Neonatal networks: clinical research and quality improvement.

Worldwide, neonatal networks have been formed to address both the research and quality improvement agenda of neonatal-perinatal medicine. Neonatal research networks have led the way in conducting many of the most important clinical trials of the last 25 years, including studies of cooling for hypoxic-ischemic encephalopathy, delivery room management with less invasive support, and oxygen saturation targeting. As we move into the future, increasing numbers of these networks are tackling quality improvement initiatives as a priority of their collaboration. Neonatal quality improvement networks h

A precarious pregnancy involving a patient with a large placental chorioangioma: a case report.

Chorioangiomas are common placental neoplasms that are primarily asymptomatic, but the larger subtypes have a variable presentation and can result in severe pregnancy complications. We report a case of a pregnant patient who presented with a 10 x 12 x 10 cm placental chorioangioma at 32 weeks, despite an unremarkable ultrasound at 28 weeks. Since these more substantial chorioangiomas can induce fetal mortality, close surveillance with perinatology and neonatology consultation is necessary to enhance the probability of a favorable patient prognosis.

Advances in perinatal care: 1970--1980.

Although recognition of the neonate as an entity deserving special consideration was slow at first, the rise of perinatology to the rank of subspecialty has occurred swiftly over the last 25 years. Considerable improvements in both the quantity and quality of perinatal survival have resulted. The more significant recent advances in selected areas (hemolytic diseases, hyperbilirubinemia, maternal diabetes, hyaline membrane disease, nutrition of the tiny premature neonate, infections, monitoring and mother-child interaction) are discussed, and speculations are made about the next five years.

Current knowledge on fertility-sparing treatment of cervical cancer patients.

INTRODUCTION: Cervical cancer directly affects the reproductive function of women of childbearing age. Up to one-third of the disease is dia-gnosed in women younger than 45 years of age, who may still have open reproductive plans. Therefore, in recent years, there has been increasing interest in performing fertility-sparing treatment (FST), which aims to preserve fertility while achieving the same oncologic safety as radical surgical treatment. OBJECTIVE: To summarize the current knowledge on FST in women with early stages of cervical cancer. METHODS: Review of published literature on the to

An update and review of nontubal ectopic pregnancy management.

PURPOSE OF REVIEW: Nontubal ectopic pregnancies appear to be increasing in prevalence. Increasingly, minimally invasive methods for management are being utilized. A current literature review and recommendations for management of nontubal ectopic pregnancy is presented in this review. RECENT FINDINGS: Nontubal ectopic pregnancies are less common than tubal ectopic pregnancies but present a unique and significant threat to patient's health and are optimally managed by specialists familiar with the condition. Early diagnosis, prompt treatment and close follow-up to resolution are critical. Recen

A principled approach to mediation analysis in perinatal epidemiology.

For many research questions in perinatal epidemiology, gestational age is a mediator that features the causal pathway between exposure and outcome. A mediator is an intermediate variable between an exposure and outcome, which is influenced by the exposure on the causal pathway to the outcome. Therefore, conventional analyses that adjust, stratify, or match for gestational age or its proxy (eg, preterm vs term deliveries) are problematic. This practice, which is entrenched in perinatal research, induces an overadjustment bias. Depending on the causal question, it may be inappropriate to adjust

Polish Gynecological Society Recommendations for Labor Induction.

Labor induction involves artificial stimulation of childbirth before the natural, spontaneous onset of labor. It is one of the most common procedures in modern obstetrics. The frequency of labor induction has doubled in recent decades due to the development of perinatology and methods of monitoring fetal well-being in particular. Currently, one in five pregnant women and 30-40% of women delivering vaginally undergo this procedure. Reasons for induction include reduction of the perinatal mortality and morbidity of the fetus and newborn, as well as the reduction of maternal complications. Howeve

[Management of mono-amnion twin pregnancy].

There have been only a few reports on monoamniotic twin pregnancies from the period of modern perinatology. Consequently, available recommendations about the management of these high-risk pregnancies are uncertain. In this study, we present the management and the outcome of three monoamniotic twin pregnancies and two further pregnancies with acardiac acephalus twin in comparison to other case reports and case series from the literature. Practical ways of caring for these high-risk pregnancies are outlined, emphasising the continuous search for entanglement of umbilical cord by ultrasound and c

Clinical and microbiological characteristics of hospital infections in the neonatal intensive care unit.

Neonates hospitalized in intensive care units, are exposed to a higher risk of infectious complications. The research involved 52 neonates hospitalized in the Neonatal Intensive Care Unit (NICU), Chair and Clinic of Obstetrics and Perinatology over a span of one year. The incidence of hospital infections as well as etiological factors were analyzed. Clinically manifested hospital infections were diagnosed in 38.5% of babies with very low or extremely low birth weight, in boys twice as often as in girls. Generalised invasive infections prevailed; in most cases they were caused by Gram-negative
