 47 patients over a ten-year period at two university hospitals in Finland. Five patients with RA were found. All the patients with RA were men with seropositive disease and subcutaneous rheumatoid nodules. Two of the patients had pulmonary fibrosis, one had cutaneous vasculitis and three had had rheumatoid pleurisy. There was a mean delay of 10 months from the first cardiac symptom to the diagnosis of cardiac compression, the most common misdiagnosis bein
